bidding on construction contracts from $100,000 to the current State limit of
$175,000, ties the County's limit to the State Public Contract Code limit
henceforth, and clarifies the purchasing agent’s authority to award construction
contracts; and DIRECT the County Administrator to update the County's Outreach
Program policies and procedures to reflect the change in the monetary limit for
informal bidding on construction contracts, as recommended by the Internal

C. 40 AMEND Board Order C.33, September 27, 2011, which approved and authorized
the Sheriff-Coroner, or designee, to apply for and accept California Homeland
Security grant funds from the California Governor's Office of Emergency
Services, by changing the award amount indicated on the board order and
resolution, from $1,000,000 to $1,475,537; and ADOPT Resolution No. 2013/394
authorizing specified Sheriff Department officials to act on behalf of the County to

C. 41 APPROVE and AUTHORIZE the Sheriff-Coroner, or designee, to execute a
contract with Men and Women of Purpose in an amount not to exceed $150,000
for the provision of mentoring and placement services to assist adult inmates
transitioning into a mainstream environment for the period July 1, 2013 through

APPROVE and AUTHORIZE execution of agreement between the County and the
following parties as noted for the purchase of equipment and/or services:

a contract amendment with Alfredo Ruvalcaba (dba BAT Fire Protection Systems)
to increase the payment limit by $220,000 to a new payment limit of $300,000, for
fire sprinkler certification and repair services, with no change in the original term
of June 1, 2012 through May 31, 2015, Countywide. (100% General Fund)
C. 43 APPROVE and AUTHORIZE the Purchasing Agent, or designee, to execute, on

C. 44 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with John R. Rostkowski, M.D., in an amount not to exceed
$116,480 to provide professional outpatient psychiatric services to patients in
Central County for the period October 1, 2013 through September 30, 2014.

C. 45 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract amendment with LocumTenens.com, LLC, effective September
1, 2013, to modify the rate schedule to include neurology services and allow
Contractor to continue to provide temporary psychiatrist and physician coverage at
Contra Costa Regional Medical and Health Centers, with no change in the
payment limit of $300,000 and no change in the original term of March 1, 2013
through February 28, 2014. (100% Enterprise Fund I)
C. 46 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Deepika Sastry, M.D. in an amount not to exceed
$145,600, to provide professional outpatient psychiatric services to mentally ill
adults in West County, for the period October 1, 2013 through September 30,
2014. (100% Mental Health Realignment)
C. 47 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract amendment with Youth Homes Incorporated, effective June 1,
2013, to increase the payment limit by $28,466 to a new payment limit of
$2,828,466 to provide additional residential day treatment services, with no change
in the original term of July 1, 2012 through June 30, 2013, and to increase the
automatic extension payment limit by $28,466 to a new payment limit of
$1,428,466, with no change in the term of the automatic extension, through
December 31, 2013. (50% Federal Financial Participation; 50% Mental Health
Realignment)
C. 48 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract Relyon Consulting, Inc., in an amount not to exceed $292,000,
to provide consultation and technical assistance with regard to support of the Epic
Electronic Health Record for the Information Technology Unit of the Health
Service Department, for the period from October 1, 2013 through September 30,
2014. (100% Enterprise I Funds)
C. 49 APPROVE and AUTHORIZE the Health Services Director, or designee, to

execute a contract with Craig Nielson, M.D., in an amount not to exceed $450,000,
to provide anesthesiology services at Contra Costa Regional Medical and Health

C. 50 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Ralph Chase, M.D., in an amount not to exceed
$1,200,000, to provide professional anesthesiology services at Contra Costa
Regional Medical and Health Centers, for the period October 1, 2013 through
September 30, 2016. (100% Enterprise Fund I)
C. 51 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Paul H. Kwok, D.O., in an amount not to exceed
$1,410,000, to provide professional anesthesiology services at Contra Costa
Regional Medical and Health Centers, for the period October 1, 2013 through
September 30, 2016. (100% Enterprise Fund I)
C. 52 CORRECT Board Action of June 25, 2013 (C.142) to APPROVE and
AUTHORIZE the Employment and Human Services Director, or designee, to
execute a contract with Lutheran Social Services of Northern California, to
provide transitional housing assistance for emancipated youth, by changing the
total contract payment limit from $171,159 to an amount not to exceed $172,159,
for the period July 1, 2013 through June 30, 2014. (100% State)
C. 53 APPROVE and AUTHORIZE the Purchasing Agent to execute, on behalf of the
Sheriff-Coroner, a purchase order with Sysco Food Services in an amount not to
exceed $300,000 to provide poultry products and related items for use in the
preparation of inmate meals at all County adult detention facilities for the period
October 1, 2013 through September 30, 2015. (100% General Fund)
C. 54 APPROVE and AUTHORIZE the Purchasing Agent to execute, on behalf of the
Sheriff-Coroner, a purchase order with Sysco Food Services in an amount not to
exceed $2,500,000 to provide non-poultry food and all non-food related products,
items, equipment and supplies as needed for use in County adult detention
facilities for the period October 1, 2013 through September 30, 2015. (100%
General Fund)
C. 55 APPROVE and AUTHORIZE the Purchasing Agent to execute, on behalf of the
Sheriff-Coroner, a purchase order with Good Source in an amount not to exceed
$200,000 to provide food products for the preparation of inmate meals in County
adult detention facilities for the period October 1, 2013 through September 30,

C. 57 CORRECT the Board Order approved by the Board of Supervisors on June 25,
2013 (C. 78) with the Child Abuse Prevention Council of Contra Costa, to reflect
the intent of the parties, to change the contract term from July 1, 2013 through
June 30, 2024 to July 1, 2012 through June 30, 2014 for the Nurturing Parenting
Program and Baby Bag Project services. (60% State, 40% County)
C. 58 ACCEPT the August 2013 update of the operations of the Employment and
Human Services Department, Community Services Bureau, as recommended by
the Employment and Human Services Department Director.
C. 59 DECLARE as surplus and AUTHORIZE the Purchasing Agent, or designee, to
dispose of fully depreciated vehicles and equipment no longer needed for public
use as recommended by the Public Works Director, Countywide. (No fiscal

C. 60 ADOPT Resolution No. 2013/380 accepting as complete the construction contract
work performed by Arboricultural Specialties, Inc. (dba The Professional Tree
Care Company) for the Tree Restoration at West County Detention Facility, 5555
Giant Highway, Richmond, for the Public Works Department, as recommended by
the Public Works Director, Richmond area. (No fiscal impact)
C. 61 CONTINUE the emergency action originally taken by the Board of Supervisors on
November 16, 1999 regarding the issue of homelessness in Contra Costa County,
as recommended by the Health Services Director. (No fiscal impact)

The Board meets in all its capacities pursuant to Ordinance Code Section 24-2.402, including as the
Housing Authority and the Successor Agency to the Redevelopment Agency. Persons who wish to
address the Board should complete the form provided for that purpose and furnish a copy of any
written statement to the Clerk.
Any disclosable public records related to an open session item on a regular meeting agenda and
distributed by the Clerk of the Board to a majority of the members of the Board of Supervisors less
than 72 hours prior to that meeting are available for public inspection at 651 Pine Street, First
Floor, Room 106, Martinez, CA 94553, during normal business hours.
All matters listed under CONSENT ITEMS are considered by the Board to be routine and will be
enacted by one motion. There will be no separate discussion of these items unless requested by a

Persons who wish to speak on matters set for PUBLIC HEARINGS will be heard when the Chair
calls for comments from those persons who are in support thereof or in opposition thereto. After
persons have spoken, the hearing is closed and the matter is subject to discussion and action by the
Board. Comments on matters listed on the agenda or otherwise within the purview of the Board of
Supervisors can be submitted to the office of the Clerk of the Board via mail: Board of
Supervisors, 651 Pine Street Room 106, Martinez, CA 94553; by fax: 925-335-1913.
The County will provide reasonable accommodations for persons with disabilities planning to
attend Board meetings who contact the Clerk of the Board at least 24 hours before the meeting, at
(925) 335-1900; TDD (925) 335-1915. An assistive listening device is available from the Clerk,
Room 106.
Copies of recordings of all or portions of a Board meeting may be purchased from the Clerk of the

necessary arrangements.
Forms are available to anyone desiring to submit an inspirational thought nomination for inclusion
on the Board Agenda. Forms may be obtained at the Office of the County Administrator or Office
