Jornlin, Director Contra Social Service Department Costa DATE: October 9, 1984 County SUBJECT: APPROVE MULTIPURPOSE SENIOR SERVICES PROPOSAL SPECIFIC REQUEST(S) OR RECOMMENDATION(S) Q BACKGROUND AND JUSTIFICATION RECOMMENDATION Authorize the Social Service Director to submit a proposal for operation of a case management site for the Multipurpose Senior Services Program (MSSP) to the State of California Health and Welfare Agency to provide public/private case management services to frail elderly to prevent or delay long term care institutionalization and to provide for the return to the community of elderly persons already imsitutionalized for the period February 1, 1985 to June 30, 1986 and requiring $968,327 in State and Federal Title XIX funds. BACKGROUND On September 14, 1984, the Office on Aging received a Request For Proposal (RFP) from the Health and Welfare Agency. The purpose of this RFP is to solicit responses from local governments and private non-Drofit corporations or a coalition of both to plan, develop, establish, administer funds for, and operate case management sites for the Multipurpose Senior Services Program (MSSP) of the California Health and Welfare Agency (HWA) , for the period February 1, 1985 through June 30, 1986 in the amount of $968,327. The State is particularly interested in selecting an array of sites which will serve geographic areas not presently served by the eight existing MSSP sites. The MSSP is a social/health case management program designed to prevent or delay long term care institutionalization of the frail elderly and to provide for the return to the community of elderly persons already institutionalized. It is limited to serving Medi-Cal eligible persons 65 years of age and older. The MSSP is designed to provide cost-effective alternatives to long-term institutional care. Cost-effectiveness is defined .as the Program's demonstrated ability to provide community-based care for clients at an average monthly public cost of skilled nursing facility (SNF) or intermediate care facility (ICF) level placements for comparable individuals. The MSSP is a result of a four year federally funded Medicaid research project in California designed to obtain information on ways to prevent unnecessary institutionalization of the frail , at-risk elderly. California also augments the Program's Title XIX funds with an_ appropriation from the State general revenues. Under the three year 2176 Federal waiver and with the Legislature's authoriza- tion, the MSSP was able to serve a maximum caseload of 1,900 clients during the first year (FY 1983-84) of the Program and will serve a total caseload of 3,400 clients during the second and current year (FY 1984-85). If the Program con- tinues to prove cost-effective in the Judgement of the Legislature, the HWA may be authorized to serve a maximum of 5,400 clients during the third year (FY 1985-86) of the Federal waiver. In the current year it is intended that the growth of the caseload from 1,900 to 3,400 clients will be accomplished through the expansion of the existing eight sites and the designation of as many as ten new sites. These new sites could be very similar to the existing MSSP sites or, given acceptable justifica- tions and compliance with the provisions of the program's existing waiver, variations from the current MSSP design. To the extent possible, MSSP Contractors must operate the site as an administratively separate unit. It is assumed that each of the ten new sites would be capable of acquiring and serving a caseload of 200 clients by June 30, 1986. Expansion in subsequent fiscal years is an option and contingent upon, among other factors, Federal and legislative approval for an additional three years. Upon approval , this proposal will be submitted by the Social Service Department as the lead agency on behalf of the public and private agencies who have been participating in the Contra Costa County Long Term Care Pilot Project. These agencies include the County Social Service Department/Office on Aging, County Health Services Department, Home Health and Counseling, John Muir Home Care, Mt. Diablo Rehabilitation Center, Diablo Valley Foundation for the Aging/Support Services for Elders, and Upjohn HealthCare Services. The proposal being drafted reflects an attempt to continue the public/private design of our pilot project within the mandated specifications of MSSP. SJ/dc 001.37
