square feet of office space located at 1535 Fred Jackson Way, Richmond, as

APPROVE and AUTHORIZE execution of agreements between the County and the
following agencies for receipt of fund and/or services:

designee, to apply for and accept funding from the Wells Fargo Foundation in an
amount not to exceed $25,000 for advising and training aspiring low-to-moderate
income business owners in East and West Contra Costa County for the period
January 1 though December 31, 2015. (No County match)
C. 44 APPROVE and AUTHORIZE the Employment and Human Services Director, or
designee, to execute a contract with WestEd, a nonprofit research and
development agency, to conduct an in-classroom educational application study,
including acceptance of $250 stipend from West Ed to purchase educational
materials, for the period December 9 through December 31, 2104. (No County
match)
C. 45 APPROVE and AUTHORIZE the County Librarian, or designee, to accept
California State Library funding in the amount of $61,713 to meet the operational
and service expenses required by Project Second Chance, the Contra Costa
County Library adult literacy program, to provide adult literacy services for the
period July 1, 2014 through June 15, 2015. (88% budgeted Library Fund match)
C. 46 APPROVE and AUTHORIZE the Health Services Director, or designee, to

execute a contract with John Muir Health Inc. (dba John Muir Medical Center –
Concord Campus), including modified indemnification language, to pay the
County in an amount not to exceed $5,000 to be designated as a Primary Stroke
Center for the period January 1, 2015 through December 31, 2017. (No County

C. 48 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute contract amendment with the County of Alameda Health Care Services
Agency, effective March 1, 2014, to increase the payment to County by $255,901
to a new payment limit of $1,583,866, for additional coordination of essential
services to Contra Costa County residents with HIV Disease and their families,
with no change in the original term of March 1, 2014 through February 28, 2015.

execute a contract with the Superior Court of California, to pay the County an
amount not to exceed $71,899 to support services provided to participants in the

Court Program for the period October 1, 2014 through September 30, 2015.

Program, for the period July 1, 2014 through June 30, 2017. (No County match)
C. 51 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute contract with the U. S. Department of Veteran Affairs Northern
California Health Care System, to pay the County an amount not to exceed
$175,506 to support homeless veterans accessing services at the West County
Adult Interim Housing Program including meals, case management and on-site
mental health and substance abuse services, for the period October 1, 2014
through September 30, 2015. (No County match)
C. 52 APPROVE and AUTHORIZE the Health Services Director, or designee, to

execute a contract amendment with the State Department of Rehabilitation,
effective January 1, 2015, make technical adjustments to the budget for
continuous vocational rehabilitation services for individuals with psychiatric

execute a contract with the California Health Facilities Financing Authority, to
pay the County an amount not to exceed $551,511 for the Behavioral Health
Services Division/Mental Health, Contra Costa Mobile Crisis Support Teams
Project, for the period April 24, 2014 through June 30, 2015. (No County match)
C. 54 APPROVE and AUTHORIZE the Sheriff-Coroner, or designee, to execute a
contract, including mutual indemnification, with the Contra Costa West Narcotics
Enforcement Team, to pay the County an amount not to exceed $60,000 for
evidence storage services for the period January 1, 2015 through December 31,

C. 55 APPROVE and AUTHORIZE the Conservation and Development Interim
Director, or designee, to execute a contract amendment with the City of Pinole, to
extend the term from December 31, 2014 through December 31, 2017 and
increase the hourly rates specified in the contract, with no change to the total
payment limit for the County's provision of building inspection services to the

APPROVE and AUTHORIZE execution of agreement between the County and the
following parties as noted for the purchase of equipment and/or services:
C. 56 APPROVE and AUTHORIZE the Conservation and Development Interim
Director, or designee, to execute a contract amendment with Lawrence G. Mallon,
Esquire, to extend the term from December 31, 2014 through December 31, 2016
and increase the payment limit by $50,050 to a new payment limit of $150,000 to
provide technical assistance services related to the San Francisco to Stockton
Navigation Improvement Study and other navigation projects. (100% Ship

Housing Authority and the Successor Agency to the Redevelopment Agency. Persons who wish to

All matters listed under CONSENT ITEMS are considered by the Board to be routine and will be
