C.22 APPROVE and AUTHORIZE the Employment and Human Services Director, or
designee, to accept funding from Richard Heath and Associates, Inc., in the amount
not to exceed $10,000, for the California LifeLine Program Outreach Campaign to
inform and educate low income households of discounted telephone services
available to eligible Californians. (No County match)
C.23 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract amendment with Public Health Foundation Enterprises, Inc.,
effective October 7, 2013, to increase payment to the County by $10,772 to a new
payment limit of $28,804, for the Centers for Disease Control and Prevention’s
Emerging Infections Program - Retail Foods Project, with no change in the original

C.24 APPROVE and AUTHORIZE the Health Services Director, or designee, to

C.45 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Brian P. Hauck, M.D. in an amount not to exceed
$411,000, to provide professional gastroenterology services at Contra Costa
Regional Medical and Health Centers, for the period January 1, 2014 through

C.46 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with City of Richmond, on behalf of its Fire Department, in an
amount not to exceed $396,358 to provide support of the City of Richmond Fire
Department’s Hazardous Materials Response Unit and Hazardous Materials
Emergency Vehicle, for the period July 1, 2013 through June 30, 2015. (100% AB
2185 fees)
C.47 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Acusis, LLC in an amount not to exceed $200,000 to
provide medical and dictation transcription services for patients at Contra Costa
Regional Medical Center and Health Center, for the period from October 1, 2013
through September 30, 2014. (100% Enterprise Fund I)
C.48 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract amendment with Lincoln Child Center, Inc., effective
September 1, 2013, to increase the payment limit by $300,000 to a total of
$3,714,585 to provide additional school-based mental health treatment services for
severely emotionally disturbed children and their families, with no change in the
original term of July 1, 2013 through June 30, 2014, and to increase the automatic
extension payment limit by $150,000 to a new total of $1,857,293, with no change
in the term of the automatic extension, through December 31, 2014 (48% Federal
Financial Participation; 34% Mental Health Realignment; 14% Mental Health

execute a contract amendment with Fitch & Associates, LLC, to extend the term

regarding County’s Emergency Medical Services Modernization Project. (100%

execute a contract with Desarrollo Familiar, Inc., in an amount not to exceed
$207,096 to continue implementation of the Mental Health Services Act
Community Services and Supports Program, for the period July 1, 2013 through
June 30, 2014, with a six-month automatic extension through December 31, 2014,

C.52 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Asian Community Mental Health Board, in an amount not
to exceed $130,000, to provide Mental Health Services Act Prevention and Early
Intervention Services, for the period July 1, 2013 through June 30, 2014, with a
six-month automatic extension through December 31, 2014, in an amount not to

C.53 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a novation contract with Center for Human Development in an amount not
to exceed $133,000, to provide Mental Health Services Act Prevention and Early
Intervention services, for the period July 1, 2013 through June 30, 2014, with a
six-month automatic extension through December 31, 2014, in an amount not to

C.54 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with La Clinica de la Raza, Inc. in an amount not to exceed
$500,000, to provide mental health services to severely emotionally disturbed

six-month automatic extension through December 31, 2014, in an amount not to

C.56 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Charis Youth Center in an amount not to exceed $442,616,
to provide a day treatment programs and therapeutic behavioral services for
seriously emotionally disturbed adolescents, for the period July 1, 2013 through
June 30, 2014, with a six-month automatic extension through December 31, 2014,
in an amount not to exceed $221,308. (50% Federal Financial Participation; 50%

C.57 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Seneca Family of Agencies in an amount not to exceed
$7,939,762, to provide mobile crisis response and school- and community-based
children’s specialty mental health services, for the period July 1, 2013 through
June 30, 2014, with a six-month automatic extension through December 31, 2014,
in an amount not to exceed $3,969,881. (34% Federal Financial Participation; 31%

amount of $171,227 for workers' compensation and liability claims system annual

emotionally disturbed adolescents, for the period July 1, 2013 through June 30,
2014, with a six-month automatic extension through December 31, 2014, in an
amount not to exceed $127,500. (50% Federal Financial Participation; 50% Mental

C.61 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Desarrollo Familiar, Inc., in an amount not to exceed
$254,700, to provide school based mental health services for Latino children and
their families, for the period July 1, 2013 through June 30, 2014, with a six-month
automatic extension through December 31, 2014, in an amount not to exceed
$127,350. (50% Federal Financial Participation; 50% Mental Health Realignment)
C.62 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute an interagency agreement with Mt. Diablo Unified School District, in an
amount not to exceed $3,866,511, to provide school-based mental health services
to seriously emotionally disturbed students in the Mt. Diablo Unified School
District for the period July 1, 2013 through June 30, 2014, with a six-month
automatic extension through December 31, 2014 in an amount not to exceed
$1,933,256. (34% Federal Financial Participation; 34% Mental Health
Realignment; 32% Mt. Diablo Unified School District)
C.63 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Precyse Solutions, LLC, including modified
indemnification language, in an amount not to exceed $575,000, to provide
temporary medical coding, tumor registrar and oncology interim management
services for Contra Costa Regional Medical Center and Health Centers, for the
period from August 1, 2013 through July 31, 2014. (100% Enterprise Fund I)
C.64 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Native American Health Center, Inc., in an amount not to
exceed $213,422, to provide Mental Health Services Act prevention and early
intervention services for the period July 1, 2013 through June 30, 2014, with a
six-month automatic extension through December 31, 2014, in an amount not to

to the City of Clayton from various Park Dedication Trust Accounts to finance the
installation of three energy-efficient multi-sport scoreboards at the Clayton

Development Block Grant Action Plan; and APPROVE and AUTHORIZE the

the payment limit by $18,000 to a new payment limit of $108,000, with no change
in contract term, to provide educational workshops to small

The Board meets in all its capacities pursuant to Ordinance Code Section 24-2.402, including as the
Housing Authority and the Successor Agency to the Redevelopment Agency. Persons who wish to
address the Board should complete the form provided for that purpose and furnish a copy of any
written statement to the Clerk.
Any disclosable public records related to an open session item on a regular meeting agenda and
distributed by the Clerk of the Board to a majority of the members of the Board of Supervisors less
than 72 hours prior to that meeting are available for public inspection at 651 Pine Street, First
Floor, Room 106, Martinez, CA 94553, during normal business hours.
All matters listed under CONSENT ITEMS are considered by the Board to be routine and will be
enacted by one motion. There will be no separate discussion of these items unless requested by a
member of the Board or a member of the public prior to the time the Board votes on the motion to
adopt.
Persons who wish to speak on matters set for PUBLIC HEARINGS will be heard when the Chair
calls for comments from those persons who are in support thereof or in opposition thereto. After
persons have spoken, the hearing is closed and the matter is subject to discussion and action by the
Board. Comments on matters listed on the agenda or otherwise within the purview of the Board of
Supervisors can be submitted to the office of the Clerk of the Board via mail: Board of
