(100% Local Road Funds)   Engineering Services   C. 2 APPROVE the Camino Tassajara Shoulder Widening Project, East of Blackhawk Drive to Finley Road; and AUTHORIZE the Public Works Director, or designee, to advertise the project, and make related findings under the California Environmental Quality Act (CEQA), Danville area. (No Fiscal Impact)   Leases   C.11 APPROVE and AUTHORIZE the Public Works Director, or designee, to execute a lease with the Housing Authority of the County of Contra Costa for a five (5) year term, for approximately 5,000 square feet of classroom and office space located at 2 California Street, Rodeo, at an annual rental of $2,000 plus estimated miscellaneous occupancy charges of approximately $25,000 per year, as requested by the Employment and Human Services Department, Rodeo area. (100% General Funds)   Grants & Contracts   APPROVE and AUTHORIZE execution of agreements between the County and the following agencies for receipt of fund and/or services:   C.12 APPROVE and AUTHORIZE the District Attorney, or designee, to submit an application and execute a grant award agreement and any extensions or amendments thereof, pursuant to State guidelines, with the Board of State and Community Corrections in the amount of $354,515 for funding of the 2012 Regional Anti-Drug Abuse Program for the period October 1, 2012 through September 30, 2013. (100% State, No County match; Budgeted) September 18, 2012 Contra Costa County Board of Supervisors 4   C.13 APPROVE and AUTHORIZE the County Librarian, or designee, to apply for and accept a grant in the amount of $5,000 from East Bay Community Foundation, administered by the Rodeo Municipal Advisory Council, for Rodeo Library services, pursuant to the local refinery Good Neighbor Agreement for the period January 1, 2013 through June 30, 2013. (No Library fund match)   C.14 APPROVE and AUTHORIZE the County Probation Officer, or designee, to apply for and accept the Intensive Probation Supervision for High-Risk Felony & Repeat DUI Offenders grant from the California Office of Traffic Safety, in an amount not to exceed $153,000 for the period of October 1, 2012 through September 30, 2013. (100% State, No County match; Budgeted)   C.15 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract with the California Department of Public Health, to pay the County an amount not to exceed $1,530,861, for the 2012-2013 Centers for Disease Control and Prevention, Public Health Emergency Preparedness, State General Fund Pandemic Influenza, Hospital Preparedness Program & Comprehensive Program, for the period from July 1, 2012 through June 30, 2013. (100% State, No County match; Budgeted)   C.16 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract with the U. S. Department of Health and Human Services, Health Resources & Services Administration, to pay the County an amount not to exceed $326,368, for the Ryan White, Part C, HIV Early Intervention Services, for the period from July 1, 2012 through June 30, 2013. (100% Federal, No County match; Budgeted)   C.17 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract amendment with the State of California, Managed Risk Medical Insurance Board, effective October 1, 2012, to increase the amount payable to County by $659, to a new total payment of $10,174, make technical adjustments, and to extend the term of the Agreement from September 30, 2012 through September 30, 2013 for the Healthy Families Program. (100% State, No County match; Budgeted)   APPROVE and AUTHORIZE execution of agreement between the County and the following parties as noted for the purchase of equipment and/or services:   C.18 APPROVE and AUTHORIZE bid documents and advertisement for the Chiller Replacement Project at the Martinez Detention Facility, 1000 Ward Street, Martinez, on behalf of the Sheriff's Office, as recommended by the Public Works Director, Martinez area. (100% General Funds)   C.19 APPROVE and AUTHORIZE the County Probation Officer, or designee, to September 18, 2012 Contra Costa County Board of Supervisors 5 C.19 APPROVE and AUTHORIZE the County Probation Officer, or designee, to execute a contract with the County of Del Norte, including modified indemnification language, for the placement of wards at the Bar-O Boys Ranch at the rate of $3,500 per ward per month for the period July 1, 2012 through June 30, 2013. (100% General Fund; Budgeted)   C.20 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute novation contract with Early Childhood Mental Health Program, in an amount not to exceed $2,422,201, to provide specialized mental health services to children and their families in West County for the period July 1, 2012 through June 30, 2013, including a six-month automatic extension through December 31, 2013, in an amount not to exceed $909,426. (50% Federal Medi-Cal Revenue, 50% State Early and Periodic Screening, Diagnostics, and Treatment Program)   C.21 APPROVE and AUTHORIZE the Purchasing Agent or designee, to execute a purchase order amendment with Air Products Group to increase the payment limit by $95,000, to a new payment limit of $180,000 for HVAC air filtration components, for the period from November 1, 2010 through October 31, 2013. (100% General Fund)   C.22 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract with the Trustees of the California State University on behalf of San Francisco State University, to provide supervised field instruction at Contra Costa Regional Medical Center and Health Centers, for Contractor’s clinical laboratory science program, dietitian and registered nurse students, for the period September 1, 2012 through August 31, 2017. (No Fiscal Impact)   C.23 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract with Greater Richmond Inter-Faith Program in an amount not to exceed $420,000, to provide consultation and technical assistance to the Health Services Department’s Clinic Services program, for the period September 1, 2012 through August 31, 2013. (71% Federal Qualified Health Care Center funds, 7% US Department of Health and Human Services Teenage Pregnancy and Prevention grant, 22% Health Services Department Community Education and Information funds)   C.24 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract with St. Helena Hospital, in an amount not to exceed $400,000, to provide inpatient psychiatric hospital services for the period from July 1, 2012 through June 30, 2013. (100% Managed Care Allocation)   C.25 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract with Mt. Diablo Unified School District, Adult Education to provide supervised field instruction in the County’s Public Health Division, for Contractor’s dental assistant students, for the period July 1, 2012 through June 30, 2014. (No Fiscal Impact)   C.26 APPROVE and AUTHORIZE the Purchasing Agent, on behalf of Health Services September 18, 2012 Contra Costa County Board of Supervisors 6 C.26 APPROVE and AUTHORIZE the Purchasing Agent, on behalf of Health Services Department, to execute a purchase order with KCI, Inc., in the amount of $123,405, for four infovac machines for Contra Costa Regional Medical Center and Contra Costa Health Centers. (100% Enterprise Fund I)   C.27 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract amendment with Lee Shratter, M.D., effective September 1, 2012, to increase the payment limit by $160,600 to a new total payment limit of $774,600, to provide additional radiology services at Contra Costa Regional Medical and Health Centers, with no change in the original term of January 1, 2011 through December 31, 2013. (100% Enterprise I Funds)   C.28 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract amendment with Jackson & Coker LocumTenens, LLC, effective September 1, 2012, to increase the payment limit by $170,000 to a new total payment limit of $884,400, to provide additional temporary psychiatric services for the Mental Health Outpatients Clinics, with no change in the original term of January 1, 2012 through December 31, 2012. (100% Mental Health Realignment)   C.29 APPROVE and AUTHORIZE the Director of Child Support Services, or designee, to execute a contract amendment with Integrated Information Systems, Inc., effective July 1, 2012 to increase the payment limit by $48,145 to a new payment limit not to exceed $147,325 for licensing and warranties for the TurboCourt application with no change to the term of July 1, 2012 through June 30, 2013. (66% Federal, 34% State)   C.30 APPROVE and AUTHORIZE the Purchasing Agent, or designee, to execute a purchase order with CDW Government, on behalf of the Chief Information Officer, in an amount not to exceed $125,000, for the purchase of Sophos Complete Security software to help protect the County against computer viruses and malware for the period of October 1, 2012 through September 30, 2013. (100% Department User fees)   C.31 APPROVE and AUTHORIZE the Purchasing Agent to execute, on behalf of the Sheriff-Coroner, a purchase order with Adamson Police Products in an amount not to exceed $100,000 for the purchase of miscellaneous police supplies for the period July 1, 2012 through June 30, 2015. (100% County General Fund; Budgeted)   Other Actions   C.32 ACCEPT the August 2012 update on the operations of the Employment and Human Services Department, Community Services Bureau, as recommended by the Employment and Human Services Department Interim Director.   C.33 MODIFY the composition of the County Service Area P-2B Citizens Advisory September 18, 2012 Contra Costa County Board of Supervisors 7 C.33 MODIFY the composition of the County Service Area P-2B Citizens Advisory Committee by adding two seats, 1st Alternate Seat and 2nd Alternate Seat, as recommended by Supervisor Andersen.   C.34 ESTABLISH an Ad Hoc Committee to review and make policy recommendations to the Board of Supervisors on the Community Warning System and Industrial Safety Ordinance and APPOINT Supervisors Gioia and Glover to serve on the Committee, as recommended by Supervisors Gioia and Glover.   GENERAL INFORMATION The Board meets in all its capacities pursuant to Ordinance Code Section 24-2.402, including as the Housing Authority and the Successor Agency to the Redevelopment Agency. Persons who wish to address the Board should complete the form provided for that purpose and furnish a copy of any written statement to the Clerk. Any disclosable public records related to an open session item on a regular meeting agenda and distributed by the Clerk of the Board to a majority of the members of the Board of Supervisors less than 72 hours prior to that meeting are available for public inspection at 651 Pine Street, First Floor, Room 106, Martinez, CA 94553, during normal business hours. All matters listed under CONSENT ITEMS are considered by the Board to be routine and will be enacted by one motion. There will be no separate discussion of these items unless requested by a member of the Board or a member of the public prior to the time the Board votes on the motion to adopt. Persons who wish to speak on matters set for PUBLIC HEARINGS will be heard when the Chair calls for comments from those persons who are in support thereof or in opposition thereto. After persons have spoken, the hearing is closed and the matter is subject to discussion and action by the Board.
