(100% AOB Programs)   C. 2 APPROVE and AUTHORIZE the Chair, Board of Supervisors, to execute the Correction to Grant Deed recorded on December 15, 1989, in Book 15543 at Page 103 with Central Contra Costa Sanitary District, for their sanitary sewer pipelines and related appurtenances located within the Iron Horse Corridor, Alamo area. (Budgeted, 100% Federal, No County match)   October 23, 2012 Contra Costa County Board of Supervisors 9 APPROVE and AUTHORIZE execution of agreement between the County and the following parties as noted for the purchase of equipment and/or services:   C. 44 APPROVE and AUTHORIZE the Employment and Human Services Interim Director, or designee, to execute a contract amendment, effective November 1, 2012, with Intelegy Corporation to provide strategic planning, onsite and offsite consultation, project management, and Office of the Future intake training; increase the amount payable by $223,285, for a new total amount not to exceed $595,015; and extending the contract term to June 30, 2013. (100% Enterprise Fund I)   C. 50 APPROVE and AUTHORIZE the Health Services Director, or designee, to October 23, 2012 Contra Costa County Board of Supervisors 10 C. 50 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract with La Clinica de la Raza, Inc. in an amount not to exceed $375,000, to provide mental health services to severely emotionally disturbed children in east Contra Costa County, for the period October 1, 2012 through June 30, 2013, with a six-month automatic extension through December 31, 2013, in an amount not to exceed $250,000. (50% Mental Health Realignment and 50% Federal Financial Participation)   C. 51 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract amendment with On Assignment Staffing Services, Inc., effective September 1, 2012, to modify the rate schedule to add additional temporary help classification of orthopedic technician, with no change in the original payment limit of $6,307,600 and no change in the original term of June 1, 2012 through May 31, 2013. (100% Enterprise Fund I)   C. 52 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a cancellation agreement with Dominic Ciotola (dba CPR Training Center) for his existing contract, effective at close of business on July 31, 2012; and to execute a new contract in an amount not to exceed $160,000 to provide cardiopulmonary resuscitation training courses for Contra Costa Regional Medical and Health Centers, for the period August 1, 2012 through June 30, 2015. (100% Enterprise Fund I)   C. 53 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract with Sodexo America, LLC in an amount not to exceed $284,060, to provide management and oversight of the Food and Nutrition Services Unit at Contra Costa Regional Medical and Health Centers, for the period November 1, 2012 through October 31, 2013.
