C. 2 AUTHORIZE expenditure of $20,000 on an annual basis for each fiscal year
from 2012/2013 through 2013/2014 from the Dougherty Valley Regional
Enhancement Fund to implement the San Ramon Valley Street Smarts Program;
AUTHORIZE the Public Works Director, or designee, to execute a
Memorandum of Understanding to implement a cost-sharing agreement,

Amendment No. 3, effective November 8, 2012, to an existing On-Call

C. 12 ADOPT Resolution No. 2012/439 for Land Use Permit 08-2034 and Minor
Subdivision 09-0008, accepting an Offer of Dedication for Roadway Purposes,
for a project being developed by Sufism Reoriented, as recommended by the
Public Works Director, Walnut Creek/Saranap area. (No Fiscal Impact)
C. 13 ADOPT Resolution No. 2012/440 accepting completion of warranty period and
release of cash deposit for faithful performance, for subdivision SD 04-08905, for
a project being developed by Davidon Homes, as recommended by the Public

C. 14 APPROVE and AUTHORIZE the Chief Engineer, Flood Control and Water
Conservation District, or designee, on behalf of the Contra Costa Clean Water
Program, to execute a Contract Amendment with Dan Cloak Environmental
Consulting, to increase the payment limit by $56,550, for a new total contract
amount not to exceed $301,500, for continuing compliance with mandated
federal and state stormwater rules contained in National Pollutant Discharge
Elimination System (NPDES) Permits issued by the San Francisco Bay and
Central Valley Regional Water Quality Control Boards for the period of August
1, 2012 through June 30, 2013, Countywide. (100% Cities and County
Stormwater Utility Fee Assessments)
C. 15 APPROVE and AUTHORIZE the Director of Airports, or designee, to execute a
month-to-month hangar rental agreement with Ronald Chris Franklin for a shade
hangar at Buchanan Field Airport effective September 15, 2012, in the monthly
amount of $172.41, Pacheco area. (100% Airport Enterprise Fund).
C. 16 APPROVE and AUTHORIZE the Director of Airports, or designee, to execute a

C. 18 ADOPT Resolution No. 2012/441 approving and authorizing the Chief Engineer,

Agreement with the California Department of Water Resources, accept funds,

and sign requests for disbursement under the Grant Agreement with the

the amount of $46,500 and appropriating it for the purchase of Dell Equalogic

APPROVE and AUTHORIZE execution of agreements between the County and the
following agencies for receipt of fund and/or services:
C. 57 APPROVE and AUTHORIZE the Agricultural Commissioner, or designee, to
execute Agreement #12-0368-SF with the State of California Department of
Food and Agriculture (CDFA) in an amount not to exceed $299,290, for Dog
Team inspection activities for the period of July 1, 2012 through June 30, 2013,
as recommended by the Agricultural Commissioner.
C. 58 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract amendment with the State of California, Department of Health
Care Services, effective October 1, 2012, to incorporate new language and
capitation rates for the addition of Community Based Adult Services, with no
change in the original term of April 1, 2005 through December 31, 2013 and no
change in the original payment limit of $317,472,000. (Budgeted, 100% State,
No County Match)
C. 59 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with the State of California, Tuberculosis Control Branch, to
pay the County, an amount not to exceed $254,373, for the period from July 1,
2012 through June 30, 2013, for the Tuberculosis Control Program, and
authorize the Purchasing Agent to issue payment(s) up to $14,884 of the contract
amount, to be used for food, shelter, incentives and enablers. (Budgeted, 100%
State, No County match)
C. 60 APPROVE and AUTHORIZE the Health Services Director, to accept on behalf
of the County, a grant from Kaiser Permanente, in the amount of $30,000, for the
East County Heal Project in Bay Point, Pittsburg and Antioch, for the period
from July 1, 2012 through June 30, 2013. (No County match)
C. 61 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with the State Department of State Hospitals, to pay the
County an amount not to exceed $1,397,741, for the Conditional Release
Program, for the period from July 1, 2012 through June 30, 2013 (Budgeted, No
County Match)

C. 62 APPROVE and AUTHORIZE the Sheriff-Coroner, or designee, to apply for and
accept the U.S. Department of Justice, 2012 Paul Coverdell Forensic Services
Improvement Grant, with the California Emergency Management Agency, as
fiscal agent, in an amount not to exceed $22,030 for support of forensic training
for the period October 1, 2012 through August 31, 2013. (100% Federal, No
County match)
C. 63 APPROVE and AUTHORIZE the Chief Information Officer, or his designee, to
execute an Integrated Data Management System (IDMS) Upgrade Support
Agreement with Computer Associates, Inc., in an amount not to exceed $80,000
to upgrade Computer Associate Inc.'s IDMS software and associated components
for the period of October 1, 2012 through September 30, 2013 (100%
Department User fees)
APPROVE and AUTHORIZE execution of agreement between the County and the
following parties as noted for the purchase of equipment and/or services:
C. 64 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a novation contract with Child Abuse Prevention Council of Contra
Costa County in an amount not to exceed $361,828, to provide Mental Health
