Approve and authorize the Health Services Director, or his designee (Wendel Brunner,
MD), to execute on behalf of the County, Contract Agreement #28 507 (#09–533) with the
First 5 Contra Costa Children and Families Commission, to pay County an amount not to
exceed $50,000, for provision of the West County First 5 Public Health Liaison (PHL) and
Resource Coordinator Program, for the period from January 1, 2009 through June 30, 2009.
FISCAL IMPACT:
This agreement will result in $50,000 from the First 5 Contra Costa Children and Families
Commission for the West County First 5 Public Health Liaison (PHL) Resource
Coordinator Program. No County match required.
BACKGROUND:
First 5 Public Health Liaison (PHL) will provide medical consultation services and resource
information to home visiting providers serving children prenatal to five years of age
residing in West Contra Costa County. Programs eligible to receive medical consultation
services are those programs without medical expertise on staff, and/or who have limited or
no access to

BACKGROUND: (CONT'D)
medical consultation and/or other medical resources. First 5 PHL will report to Public
Health Clinic Services (PHCS). Approval of Contract #28–507 will allow First 5 PHL to
serve as a liaison between County Health Services and Home Visiting Programs/ Home
Visitors and other West County Service Providers through June 30, 2009, including mutual
indemnification to hold harmless both parties for any claims arising out of the performance
of this Contact.
Three certified sealed copies of the Board Order should be returned to the Contracts and
Grants Unit.
