Approve and authorize the Health Services Director, or his designee (Sherman Quinlan) to
execute on behalf of the County, Contract Amendment/Extension Agreement #22–871–7
with Decade Software Company, LLC, a Limited Liability Company, effective March 1,
2009, to amend Contract #22–871 (as amended by Contract Amendment/Extension
Agreements #22–871–1 through #22–871–6) to increase the payment limit by $72,000,
from $290,620 to a new payment limit of $362,620 and extend the term from April 30,
2009 through April 30, 2010.
FISCAL IMPACT:
This Contract is funded 100% by Environmental Health program fees.
BACKGROUND:
On June 3, 2003 the Board of Supervisors approved Contract #22–871 (as amended by
Contract Amendment/Extension Agreements #22–871–1 through #22–871–6) with Decade
Software Company, LLC, for the period from July 1, 2003 through April 30, 2009,

for the provision of a personal computer-based data management system, including
Envision for Windows, Press-Agent, and other required software. Approval of Contract

providing software licenses and support services for an additional term, through April 30,
2010.
