(100% Health Services Department Hospital Enterprise Fund)   Grants & Contracts   APPROVE and AUTHORIZE execution of agreements between the County and the following agencies for receipt of fund and/or services:   C. 38 APPROVE and AUTHORIZE the District Attorney, or designee, to submit an September 25, 2012 Contra Costa County Board of Supervisors 8 C. 38 APPROVE and AUTHORIZE the District Attorney, or designee, to submit an application and execute a grant award agreement, and any extensions or amendments thereof, pursuant to State guidelines, with the California Emergency Management Agency, Victim Services Branch, for funding of the Underserved Victim Advocacy and Outreach Program for the period October 1, 2012 through September 30, 2013 in the amount of $100,000. (75% State, 25% In-Kind match; Budgeted)   C. 39 APPROVE and AUTHORIZE the Conservation and Development Director, or designee, to execute a subcontract agreement with Quantum Energy Services and Technologies, Inc. (QuEST Inc.) to pay the County grant funding in the amount of $150,000 from Pacific, Gas & Electric, with QuEST Inc. acting as fiscal agent, to provide marketing, educational programs and outreach for the Whole House Rebate Performance Program for the period August 1 through December 31, 2012. (100% PG&E Grant funding)   C. 40 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract with the California Department of Public Health, Network for a Healthy California, in an amount payable to the County, not to exceed $3,754,665, for the “California Nutrition Network” Project, for the period from October 1, 2012 through September 30, 2016. (100% State, No County match; Budgeted)   APPROVE and AUTHORIZE execution of agreement between the County and the following parties as noted for the purchase of equipment and/or services:   C. 41 APPROVE and AUTHORIZE the Employment and Human Services Interim Director, or designee, to execute a contract amendment with Larry Digman, a self-employed individual, to increase the payment limit by $81,600 to a new payment limit not to exceed $326,400 for data warehouse analyst services, and extend the term from September 30, 2012 through March 31, 2013. (10% County, 45% State, 45% Federal)   C. 42 APPROVE and AUTHORIZE the Public Works Director, or designee, to execute a Consulting Services Agreement with Carey & Co. Inc. in an amount not to exceed $320,000 to provide architectural services for the Exterior Renovations Project at 625 Court Street, Martinez. (100% General Fund)   C. 43 APPROVE Amendment No. 1 in the amount of $52,585 with Cleary Bros. Landscape, Inc. ("Cleary Bros.") for the Iron Horse Corridor Landscaping at Walden Green, Phase 2, Between Mayhew Way and Coggins Drive, Pleasant Hill, for the Department of Conservation and Development and AUTHORIZE the Public Works Director, or designee, to execute the amendment which provides for additional work and increases the payment limit by $52,585 for a total contract amount of $588,604, Pleasant Hill area. (100% Redevelopment Reserves)   C. 44 APPROVE and AUTHORIZE the Employment and Human Services Interim September 25, 2012 Contra Costa County Board of Supervisors 9 C. 44 APPROVE and AUTHORIZE the Employment and Human Services Interim Director, or designee, to execute a contract amendment with STAND! For Families Free of Violence to change the payment provisions from fee basis to cost basis with no change to the payment limit of $162,690 and no change to the contract term of July 1, 2012 to June 30, 2013. (29% County General Fund; 71% Marriage License Fee)   C. 45 APPROVE AND AUTHORIZE the Chief Information Officer, or designee, to execute a contract with Keven J. Ormiston Consulting LLC, in an amount not to exceed $133,000 to provide Accela Automation Software, GIS Database and Application support for the period of October 1, 2012 through September 30, 2013. (100% GIS User fees)   C. 46 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract amendment with CJS Solutions Group, LLC, effective August 16, 2012, to increase the payment limit by $2,200,000 to a new payment limit of $6,925,000 to provide additional hours of consultation and technical assistance with regard to the implementation of the Epic Electronic Health Record System, with no change in the original term of July 1, 2011 through December 31, 2012. (100% Medicaid Electronic Health Records Incentive Program of the American Recovery and Reinvestment Act)   C. 47 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract with H. Michael Jaffin, M.D., in an amount not to exceed $160,000, to provide professional orthopedic surgery services for Contra Costa Health Plan members, for the period September 1, 2012 through August 31, 2014. (100% Members Premiums)   C. 48 APPROVE and AUTHORIZE the Purchasing Agent, or designee to execute a third party lease agreement with IBM Global Finance, in an amount not to exceed $71,598, for the purchase of a computer server and a UPS backup system, for the period of November 1, 2012 through May 1, 2017 as recommended by the Public Works Director; and APPROVE and AUTHORIZE the Purchasing Agent, to execute purchase orders on behalf of the Department of Child Support Services in an amount not to exceed $23,674 with Fluke Networks, for the purchase of a computer server and $43,938 for a UPS backup system, Countywide. (66% Federal and 34% State Funds)   C. 49 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract with AYA Healthcare, Inc., in an amount not to exceed $250,000 to provide temporary nursing staff for Contra Costa Regional Medical and Health Centers, for the period September 1, 2012 through August 31, 2013. (100% Enterprise Fund I)   Other Actions   C. 50 APPROVE the Second Amended 2012/2013 North Richmond Waste and Recovery Mitigation September 25, 2012 Contra Costa County Board of Supervisors 10 C. 50 APPROVE the Second Amended 2012/2013 North Richmond Waste and Recovery Mitigation Fee Expenditure Plan, which specifies intended activities and respective allocations of Mitigation Fee revenue through June 30, 2013, as recommended by the North Richmond Waste and Recovery Mitigation Fee Joint Expenditure Planning Committee.
