APPROVE and AUTHORIZE execution of agreements between the County and the
following agencies for receipt of fund and/or services:

designee, to accept a grant in the amount of $50,000 from Chevron Corporation
for the Employment and Human Services Department to provide consultation,
training, and business information to small businesses in the City of Brentwood.

execute and submit a non-financial agreement to the State of California Health
Benefit Exchange, for the County to act as a Certified Enrollment Entity for the

C. 25 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract amendment with the State of California, Managed Risk
Medical Insurance Board, effective October 1, 2013, for County’s participation in
the Healthy Families Program, with no change in amount payable to County of
$34,202,678, to make technical adjustments to the agreement, and to extend the
term through June 30, 2014. (No County match)
C. 26 APPROVE and AUTHORIZE the Employment & Human Services Department
Director, or designee, to accept funding from First 5 Contra Costa Children and
Families Commission, in an amount not to exceed $45,000, for Quality Rating
and Improvement System stipends for the period January 1, 2014 through

C. 27 APPROVE and AUTHORIZE the Employment & Human Services Director, or
designee, to apply for and accept funding from the U.S. Department of Health and
Human Services, Administration for Children and Families, in an amount not to
exceed $14,679,829 for Head Start program services for the period of January 1 -

C. 28 APPROVE and AUTHORIZE the Employment & Human Services Director, or
designee, to apply for and accept funding from the U.S. Department of Health and

APPROVE and AUTHORIZE execution of agreement between the County and the
following parties as noted for the purchase of equipment and/or services:

behalf of the Public Works Director, a purchase order amendment with Air
Products Group to increase the payment limit by $65,000 to a new payment limit
of $245,000, and extend the term from November 1, 2010 through October 31,
2014, for heating, ventilation and air conditioning air filtration components,

C. 31 APPROVE and AUTHORIZE the Public Works Director, or designee, to execute
a contract amendment with Matt Colchico dba Summit Building Services to

with the Foundation Center in an amount not to exceed $1,500 to provide online
access to a database of U.S. funders and their grants for the period October 1,

designee, to execute a contract amendment with Liberty Adult Education to

designee, to execute a contract amendment with Mt. Diablo Adult Education to

$132,015 for education and training services to CalWORKs clients, and to extend
the term from August 31, 2013 to October 31, 2013. (100% State)
C. 38 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Denis Mahar M.D., in an amount not to exceed
$1,728,000, to provide cardiology services at Contra Costa Regional Medical and
Health Centers, for the period from October 1, 2013 through September 30, 2016.

C. 39 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Thomas Paige, M.D., in an amount not to exceed
$348,000, to provide dermatology services at Contra Costa Regional Medical and
Health Centers, for the period from October 1, 2013 through September 30, 2016.

C. 40 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract amendment with Janet Diaz, M.D., Inc., effective August 1,
2013, to increase the payment limit by $70,000 to a new payment limit of
$420,000, to provide additional professional pulmonary services, and to modify
the termination date from October 31, 2015 to October 31, 2014. (100%

C. 41 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Christopher John Voscopoulos, M.D., in an amount not to
exceed $345,000, to provide anesthesiology services at Contra Costa Regional
Medical and Health Centers, for the period September 1, 2013 through August 31,
2016. (100% Enterprise Fund I)
C. 42 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a novation contract with RYSE, Inc., a California Nonprofit Corporation,
in an amount not to exceed $460,119, to provide Mental Health Services Act
prevention and early intervention services for the period July 1, 2013 through
June 30, 2014, with a six-month automatic extension through December 31,
2014, in an amount not to exceed $230,060. (100% Mental Health Services Act)
C. 43 APPROVE and AUTHORIZE the Purchasing Agent, on behalf of the Health

Services Department, to execute a purchase order with Hill-Rom Company, Inc.
in the amount of $200,000, for preventive maintenance service and repair of
specialty beds at Contra Costa Regional Medical Center, for the period from
November 1, 2013 through October 31, 2016. (100% Enterprise Fund I)
C. 44 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a novation contract with Bay Area Community Resources, Inc., in an
amount not to exceed $365,814, to provide drug abuse prevention and treatment
services, for the period July 1, 2013 through June 30, 2014, with a six-month
automatic extension through December 31, 2014, in an amount not to exceed
$182,907. (88% Federal Primary Prevention Set-Aside; 12% Bay Area Service
Network)
C. 45 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a novation contract with Lifelong Medical Care, in an amount not to
exceed $118,970, to provide Mental Health Services Act Prevention and Early
Intervention services for the period July 1, 2013 through June 30, 2014, with a
six-month automatic extension through December 31, 2014, in an amount not to
exceed $59,485. (100% Mental Health Services Act)
C. 46 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a novation contract with Adolescent Treatment Centers, Inc. in an amount
not to exceed $358,838, to provide day treatment and mental health services for
seriously emotionally disturbed dually-diagnosed adolescents at its Thunder Road
Facility, for the period July 1, 2013 through June 30, 2014, with a six-month
automatic extension through December 31, 2014, in an amount not to exceed
$179,419. (50% Federal Financial Participation; 50% Mental Health Realignment)
C. 47 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a novation contract with Community Health for Asian Americans in an
amount not to exceed $1,588,249, to provide mental health services, wraparound
services, and dual-diagnosis treatment services for the period from July 1, 2013
through June 30, 2014, with a six-month automatic extension through December
31, 2014, in an amount not to exceed $794,475. (48% Federal Financial
Participation, 48% Mental Health Realignment; 4% Public Health Realignment)
C. 48 APPROVE and AUTHORIZE the Purchasing Agent, on behalf of the Health
Services Department, to execute a purchase order with Alcon Laboratories, Inc, in
the amount of $550,000 for various eye surgery supplies at Contra Costa Regional
Medical Center for the period of October 1, 2013 through September 30,
2015. (100% Enterprise Fund I)
C. 49 APPROVE and AUTHORIZE the Purchasing Agent, or designee, to execute, on

C. 49 APPROVE and AUTHORIZE the Purchasing Agent, or designee, to execute, on
behalf of the Department of Information Technology, a purchase order in an
amount not to exceed $112,174 with TeleManagement Technologies, Inc., to
provide continuing software maintenance and support of the WinCall and WinBill
call accounting systems for the period October 1, 2013 through September 30,
2014, under the Equipment Software and Services Agreement. (100%

C. 50 APPROVE and AUTHORIZE the Employment and Human Services Department
Director, or designee, to execute a contract amendment with We Care Services for
Children, to increase the payment limit by $28,124, to a new payment limit not to
exceed $196,863, to provide State Preschool services with no change to the term
July 1, 2013 through June 30, 2014. (100% State)
C. 51 APPROVE and AUTHORIZE the Employment and Human Services Director, or
designee, to execute a contract amendment with First Baptist Church of Pittsburg,
to increase the payment limit by $64,744, to a new payment limit not to exceed
$828,885, to provide State Preschool, Pre-kindergarten Literacy and Head Start
and Early Head Start Program services with no change to the term July 1, 2013
through June 30, 2014. (85% State, 15% Federal)
Other Actions
C. 52 APPROVE AND AUTHORIZE the County Auditor-Controller to establish an
interest-bearing account as required by federal regulations for all HOME
Investment Partnership Act funds as recommended by the Director of
Conservation and Development. (100% Home Investment Partnership Act fund)
C. 53 APPROVE and AUTHORIZE submission of the FY 2012/13 Consolidated
Annual Performance and Evaluation Report to the U.S. Department of Housing
and Urban Development for the following federal programs: Community
Development Block Grant, HOME Investment Partnerships Act, Emergency
Solutions Grant, Housing Opportunities for Persons with AIDS,
CDBG-Recovery, Homeless Prevention and Rapid Re-housing, and
Neighborhood Stabilization Program, as recommended by the Conservation and
Development Director. (No fiscal impact)
C. 54 APPROVE and AUTHORIZE County Counsel, or her designee, to execute on
behalf of the County a conflict waiver acknowledging a potential conflict of
interest and consenting to representation of Contra Costa County by Goldfarb &
Lipman in connection with a loan of federal funds for an affordable housing
project on property located at 1540 Third Avenue in Walnut Creek.
C. 55 APPROVE and AUTHORIZE the Employment and Human Services Director, or

C. 55 APPROVE and AUTHORIZE the Employment and Human Services Director, or
designee, to execute a non-financial agreement with California State University,
San Bernadino, for student internship program and use of the Community
Services Bureau Head Start Childcare facilities, including modified
indemnification language, for the period of September 1, 2013 through August
31, 2016. (No fiscal impact)
C. 56 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute an Unpaid Student Training Agreement with University of San Francisco,
School of Nursing and Health Professions, to provide field instruction in the
Health Services Department for the Contractor’s registered nurse students, for the

manage, on behalf of current and former foster care assistance clients, child

execute a contract with West Contra Costa Healthcare District (dba Doctors
Medical Center, San Pablo) to act as a designated center to assist patients who

ST Elevation Myocardial Infarction (STEMI), for the period from September 1,
2013 through August 31, 2016. (No County match)
C. 60 APPROVE the provider recommended by the Contra Costa Health Plan's Medical
Director on August 23, 2013, and by the Health Services Director, as required by
the State Departments of Health Care Services and Managed Health Care, and the
Centers for Medicare and Medicaid Services.
C. 61 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Sutter East Bay Hospitals (dba Sutter Delta Medical
Center), including modified indemnification language, to act as a designated
center to assist patients who have been assessed by ambulance personnel with a
cardiac condition known as ST Elevation Myocardial Infarction (STEMI), for the
period from September 1, 2013 through August 31, 2016. (No County match)
C. 62 APPROVE and AUTHORIZE the Chair of the Board of Supervisors to sign the
California Department of Veterans Affairs Special Certificate of Compliance for
the "One Time Funding" Subvention Program.

C. 64 Acting as the Governing Board to the Successor Agency of the Contra Costa
County Redevelopment Agency, ADOPT Resolution No. 2013/369 approving an
administrative budget and the Recognized Obligation Payment Schedule for the
period January 1, 2014 through June 30, 2014 and ADOPT related California
Environmental Quality Act findings, as recommended by the Director of
Conservation and Development. (100% Redevelopment Property Tax Trust Fund)
GENERAL INFORMATION
The Board meets in all its capacities pursuant to Ordinance Code Section 24-2.402, including as
the Housing Authority and the Successor Agency to the Redevelopment Agency. Persons who
wish to address the Board should complete the form provided for that purpose and furnish a copy
of any written statement to the Clerk.
Any disclosable public records related to an open session item on a regular meeting agenda and
distributed by the Clerk of the Board to a majority of the members of the Board of Supervisors less
than 72 hours prior to that meeting are available for public inspection at 651 Pine Street, First
Floor, Room 106, Martinez, CA 94553, during normal business hours.
All matters listed under CONSENT ITEMS are considered by the Board to be routine and will be
enacted by one motion. There will be no separate discussion of these items unless requested by a
member of the Board or a member of the public prior to the time the Board votes on the motion to
adopt.
Persons who wish to speak on matters set for PUBLIC HEARINGS will be heard when the Chair
calls for comments from those persons who are in support thereof or in opposition thereto. After
persons have spoken, the hearing is closed and the matter is subject to discussion and action by the
Board. Comments on matters listed on the agenda or otherwise within the purview of the Board of
Supervisors can be submitted to the office of the Clerk of the Board via mail: Board of
