the First Amendment to Lease with Shelter, Inc. of Contra Costa County to extend
the lease for a one-year term beginning July 1, 2014 and ending June 30, 2015 for
approximately 5,400 square feet of residential emergency shelter space located at
1391 Shell Avenue, Martinez, as requested by the Health Services Department,
under the terms and conditions set forth in the lease. (100% General Fund)

APPROVE and AUTHORIZE execution of agreements between the County and the
following agencies for receipt of fund and/or services:

accept a grant in the amount of $500 from Healthy and Active Before 5 to provide
nutrition programming to children at the El Cerrito Library for the period June 1,

designee, to execute a contract with the City of Richmond, Employment and
Training Department to pay County an amount not to exceed $100,000 for

execute a contract with Contra Costa County Conservation & Development
Department, in an amount not to exceed, up to $62,056, for CBGD funding to
operate County’s Health Services Homeless Adult Interim Housing program, for
the period from July 1, 2014, through June 30, 2015. (No County match)
C. 26 APPROVE and AUTHORIZE the Sheriff-Coroner, or designee, to execute a

Director, or designee, to accept Transportation Development Act Grant funds from

C. 28 ADOPT Resolution No. 2014/327 authorizing the Sheriff-Coroner, or designee, to
apply for and to accept State Homeland Security Grant Program (SHSGP) funds in

Sheriff's Office officials to act on behalf of the County to obtain funding for the

APPROVE and AUTHORIZE execution of agreement between the County and the

C. 31 RATIFY a Memorandum of Understanding, executed by the District Attorney,
with Howard University School of Law in an amount not to exceed $2,000 for a

C. 32 APPROVE and AUTHORIZE the Conservation and Development Interim
Director, or designee, to execute a contract amendment with Environmental
Science Associates to increase the payment limit by $80,268 to a new payment
limit of $201,148 for the continued assistance and technical services to complete
the Phillips 66 Propane Recovery Project Environmental Impact Report. (100%

Systems, and AT&T, and a third-party lease agreement with IBM Global Finance
in an amount not to exceed $895,167 for the purchase of hardware required to
maintain the County's wide area network for the period November 1, 2014 through
November 1, 2018, as recommended by the Public Works Director. (100%

C. 34 APPROVE and AUTHORIZE the Purchasing Agent, on behalf of the Health
Services Department, to execute a purchase order amendment with R-Computer,
LLC., to increase the amount payable by $379,000 to a new amount not to exceed
$739,000 for Hewlett Packard personal computer systems and related computer
hardware accessories, with no change in original term of September 25, 2013

C. 35 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Stephen B. Arnold, M.D., in an amount not to exceed
$1,455,000, to provide cardiology services at Contra Costa Regional Medical and
Health Centers, for the period from September 1, 2014 through August 31, 2017.

C. 36 APPROVE and AUTHORIZE the Purchasing agent, on behalf of the Health
Services Department, to execute purchase order amendment with Life Cell
Corporation in the amount of $135,000 for a new total of $175,000, for Alloderm

Centers, for the period from January 1, 2014 through December 31, 2016. (100%

C. 37 APPROVE and AUTHORIZE the Purchasing Agent, on behalf of the Health
Services Department, to execute purchase order with GE Healthcare, Inc. in an
amount not to exceed $190,000 for seven GE Infant Warmers for Contra Costa

C. 38 APPROVE and AUTHORIZE the Purchasing Agent, on behalf of the Health
Services Director, to execute purchase order with Applied Computer Solutions, in
an amount not to exceed $353,000 for hardware, support and maintenance, for the
period from September 30, 2014 through Sept 29, 2015. (100% Enterprise Fund I)
C. 39 APPROVE and AUTHORIZE the Purchasing Agent, on behalf of the Health
Services Director, to execute a purchase order with Craneware in an amount not to
exceed $195,000 for software licensing and maintenance, from August 15, 2014

to exceed $300,000 for scanners, printers, mobile devices, and minor hardware

C. 42 APPROVE and AUTHORIZE the Purchasing Agent, on behalf of the Health
Services Director, to execute a purchase order with Citrix Systems Inc. in an
amount not to exceed $300,000 for renewal of maintenance support for the ccLink
Electronic Health Record system, from September 30, 2014 through October 1,

C. 43 APPROVE and AUTHORIZE the Purchasing Agent to execute, on behalf of the
Chief Information Officer, a purchase order with Medtel Services, LLC, in an
amount not to exceed $140,000 to procure Medtel Arcata 2826 Dual Mode

C. 44 APPROVE and AUTHORIZE the Director of Child Support Services, or
designee, to execute a contract with Maximus Human Services, Inc., in an amount
not to exceed $750,000, to provide Early Intervention Delinquency Prevention
Program services for the period October 1, 2014 through September 30, 2015.

C. 45 APPROVE Board Order clarifying the Board's action of August 12, 2014 (C.98)
approving a contract with Bay Area Orthopedic Surgery and Sports to reflect the
proper legal name of the contractor (Teodoro P. Nissen, M.D., Inc., DBA Bay
Orthopedic Surgery and Sports Medicine) instead of Bay Area Orthopedic Surgery
and Sports Medicine, Inc., to provide Orthopedic Surgery services with no change
in the original term of the contract from August 1, 2014 through July 31, 2016.

Sheriff-Coroner, a purchase order with Bell Helicopter, in an amount not to exceed
$4,754,579 for the purchase of a Bell Standard 407GX Helicopter for the Office of
the Sheriff, as recommended by the Finance Committee. (82% General Fund, 18%

C. 49 ACCEPT the Treasurer’s Quarterly Investment Report as of June 30, 2014.
C. 50 APPROVE the revised bylaws for the Contra Costa Mental Health Commission,
as recommended by Health Services Director.
C. 51 AUTHORIZE the Chair of the Board of Supervisors to sign a letter of interest to
accept $250,000 in funding from the Board of State and Community Corrections
(BSCC) to provide grants to nongovernmental entities that provide community
recidivism and crime reduction services, as recommended by the Chief Probation
Officer. (100% State Revenue)
GENERAL INFORMATION
The Board meets in all its capacities pursuant to Ordinance Code Section 24-2.402, including as the
Housing Authority and the Successor Agency to the Redevelopment Agency. Persons who wish to
address the Board should complete the form provided for that purpose and furnish a copy of any
written statement to the Clerk.
Any disclosable public records related to an open session item on a regular meeting agenda and
distributed by the Clerk of the Board to a majority of the members of the Board of Supervisors less
than 72 hours prior to that meeting are available for public inspection at 651 Pine Street, First
Floor, Room 106, Martinez, CA 94553, during normal business hours.
All matters listed under CONSENT ITEMS are considered by the Board to be routine and will be
enacted by one motion. There will be no separate discussion of these items unless requested by a
member of the Board or a member of the public prior to the time the Board votes on the motion to
adopt.
Persons who wish to speak on matters set for PUBLIC HEARINGS will be heard when the Chair
calls for comments from those persons who are in support thereof or in opposition thereto. After
persons have spoken, the hearing is closed and the matter is subject to discussion and action by the
Board. Comments on matters listed on the agenda or otherwise within the purview of the Board of
Supervisors can be submitted to the office of the Clerk of the Board via mail: Board of
