Cancellation Agreement #26 975-4 with Mark Van Handel, M.D. (Specialty: Neurology), a self-employed individual, effective on the close of business on May 31, 2009; and 2. Contract #26-975-5 with Mark Van Handel, M.D. (Specialty: Neurology), a self-employed individual, in an amount not to exceed $1,050,000 for the provision of professional neurology services for patients at Contra Costa Regional Medical Center and Contra Costa Health Centers, for the period from June 1, 2009 through May 31, 2012. FISCAL IMPACT: This Contract is funded by Enterprise Fund I. Cost to the County depends upon utilization. As appropriate, patients and/or third party payors will be billed for services. Twa, County Administrator and Clerk of the Board of Supervisors By: Celicia Nelson, Deputy cc: Tasha Scott, Barbara Borbon C.60 To:Board of Supervisors From:William Walker, M.D., Health Services Director Date:June 2, 2009 Contra Costa County Subject:Cancellation Agreement #26-975-4 and Contract #26-975-5 with Mark Van Handel, M.D. BACKGROUND: On July 11, 2006, the Board of Supervisors approved Contract #26-975-2 (as amended by Amendment Agreement #26-975-3) with Mark Van Handel, M.D., to provide neurology services including clinic coverage, consultation, training, on-call and medical and/or surgical procedures, for patients at Contra Costa Regional Medical Center and Contra Costa Health Centers, for the period from July 1, 2006 through June 30, 2009. In consideration of Contractor’s agreement to continue providing services, both parties have agreed to re-negotiate payment terms. This will allow the Contractor provide expanded duties due to leave of other neurologists. The Department and the Contractor have mutually agreed to renegotiate the Contract Payment Provisions and the term to reflect the intent of the parties. Therefore, in accordance with Special Conditions, Paragraph 2. (Modification to Termination and Cancellation) of the Contract, the Department and Contractor have agreed to a mutual cancellation of Contract #26-975-3. Approval of Cancellation Agreement #26-975-4 will accomplish this termination. Under Contract #26-975-5, Contractor will provide professional neurology services, including, clinic coverage, consultation, training, on-call services, and medical and/or surgical procedures, through May 31, 2012.
