(100% Various Funds)   C. 45 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract with Chiu-Hung Tung, M.D., Inc., in an amount not to exceed $1,425,000, to provide professional anesthesiology services at Contra Costa Regional Medical and Health Centers, for the period October 1, 2013 through September 30, 2016. (100% Enterprise Fund I)   C. 46 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract with Jeffrey Saadi, M.D., in an amount not to exceed $1,200,000, to provide professional anesthesiology services at Contra Costa Regional Medical and Health Centers, for the period October 1, 2013 through September 30, 2016. (100% Enterprise Fund I)   C. 47 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract amendment with Docustream, Inc., effective September 30, 2013, to increase the payment limit by $40,000, to a new payment limit of $440,000, to provide additional electronic claims processing services with no change in the original term of November 1, 2012 through October 31, 2013. Leon, M.D., effective October 1, 2013, to increase the payment limit by $34,720 to a new payment limit of $151,200 to provide additional outpatient psychiatric services in East County, with no change in the original term of April 1, 2013 through March 31, 2014. (100% Airport Enterprise Funds)   C. 53 APPROVE and AUTHORIZE the County Probation Officer, or designee, to execute a contract with STAND! For Families Free of Violence in an amount not to exceed $124,242 to provide parenting training for participants ages 14-25 who are involved with the criminal justice system for the period July 1, 2013 through June 30, 2014. (100% State)   C. 54 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract with Greater Richmond Inter-Faith Program, in an amount not to exceed $1,188,191, to provide emergency shelter program services for youth, for the period October 1, 2013 through September 30, 2014. (32% Federal, 48% State Mental Health Services Act, 16% Employment and Human Services Department, 4% General Fund)   C. 55 APPROVE and AUTHORIZE the Health Services Director, or designee, to October 22, 2013 Contra Costa County Board of Supervisors 10 C. 55 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract with Daniel Forkin, M.D. in an amount not to exceed $145,600, to provide professional outpatient psychiatric services to mentally ill adults in West County, for the period October 1, 2013 through September 30, 2014. (100% Mental Health Realignment)   C. 56 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract with Robert J. Lindsay, M.D., in an amount not to exceed $1,050,000, to provide professional anesthesiology services at Contra Costa Regional Medical and Health Centers, for the period October 1, 2013 through September 30, 2016. (100% Enterprise Fund I)   C. 57 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract with Bayfront Youth and Family Services in an amount not to exceed $150,000, to provide specialty mental health services to seriously emotionally disturbed children, for the period September 1, 2013 through June 30, 2014, including a six-month automatic extension through December 31, 2014 in an amount not to exceed $75,000. (50% Federal Financial Participation; 50% Mental Health Realignment)   C. 58 APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a novation contract with Community Options for Families and Youth, Incorporated, in an amount not to exceed $1,620,834, to provide therapeutic behavioral services and multisystemic behavioral therapy for the period July 1, 2013 through June 30, 2014, with a six-month automatic extension through December 31, 2014 in an amount not to exceed $810,417.
