Approve and authorize the Health Services Director, or his designee (Jeff Smith, M.D.) to
issue a thirty (30) day advance written notice to the Regents of the University of California,
on behalf of the University of California, San Francisco (UCSF), an educational institution,
to terminate Contract #26-495-4 for provision of vascular surgery services at Contra Costa
Regional Medical Center and Contra Costa Health Centers, effective close of business on
June 19, 2009.
FISCAL IMPACT:
This Contract was funded 100% by Enterprise I Funds.
BACKGROUND:
On May 8, 2007, the Board of Supervisors approved Contract #26-495-4 with the Regents
of the University of California, on behalf of the University of California, San Francisco
(UCSF), for the period from February 1, 2007 through January 31, 2010, for the provision
of vascular surgery services at Contra Costa Regional Medical Center and Contra Costa
Health Centers.

#26-912-9 Special Conditions, Paragraph 2. (Modification to Termination and
Cancellation), that the Contract is terminated effective close of business on June 19, 2009.
