Subject:Contract Amendment Agreement #26-856-26 with Diablo Pulmonary Medical Group
BACKGROUND: (CONT'D)
Centers, for the period from November 1, 2008 through October 31, 2011.
Due to the mutual mistake of the County and the Contractor, the Contract payment
provisions did not accurately reflect the intent of the parties, which was that the Contractor
be paid at a rate of $14,400 for provision of sixteen (16) clinic session instead of the
previously stated $14,000.
Approval of Contract Amendment Agreement #26-856-26 will modify the Contract
Payment Provisions, to make the formal Contract consistent with the oral agreement, which
was negotiated with the Contractor and allow the Contractor to continue providing services
through October 31, 2011.
