County an amount not to exceed $56,526, to perform regulatory enforcement
activities related to Sudden Oak Death disease, for the period of July 1, 2013

C. 40 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with the Department of Housing and Urban Development, to
pay County an amount not to exceed $173,567, for County’s McKinney-Vento
Rapid Re-housing Assistance project, for the period from July 1, 2013 through

C. 41 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with the State of California Emergency Medical Services
Authority, to pay the County an amount not to exceed $30,000, for County’s
Emergency Medical Services Agency workflow environment assessment project,
for the period from September 1, 2013 through December 31, 2013. (No County

C. 42 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with the State of California, Office of Administrative Hearings,
in an amount not to exceed $48,000, to allow the State to conduct hearings for
disciplinary actions against Emergency Medical Technicians as required by State
regulations, for the period from October 1, 2013 through September 30, 2018.

C. 43 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Vista Staffing Solutions, Inc. in an amount not to exceed
$350,000, to provide temporary physicians at Contra Costa Regional Medical and
Health Centers and the main detention facility, for the period October 1, 2013
through September 30, 2014. (100% Enterprise Fund I)
C. 44 APPROVE and AUTHORIZE the Employment and Human Services Director, or
designee, to execute a contract amendment with Robert Half International, Inc., to
increase the payment limit by $500,000, to a new payment limit of $1,000,000, for
temporary help for technology initiatives, and extend the contract term from
October 31, 2013 to June 30, 2014. (10% County; 45% State; 45% Federal)
C. 45 APPROVE and AUTHORIZE the Employment and Human Services Director, or
designee, to execute contract amendments with Northwoods Consulting Partners,
Inc., to increase the payment limit by $499,086, to a new payment limit of
$4,563,660, to provide additional software, support, training and services for the
electronic capture, storage, and retrieval of case files, for the period December 4,

C. 46 APPROVE and AUTHORIZE the Employment and Human Services Director, or
designee, to execute a contract amendment with Community Violence Solutions,
Inc., to increase the amount payable by $57,000, for new total not to exceed
$167,000, for the continued operation of the Children's Interview Center, with no
change in term, July 1, 2013 through June 30, 2014. (27% County; 38% Local

C. 47 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Sodexo America, LLC in an amount not to exceed
$284,060, to provide management and oversight of the Food and Nutrition
Services Unit at Contra Costa Regional Medical and Health Centers, for the period
November 1, 2013 through October 31, 2014. (100% Enterprise Fund I)
C. 48 APPROVE and AUTHORIZE the Employment and Human Services Director, or
designee, to execute a contract amendment with STAND! For Families Free of
Violence, to include advocacy services, with no change in the payment limit of
$233,000, and no change in the contract term of January 1, 2013 through
December 31, 2014. (100% Federal)
C. 49 APPROVE and AUTHORIZE the Health Services Director, or designee, to

execute a contract with East Bay Sports Medicine and Orthopaedic Associates, A
Medical Corporation, in an amount not to exceed $180,000, to provide orthopedic

C. 50 APPROVE and AUTHORIZE the Employment and Human Services Director, or
designee, to execute a contract amendment with Lao Family Community
Development Center, making minor changes to the contract deliverables, with no
change in the payment limit of $105,668, and no change in the contract term of

C. 51 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Chris Cammisa, M.D., (DBA Cammisa Healthcare
Consulting) in an amount not to exceed $150,000, to provide healthcare
consultation and technical assistance to the Health Plan Medical Management
team, for the period October 1, 2013 through September 30, 2014. (100% Member

C. 52 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Asante Alliance, LLC, in an amount not to exceed
$360,000, to provide consultation and technical assistance with regard to support
of the Epic Electronic Health Record Project for the Health Service Department,
for the period from November 1, 2013 through October 31, 2014. (100%
Enterprise Fund I)
C. 53 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Aspira Technologies, Inc. in an amount not to exceed
$695,000, to provide consultation and technical assistance to the Department’s
Information Systems Unit for the period from December 1, 2013 through
November 30, 2014. (100% Enterprise Fund I)
C. 54 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract amendment with Aspira Technologies, Inc., effective October
1, 2013, to increase the payment limit by $70,000 to a new payment limit of
$770,000 to provide additional hours of services with regard to designing and
developing complex clinic software applications and interfaces for the Health
Services Department’s Information Systems Unit, with no change in the original
term of December 1, 2012 through November 30, 2013. (100% Enterprise Fund I)
C. 55 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with David Safianoff, M.D. in an amount not to exceed
$1,080,000, to provide professional pulmonary services at Contra Costa County
Regional Medical and Contra Costa Health Centers, for the period October 1, 2013
through September 30, 2016. (100% Enterprise Fund I)

C. 56 APPROVE and AUTHORIZE the County Probation Officer, or designee, to
execute a contract extension with Rubicon Programs to change the contract
termination date from September 30, 2013 to December 31, 2013 to provide
reentry services to probationers enrolled in the Second Chance Reentry Initiative
with no change to the payment. (No fiscal impact)
C. 57 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a novation contract with the Jewish Family and Children’s Services of the
East Bay, in an amount not to exceed $159,699, to provide Mental Health Services
Act prevention and early intervention services for the period July 1, 2013 through
June 30, 2014, with a six-month automatic extension through December 31, 2014,
in an amount not to exceed $79,849. (100% Mental Health Services Act)
C. 58 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a novation contract with Neighborhood House of North Richmond in an
amount not to exceed $13,000, including modifications to standard general
conditions and indemnification language, to provide congregate meals for the
Senior Nutrition Program, for the period from July 1, 2013 through June 30, 2014,
with a three-month automatic extension through September 30, 2014, in an
amount not to exceed $3,250. (100% Federal Title III C-1 of the Older Americans
Act of 1965)
C. 59 APPROVE and AUTHORIZE the Purchasing Agent, or designee, to execute, on
behalf of the County Librarian, a purchase order with Dell Computers in an
amount not to exceed $210,000 for replacement of computer systems for the period
November 15, 2013 through June 30, 2014. (100% Library Fund)
C. 60 APPROVE and AUTHORIZE the Health Services Director, or designee, to

contract with Sheriff's Deputy Tim Allen to pay the County $1.00 for retired

All matters listed under CONSENT ITEMS are considered by the Board to be routine and will be
enacted by one motion. There will be no separate discussion of these items unless requested by a
member of the Board or a member of the public prior to the time the Board votes on the motion to
adopt.
Persons who wish to speak on matters set for PUBLIC HEARINGS will be heard when the Chair
calls for comments from those persons who are in support thereof or in opposition thereto. After
persons have spoken, the hearing is closed and the matter is subject to discussion and action by the
Board. Comments on matters listed on the agenda or otherwise within the purview of the Board of
Supervisors can be submitted to the office of the Clerk of the Board via mail: Board of
