a consulting services contract with DKS Associates, in an amount not to exceed
$650,000 to provide transportation engineering services for the Area of Benefit
(AOB) Program Updates project, for the period October 23, 2012 through
October 23, 2015, Countywide. (100% AOB Programs)
C. 2 APPROVE and AUTHORIZE the Chair, Board of Supervisors, to execute the
Correction to Grant Deed recorded on December 15, 1989, in Book 15543 at
Page 103 with Central Contra Costa Sanitary District, for their sanitary sewer
pipelines and related appurtenances located within the Iron Horse Corridor,
Alamo area. (100% Iron Horse Corridor Funds)
C. 3 APPROVE and AUTHORIZE the Chair, Board of Supervisors, or designee, to
execute the Fifth Amendment to Agreement No. 208 to provide right of way
services by Contra Costa County (County) to the Contra Costa Transportation
Authority (CCTA) for the State Route 4 East Widening Somersville Road to
State Route 160 Project to increase the payment limit by $500,000 to a new
payment limit of $6,778,000, effective September 19, 2012, and to amend the
scope of work for services provided by the County to CCTA for State Route 4
East Widening Somersville Road to State Route 160 Project, East County area.

C. 7 ADOPT Resolution No. 2012/450 approving and authorizing the Public Works

Director, or designee, to fully close a portion of Jackson Way and the north

Annual Alamo Tree Lighting Ceremony, Alamo area. (No Fiscal Impact)
C. 8 APPROVE and AUTHORIZE the Public Works Director, or designee, to execute
Amendment No. 1, effective March 15, 2012 for an existing On-Call Contract
with Entrix, Inc., d/b/a Cardno Entrix, to add one additional sub-contractor and
incorporate new DBE language per Caltrans requirements as defined in the
Amendment, Countywide. (48% Flood Control & Water Conservation District,

on behalf of the County a Consent to Assignment of Lease between the County

C. 10 APPROVE and AUTHORIZE the Director of Airports, or designee, to execute a
month-to-month hangar rental agreement with Concord Flying Club for a shade

C. 11 APPROVE and AUTHORIZE the Director of Airports, or designee, to execute a
month-to-month hangar rental agreement with John Cicero and Raymond Nilson

between the District and the City of Antioch, in an amount not to exceed

APPROVE and AUTHORIZE execution of agreements between the County and the
following agencies for receipt of fund and/or services:

a grant in the amount of $1,500 from LEGO Children’s Fund to provide books,

C. 38 APPROVE and AUTHORIZE the Health Services Director, to accept on behalf
of the County, a grant award from Kaiser Permanente in the amount of $45,000,
for the Health Access For Kids Project, for the period from November 1, 2012
through November 1, 2013. (No County match)
C. 39 APPROVE and AUTHORIZE the Health Services Director, to accept on behalf
of the County, Grant Award from Kaiser Permanente in an the amount of
$25,000, for the Prenatal Care Now Project in the East and Central County

C. 40 APPROVE and AUTHORIZE the Health Services Director, or his designee, to
execute a contract amendment with the California Department of Public Health,
effective July 1, 2011, to make technical adjustments to the agreement and to

County in an amount to exceed $3,091,582, for the County’s AIDS Programs,
with no change in the original term from July 1, 2010 through June 30, 2013.

execute a contract amendment with the State of California, Managed Risk

$4,310,485 to a new total of $34,202,678, to make technical adjustments to the
agreement, and to extend the term through September 30, 2013. (No County

C. 42 APPROVE and AUTHORIZE the Employment & Human
Services Interim Director, or designee, to accept funding from the California
Department of Education for facilities renovation and repair in an amount not to

Interim Director, or designee, to execute a contract amendment with California
Department of Community Services and Development, including modified

Program by increasing the payment limit by $30,655 to a new total not to exceed

APPROVE and AUTHORIZE execution of agreement between the County and the
following parties as noted for the purchase of equipment and/or services:
C. 44 APPROVE and AUTHORIZE the Employment and Human Services Interim
Director, or designee, to execute a contract amendment, effective November 1,
2012, with Intelegy Corporation to provide strategic planning, onsite and offsite
consultation, project management, and Office of the Future intake training;
increase the amount payable by $223,285, for a new total amount not to exceed
$595,015; and extending the contract term to June 30, 2013. (10% County, 45%
State, 45% Federal)
C. 45 APPROVE and AUTHORIZE the Purchasing Agent or designee on behalf of the
Public Works Director to execute an increase to the payment limit of Blanket
Purchase Order 37882 with JC Paper Company for carbonless forms and
recycled copy paper by $100,000, to an amount of $199,000, to meet demand for
the period of June 01, 2012 through June 30, 2013. (100% Interdepartmental

C. 46 APPROVE and AUTHORIZE the Purchasing Agent or designee, on behalf of
the Public Works Director to execute an increase to the payment limit of Blanket
Purchase Order 39273 with California Envelope Company (CEC) for envelopes
by $100,000, to an amount of $199,000, to meet demand for the period October
01, 2011 through September 30, 2013, Countywide. (100% General Fund)
C. 47 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract amendment with Robert Half International, Inc., effective
August 20, 2012, to increase the payment limit by $750,000 to a new payment
limit of $2,000,000 to provide additional temporary staffing services for
specialized consulting and computer programming, with no change in the original
term of July 1, 2011 through December 31, 2012. (100% American Recovery &

C. 48 APPROVE Change Order No. 12 in the amount of $788,493 with Vila
Construction Company, Inc. for the Remodeling of 30 and 40 Muir Road,
Martinez, for the Department of Conservation and Development; and
AUTHORIZE the Public Works Director, or designee, to execute the Change

C. 49 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Dino Elyassnia, M.D., A Professional Corporation, in an
amount not to exceed $480,000, to provide plastic and hand surgery services at
Contra Costa Regional Medical and Health Centers, for the period November 1,
2012 through October 31, 2014. (100% Enterprise Fund I)
C. 50 APPROVE and AUTHORIZE the Health Services Director, or designee, to

C. 50 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with La Clinica de la Raza, Inc. in an amount not to exceed
$375,000, to provide mental health services to severely emotionally disturbed
children in east Contra Costa County, for the period October 1, 2012 through
June 30, 2013, with a six-month automatic extension through December 31,
2013, in an amount not to exceed $250,000. (50% Mental Health Realignment
and 50% Federal Financial Participation)
C. 51 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract amendment with On Assignment Staffing Services, Inc.,
effective September 1, 2012, to modify the rate schedule to add additional
temporary help classification of orthopedic technician, with no change in the
original payment limit of $6,307,600 and no change in the original term of June
1, 2012 through May 31, 2013. (100% Enterprise Fund I)
C. 52 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a cancellation agreement with Dominic Ciotola (dba CPR Training
Center) for his existing contract, effective at close of business on July 31, 2012;

execute a contract with Sodexo America, LLC in an amount not to exceed
$284,060, to provide management and oversight of the Food and Nutrition
Services Unit at Contra Costa Regional Medical and Health Centers, for the
period November 1, 2012 through October 31, 2013. (100% Enterprise Fund I)
C. 54 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract amendment with Credentials Services, Inc., effective
September 30, 2012, to increase the payment limit by $60,000 to a new payment
limit of $120,000, to provide additional credentialing services for healthcare
providers, and extend the term from September 30, 2012 through September 30,
2014. (100% Contra Costa Health Plan member premiums)
C. 55 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Santa Rosa Consulting, Inc., in an amount not to exceed
$800,000, to provide consultation and technical assistance with regard to the
Department’s Information Systems, for the period September 1, 2012 through
August 31, 2013. (100% Enterprise Fund I)
C. 56 APPROVE and AUTHORIZE the Health Services Director, or designee, to

C. 56 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract amendment with Ujima Family Recovery Services, effective
June 1, 2012, to increase the payment limit by $414,000 to a new payment limit
of $820,031 to provide additional CalWORKS assessment, drug and alcohol
treatment services, and group and individual counseling, with no change in the
original term of March 1, 2012 through June 30, 2013, and to increase the
automatic extension payment limit by $207,000 to a new payment limit of
$357,000, with no change in the term of the automatic extension, through
December 31, 2013. (59% by CalWORKs Alcohol and Other Drug Services,
29% CalWORKs Mental Health and 12% Substance Abuse Prevention and
Treatment Federal Block Grant)
C. 57 APPROVE and AUTHORIZE the Purchasing Agent, or designee, to execute a
purchase order, on behalf of the Chief Information Officer, with IBM
Corporation, in an amount not to exceed $200,000 for maintenance and support
of County mainframe computer software for the period January 1, 2013 through
December 31, 2013. (100% Department User fees)
C. 58 APPROVE and AUTHORIZE the Purchasing Agent, or designee, to execute a
purchase order amendment, on behalf of the Chief Information Officer, with IBM
Corporation, to increase the payment limit by $26,400 to a new payment limit of
$191,400 for maintenance on software applications used on the County's
mainframe computer system, with no change in the original term of July 1, 2011
through December 31, 2012. (100% Department User Fees)

C. 59 APPROVE the design and bid documents, including plans and specifications, for
the Tree Restoration Project at the West County Detention Facility, 5555 Giant
Highway, Richmond; and AUTHORIZE the Director of Public Works, or
designee, to solicit bids to be received on or about December 4, 2012, and to
issue bid addenda, as needed, for clarification of the contract bid documents.

C. 60 ACCEPT the September 2012 update on the operations of the Employment and
Human Services Department, Community Services Bureau, as recommended by
the Employment and Human Services Department Interim Director.
C. 61 APPROVE and AUTHORIZE the County Treasurer-Tax Collector to execute a
Software License Agreement with Columbia Ultimate, Inc. in an amount not to
exceed $25,000, for the use of software for delinquent property tax collection, for
the period of October 16, 2012 to October 15, 2013, with automatic one-year
renewal periods thereafter. (100% General Fund).
C. 62 DECLARE as surplus and AUTHORIZE the Purchasing Agent, or designee, to

dispose of fully depreciated vehicles no longer needed for public use as
recommended by the Public Works Director, Countywide. (No Fiscal Impact)
C. 63 ADOPT Resolution No. 2012/448 authorizing the sale of specified tax-defaulted
property at public auction, pursuant to the California Revenue and Taxation
Code ("R&T") §3698, as recommended by the Treasurer-Tax Collector.
C. 64 ACCEPT the Contra Costa County Fish and Wildlife Committee 2011 Annual
Report, as recommended by the Fish and Wildlife Committee. (No fiscal impact)
C. 65 APPROVE the Notice of Intention to Convey Real Property to the City of
Richmond, as recommended by the Public Works Director, Richmond area. (No
Fiscal Impact)
C. 66 ACCEPT the annual report on the activities and achievements of the Service
Integration Program in the Employment and Human Services Department, as
recommended by the Family and Human Services Committee.
C. 67 ACCEPT the report prepared by the Office of the Sheriff in accordance with
Penal Code Section 4025(e) representing an accounting of all Inmate Welfare
Fund receipts and disbursements for Fiscal Year 2011/12, as recommended by
the Sheriff-Coroner. (No fiscal impact)
C. 68 ACCEPT the annual report from the Employment and Human Services
Department on the activities of the Community Services Bureau, as
recommended by the Family and Human Services Committee.
C. 69 ACCEPT annual report from the Employment and Human Services Department,
acknowledge the services provided to disabled, frail and elderly residents, and
consider providing, when discretionary funds become available, additional
funding for this program, as recommended by the Family and Human Services
Committee.

The Board meets in all its capacities pursuant to Ordinance Code Section 24-2.402, including as
the Housing Authority and the Successor Agency to the Redevelopment Agency. Persons who
wish to address the Board should complete the form provided for that purpose and furnish a copy
of any written statement to the Clerk.
Any disclosable public records related to an open session item on a regular meeting agenda and
distributed by the Clerk of the Board to a majority of the members of the Board of Supervisors less
than 72 hours prior to that meeting are available for public inspection at 651 Pine Street, First
Floor, Room 106, Martinez, CA 94553, during normal business hours.
All matters listed under CONSENT ITEMS are considered by the Board to be routine and will be
enacted by one motion. There will be no separate discussion of these items unless requested by a
member of the Board or a member of the public prior to the time the Board votes on the motion to

Persons who wish to speak on matters set for PUBLIC HEARINGS will be heard when the Chair
calls for comments from those persons who are in support thereof or in opposition thereto. After
