Health Services Department, under the terms and conditions set forth in the lease.

designee, to execute a contract with the California Department of Aging to pay
County an amount not to exceed $74,078 for Supplemental Nutrition Assistance
Program-Education for the period July 1, 2014 through June 30, 2015. (No County

C. 32 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract amendment with the City of Antioch for its Community
Development Department, Block Grant funding, effective July 1, 2014, to pay the
County in an amount up to $8,500, for continuation of the operation of the Adult
Interim Housing Program, for the period July 1, 2014 through June 30, 2015. (No

C. 33 APPROVE and AUTHORIZE the Agricultural Commissioner, or designee, to
execute contract with the California Department of Food and Agriculture to

enforcement activities related to Sudden Oak Death for the period July 1, 2014

C. 34 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with the California Department of Resources Recycling and
Recovery, to pay County an amount not to exceed $26,603, for the continuation of
the Local Enforcement Agency assistance funds for the Department's
Environmental Health Division Solid Waste Program, for the period July 1, 2014

C. 40 APPROVE and AUTHORIZE the Public Works Director, or designee, to execute
a contract with ECS Imaging, Inc., in an amount not to exceed $289,714 to
implement Laserfiche, an electronic records content management system, for the

C. 41 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with K.H. Connie Wang-Chen, O.D., in an amount not to
exceed $125,000, to provide professional optometry services, for the period

C. 42 APPROVE and AUTHORIZE the County Clerk-Recorder, or designee, to execute
a contract amendment with SOE Software, Inc., to extend the term through August
20, 2015 and increase the payment limit by $51,742 to a new payment limit of

C. 43 APPROVE and AUTHORIZE the Purchasing Agent to execute, on behalf of the

execute a contract amendment with Thomas J. McDonald, M.D., effective May 1,
2014, to increase the payment limit by $10,805 to a new payment limit of
$464,805 to provide additional ophthalmology services with no change in the
original term of June 1, 2013 through May 31, 2014. (100% Enterprise Fund I)
C. 45 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with A.K. Bhattacharyya, M.D., Inc., in an amount not to
exceed $150,000, to provide professional neurology services to Contra Costa
Health Plan members and County recipients, for the period October 1, 2014

C. 46 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Taraneh Mostaghasi, M.D., Inc., in an amount not to
exceed $450,000, to provide professional pediatric primary care services for
Contra Costa Health Plan members, for the period September 1, 2014 through
August 31, 2016. (100% CCHP Enterprise Fund III)
C. 47 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Diablo Valley Pediatric Medical Group, Inc., in an amount
not to exceed $1,200,000, to provide professional pediatric primary care services
for Contra Costa Health Plan members, for the period September 1, 2014 through
August 31, 2016. (100% CCHP Enterprise Fund III)
C. 48 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Planned Parenthood: Shasta-Diablo, Inc. in an amount not
to exceed $1,800,000, to provide professional Obstetrics/ Gynecology and Family
Planning Services to CCHP members and County recipients, for the period
October 1, 2014 through September 30, 2015. (100% CCHP Enterprise Fund III)
C. 49 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Patrick Dixon, Ph.D., MFT, a self-employed individual in
an amount not to exceed $140,000, to provide outpatient psychotherapy services
for CCHP members and County recipients, for the period October 1, 2014 through
September 30, 2016. (100% CCHP Enterprise Fund III)
C. 50 APPROVE and AUTHORIZE the Health Services Director, or designee, to

C. 50 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract amendment with Aaron K. Hayashi, M.D., Inc., effective June
1, 2014, to increase the payment limit by $10,000 to a new payment limit of
$1,685,000 to provide additional professional radiology services at Contra Costa
Regional Medical and Health Centers, with no change in the original term of July
1, 2011 through June 30, 2014. (100% Enterprise Fund I)
C. 51 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Traditions Psychology Group, Inc (dba Traditions
Behavioral Health), in an amount not to exceed $21,450,000, to provide
psychiatric staffing and leadership for the Inpatient Psychiatric, Crisis
Stabilization Unit at Contra Costa Regional Medical Center and the County’s
Main Detention Facility, for the period September 1, 2014 through August 30,
2017. (100% Enterprise Fund I)
C. 52 APPROVE and AUTHORIZE the Purchasing Agent, on behalf of the Health
Services Department, to execute a purchase order with Epic Systems Corporation
in an amount not to exceed $8,160,000 for license and maintenance of the Epic
Electronic Health Record for Contra Costa Health Services, for the period July 1,
2014 through June 30, 2017. (100% Enterprise Fund I)
C. 53 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Thomas B. Hargrave, M.D., in an amount not to exceed
$342,000, to provide professional gastroenterology services at Contra Costa
Regional Medical and Health Centers, for the period September 1, 2014 through
August 31, 2017. (100% Enterprise Fund I)
C. 54 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a novation contract with R.E.A.C.H. Project in an amount not to exceed
$712,988, to provide drug abuse prevention and treatment services for youth and
adults in East County, for the period July 1, 2014 through June 30, 2015. (74%
Federal Substance Abuse Prevention and Treatment Set-Aside Grants; 10% Drug

execute a contract amendment with Advanced Medical Personnel Services, Inc.,

Medical and Health Centers, with no change in the original term of July 1, 2013

execute a contract amendment with SHC Services, Inc. (dba Supplemental Health
Care), effective September 1, 2014, to amend the contract to provide two
additional classifications to include Medical Social Worker and Medical Social
Worker Manager services at Contra Costa Regional Medical, Health Centers and
the Main Detention Facility, with no change in the original payment limit of
$3,173,000 and no change in the original term of April 1, 2013 through March 31,

execute a contract amendment with Jaison James, M.D., effective July 31, 2014,
to extend the term from July 31 through September 30, 2014 and increase the
payment by $150,000 to a new payment limit of $2,000,000 to provide additional
orthopedic services to Contra Costa Regional Medical and Health Centers. (100%
Enterprise Fund I)

Street, Martinez due to the Napa earthquake and flooding for the period September
1, 2014 through December 31, 2014. (100% County General Fund)
C. 65 RESCIND Board Order dated June 24, 2014 that authorized the County
Administrator, or designee, to execute a contract with Karpel Solutions in an
amount not to exceed $1,203,205 to provide a case management system for
District Attorney prosecution services; and APPROVE and AUTHORIZE the
County Administrator, or designee, to execute a contract with Karpel Solutions in
an amount not to exceed $1,223,205 to provide perpetual software licensing and
configuration, including four years of maintenance and on-site "go-live" support,
of a case management system for District Attorney prosecution services. (100%

execute a contract amendment with Keven J.Ormiston LLC, effective September
29, 2014, to extend the termination date from September 30, 2014 to September

$266,000 to provide continued Accela Automation Software, GIS (Geographic

period January 1, 2015 through June 30, 2015, and ADOPT related California
Environmental Quality Act findings, as recommended by the Conservation and

All matters listed under CONSENT ITEMS are considered by the Board to be routine and will be
enacted by one motion. There will be no separate discussion of these items unless requested by a
member of the Board or a member of the public prior to the time the Board votes on the motion to
adopt.
Persons who wish to speak on matters set for PUBLIC HEARINGS will be heard when the Chair
calls for comments from those persons who are in support thereof or in opposition thereto. After
persons have spoken, the hearing is closed and the matter is subject to discussion and action by the
Board. Comments on matters listed on the agenda or otherwise within the purview of the Board of
