Camino Tassajara Shoulder Widening, 1.1 miles south of Highland Road to 0.30

Human Services (0501), Public Works (0650) for fiscal year 2013-14 Venture

C. 32 Probation Programs (0308): APPROVE Appropriations and Revenue Adjustment
No. 5013 authorizing new revenue in Probation Programs (0308) in the amount of
$139,242 to fund the Proud Parenting Program, a collaboration between the
County Probation Department and STAND! For Families Free of Violence. (90%
State, 10% STAND! For Families Free of Violence)
C. 33 Probation Programs (0308): APPROVE Appropriations and Revenue Adjustment
No. 5005 authorizing new revenue in the amount of $248,668 from the U.S.
Department of Justice "Smart Probation: Reducing Prison Populations, Saving
Money, and Creating Safer Communities" grant and appropriating it to implement
evidence-based practices that target high-risk probationers by identifying
medium/high-risk offenders, improving pre-release contact, and increasing
capacity for cognitive behavior treatment. (100% Federal)

C. 34 ADOPT Position Adjustment Resolution No. 21371 to reallocate the salary of the
Fire Chief-Contra Costa County Fire Protection District Exempt (unrepresented),

C. 35 APPROVE and AUTHORIZE the Public Works Director, or designee, to execute

the First Amendment to Lease with Sand Creek Business Associates I, LLC, to
extend the term for ten years and six months beginning February 1, 2015 and
ending August 31, 2025, for use of approximately 10,052 square feet of medical
office space located at 171 Sand Creek Road, Units A through E, Brentwood, for
continued occupancy by the Health Services Department, under the terms and
conditions set forth in the Lease. (100% Health Services Enterprise Fund)

APPROVE and AUTHORIZE execution of agreements between the County and the
following agencies for receipt of fund and/or services:
C. 36 APPROVE and AUTHORIZE the Animal Services Department Director, or
designee, to accept funding in the amount of $15,000 from the State of California
Food and Agriculture Department's 2013 Municipal Spay-Neuter Grant for the
County’s Spay and Neuter Program. (100% State Funds)
C. 37 APPROVE and AUTHORIZE the Employment and Human Services Director, or
designee, to accept funding from California Health Advocates in the amount of
$4,500, for the Senior Medicare Patrol Volunteer Liaison, for the period October 1,
2013 through August 31, 2014. (No County match)
C. 38 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with the Department of Housing and Urban Development to pay
the County an amount not to exceed $511,063 in homeless assistance funds for
continuation of Project Coming Home - Addressing Addiction to Alcohol
Program, for the period June 1, 2013 through May 31, 2014. (No County match)
C. 39 APPROVE and AUTHORIZE the Health Services Director, or designee, to accept
the funds from the National Children’s Oral Health Foundation, in an amount not
to exceed $15,000, for the County’s Public Health Comprehensive Preventative
Oral Health Program and Community Outreach and Clinic Program, for the period
July 1, 2013 through June 30, 2014. (No County match)
APPROVE and AUTHORIZE execution of agreement between the County and the
following parties as noted for the purchase of equipment and/or services:
C. 40 APPROVE and AUTHORIZE the Public Works Director, or designee, to execute a contract
amendment with Kitchell CEM effective October 31, 2013, to increase the payment limit by
$130,000 to a new payment limit of $490,000, to provide additional construction management
services for the New Martinez Wellness Center, 2500 Alhambra Avenue, Martinez, and the Crisis
Residential Facility, 300 Ilene Street, Martinez and extend the contract period through April 30,
2014. (100% 2012 Lease Revenue Bonds)

C. 41 APPROVE and AUTHORIZE the Purchasing Agent, or designee, to execute, on
behalf of the Sheriff-Coroner, a purchase order with Engineered Control Systems
in an amount not to exceed $240,605 for the upgrade and remodel of the Central
Control system at the Martinez Detention Facility. (100% General Fund)
C. 42 APPROVE and AUTHORIZE the Conservation and Development Director, or
designee, to execute a contract amendment with Macias, Gini & O'Connell, LLP,
to increase the payment limit by $50,000 to a new payment limit of $594,000 to
provide additional special accounting services, with no change in the original term
of July 1, 2011 through December 31, 2013. (100% Redevelopment Property Tax
Trust Fund)
C. 43 APPROVE and AUTHORIZE the Clerk-Recorder, or designee, to execute a
contract amendment with S.O.E. Software Corporation, effective October 1, 2013,
to increase the payment limit by $28,325 to a new payment limit of $464,198 to
provide mobile access capability to the Clerk-Recorder-Elections web information
management and election night results reporting systems, with no change to the
term August 21, 2007 through August 20, 2014. (100% General Fund, Budgeted)
C. 44 APPROVE and AUTHORIZE the Public Works Director, or designee, to execute
a Consulting Services Agreement dated October 22, 2013 through October 22,
2017 (with a one-year extension option), with DLR Group, Inc., Sacramento, in
the amount of $400,000, to provide as-needed architectural services for various
facilities projects, Countywide. (100% Various Funds)
C. 45 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Chiu-Hung Tung, M.D., Inc., in an amount not to exceed
$1,425,000, to provide professional anesthesiology services at Contra Costa
Regional Medical and Health Centers, for the period October 1, 2013 through
September 30, 2016. (100% Enterprise Fund I)
C. 46 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Jeffrey Saadi, M.D., in an amount not to exceed
$1,200,000, to provide professional anesthesiology services at Contra Costa
Regional Medical and Health Centers, for the period October 1, 2013 through
September 30, 2016. (100% Enterprise Fund I)
C. 47 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract amendment with Docustream, Inc., effective September 30,
2013, to increase the payment limit by $40,000, to a new payment limit of
$440,000, to provide additional electronic claims processing services with no
change in the original term of November 1, 2012 through October 31, 2013.

C. 48 APPROVE and AUTHORIZE the Animal Services Department Director, or

Medicine Support, in an amount not to exceed $1,644,612, to provide general

C. 49 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract amendment with Ronald L. Leon, M.D., effective October 1,
2013, to increase the payment limit by $34,720 to a new payment limit of
$151,200 to provide additional outpatient psychiatric services in East County, with
no change in the original term of April 1, 2013 through March 31, 2014. (100%
Mental Health Realignment Fund)
C. 50 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Tom H. Piatt, M.D., Inc., in an amount not to exceed
$100,000, to provide diagnostic imaging services to Contra Costa Health Plan
members, for the period from September 1, 2013 through August 31, 2015. (100%
Member Premiums)
C. 51 APPROVE and AUTHORIZE the Sheriff-Coroner, or designee, to execute a
contract with Synesis Inc. in an amount not to exceed $250,000 to provide
programming services to generate AB109 reports that will be a module of ARIES
for the period September 1, 2013 through August 31, 2014. (91% State, 9%
County General Fund)
C. 52 APPROVE a termination agreement that terminates the construction contract dated
June 4, 2013, between the County and EVRA Construction, Inc., for the Painting
Hangar Rows at the Buchanan Field Airport project, 550 Sally Ride Drive,
Concord, as recommended by the Public Works Director. (100% Airport
Enterprise Funds)
C. 53 APPROVE and AUTHORIZE the County Probation Officer, or designee, to
execute a contract with STAND! For Families Free of Violence in an amount not
to exceed $124,242 to provide parenting training for participants ages 14-25 who
are involved with the criminal justice system for the period July 1, 2013 through
June 30, 2014. (100% State)
C. 54 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Greater Richmond Inter-Faith Program, in an amount not
to exceed $1,188,191, to provide emergency shelter program services for youth,
for the period October 1, 2013 through September 30, 2014. (32% Federal, 48%
State Mental Health Services Act, 16% Employment and Human Services
Department, 4% General Fund)
C. 55 APPROVE and AUTHORIZE the Health Services Director, or designee, to
October 22, 2013 Contra Costa County Board of Supervisors 10

execute a contract with Daniel Forkin, M.D. in an amount not to exceed $145,600,
to provide professional outpatient psychiatric services to mentally ill adults in
West County, for the period October 1, 2013 through September 30, 2014. (100%

C. 56 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Robert J. Lindsay, M.D., in an amount not to exceed
$1,050,000, to provide professional anesthesiology services at Contra Costa
Regional Medical and Health Centers, for the period October 1, 2013 through
September 30, 2016. (100% Enterprise Fund I)
C. 57 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Bayfront Youth and Family Services in an amount not to
exceed $150,000, to provide specialty mental health services to seriously
emotionally disturbed children, for the period September 1, 2013 through June 30,
2014, including a six-month automatic extension through December 31, 2014 in

execute a novation contract with La Clínica de La Raza, Inc., in an amount not to
exceed $256,750, to provide Mental Health Services Act Prevention and Early
Intervention services for the period July 1, 2013 through June 30, 2014, with a
six-month automatic extension through December 31, 2014, in an amount not to
exceed $128,375. (100% Mental Health Services Act)
C. 60 APPROVE and AUTHORIZE the County Administrator, or designee, to execute a
contract with Cheiron, Inc. in an amount not to exceed $265,000 for analytical
services and facilitation of the County's Health Care Options Workgroup for the
period October 1, 2013 through September 30, 2016, as recommended by the
County Administrator. (100% Benefits Administration Charges)
C. 61 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Diablo Pulmonary Medical Group, Inc., in an amount not
to exceed $227,700, to provide pulmonary disease specialty services at Contra
Costa Regional Medical and Health Centers, for the period October 1, 2013
through September 30, 2016. (100% Enterprise Fund I)

C. 62 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract amendment with Process Improvement Institute, Inc., effective
September 1, 2013, to modify County’s General Conditions to include mutual
indemnification, for the provision of consultation and technical assistance with
regard to a Safety Evaluation Report of the Chevron Richmond Refinery, with no
change in the payment limit of $876,600, and no change in the original term of
August 1, 2013 through July 31, 2015. (100% Reimbursed by Chevron)
C. 63 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract amendment with Kaiser Foundation Health Plan, Inc., effective
September 30, 2013, including modified indemnification language, to increase the
payment limit by $7,000,000 to a new payment limit of $67,000,000 to provide
additional professional health care services for Medi-Cal recipients enrolled in
Kaiser Foundation Health Plan, and to extend the term from September 30, 2013
through December 31, 2013. (100% Contra Costa Health Plan member premiums)
C. 64 APPROVE and AUTHORIZE the County Administrator, or designee, to execute a
contract with Milliman, Inc. in an amount not to exceed $400,000 for actuarial
services for the period October 1, 2013 through September 30, 2016, as

C. 65 APPROVE and AUTHORIZE County Risk Manager to execute a contract with
Arnold & Porter LLP for legal services commencing October 1, 2013, in an
amount not to exceed $150,000, and APPROVE and AUTHORIZE County
Counsel to execute a conflict waiver with Arnold & Porter LLP. (100% General

C. 66 APPROVE and AUTHORIZE the County Administrator, or designee, to execute a
contract with Resource Development Associates in an amount not to exceed
$246,000, to provide data collection and program evaluation services for AB 109,

C. 78 APPROVE and AUTHORIZE the allocation of $219,177 from the Crockett
Co-Generation Property Tax Allocation for four projects as recommended by the
Crockett Community Foundation and Supervisor Glover. (100% General Fund)

Housing Authority and the Successor Agency to the Redevelopment Agency. Persons who wish to

distributed by the Clerk of the Board to a majority of the members of the Board of Supervisors less

All matters listed under CONSENT ITEMS are considered by the Board to be routine and will be
enacted by one motion. There will be no separate discussion of these items unless requested by a
member of the Board or a member of the public prior to the time the Board votes on the motion to
adopt.
Persons who wish to speak on matters set for PUBLIC HEARINGS will be heard when the Chair
calls for comments from those persons who are in support thereof or in opposition thereto. After
persons have spoken, the hearing is closed and the matter is subject to discussion and action by the
Board. Comments on matters listed on the agenda or otherwise within the purview of the Board of
