Approve and authorize the Health Services Director, or his designee (Donna Wigand) to
execute on behalf of the County, Contract Amendment Agreement #24-939-39(9) with
Kiran U. Koka, M.D., a self-employed individual, effective April 1, 2009, to amend
Contract #24-939-39(8), to increase the payment limit by $45,000, from $90,000 to a new
payment limit of $135,000, with no change in the original term of July 1, 2007 through June
30, 2009.
FISCAL IMPACT:
This Contract is funded 100% by Medi-Cal Funds, offset 50% State and 50% Federal.
BACKGROUND:
On June 5, 2007 the Board of Supervisors approved Contract #24-939-39(7) [as amended
by Contract Amendment Agreement #24-939-39(8)] with Kiran U. Koka, M.D., for the
period from July 1, 2007 through June 30, 2009, to provide Medi-Cal specialty mental
health services. At the time of negotiations, the payment limit was based on target levels of
utilization. However, the utilization during the term of the agreement was higher than
originally anticipated. Approval of Contract Amendment Agreement #24-939-39(9) will
