APPROVE and AUTHORIZE execution of agreements between the County and the
following agencies for receipt of fund and/or services:
C. 24 APPROVE and AUTHORIZE the Conservation and Development Director, or
designee, to execute a tolling agreement with AT&T Mobility to extend the time to
act on the permit application for a new wireless telecommunication facility
proposed within the Oakley area from April 5 through June 16, 2014, and beyond
if necessary. (No fiscal impact)
C. 25 APPROVE and AUTHORIZE the District Attorney, or designee, to execute a
contract with the City of San Jose for reimbursement of Internet Crimes Against
Children Federal grant-related expenses for District Attorney staff to attend Silicon
Valley Internet Crimes Against Children Task Force training events, to remain
effective until notice of termination. (No fiscal impact)
C. 26 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Alameda County to pay the County an amount not to
exceed $51,000 for the provision of HIV laboratory testing services provided by
the County's Public Health Laboratory under the Therapeutic Monitoring Program,
for the period July 1, 2013 through March 31, 2014. (No County match)
C. 27 APPROVE and AUTHORIZE the Librarian, or designee, to apply for and accept a
grant in the amount of $6,000 from Target Corporation to provide early childhood
reading materials, equipment and programs for the Antioch, El Cerrito, and
Hercules libraries for the period September 1, 2014 through June 30, 2015. (No
Library Fund match)
C. 28 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Office of Statewide Health Planning and Development, to
pay the County an amount not to exceed $436,386 for the Peer Personnel
Preparation Project in Contra Costa County, for the period April 9, 2014 through
June 30, 2016. (No County match)
C. 29 APPROVE and AUTHORIZE the Health Services Director, or designee, to

execute a contract with the Contra Costa County Superior Court of California, to
pay the County an amount not to exceed $71,899, to support services provided to

Other Drug Services’ Drug Court Program for the period from October 1, 2013

APPROVE and AUTHORIZE execution of agreement between the County and the
following parties as noted for the purchase of equipment and/or services:

execute a contract with David A. Schwartz, M.D., in an amount not to exceed
$145,600 to provide outpatient psychiatric services for adults in Central Contra
Costa County, for the period May 1, 2014 through April 30, 2015. (100% Mental

C. 31 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with John Muir Health in an amount not to exceed $3,000 for
County’s use of a mobile van for the provision of health services in Contra Costa
County, for the period January 1 through December 31, 2014. (100% Federal
Healthcare for the Homeless Grant)
C. 32 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Scott Turpin, M.D., APC, in an amount not to exceed
$116,480 to provide professional outpatient psychiatric services to mentally ill
adults, for the period May 1, 2014 through April 30, 2015. (100% Mental Health
Realignment)
C. 33 APPROVE and AUTHORIZE the County Probation Officer, or designee, to
execute a contract with Behavioral Interventions Incorporated in an amount not to
exceed $175,000 to provide electronic monitoring equipment for the period
February 1, 2014 through January 31, 2015. (100% General Fund)
C. 34 APPROVE and AUTHORIZE the District Attorney, or designee, to execute an
Agreement and Addendum with the University of San Francisco School of Law,
including modified indemnification language, in an amount not to exceed $2,000
to fund a stipend for one student participating in the District Attorney's office
short-term training program for the period May 20, 2013 through June 1, 2014.

C. 35 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Daniel L. Zimmerman, M.D., in an amount not to exceed
$350,000 to provide professional obstetrics and gynecology services to Contra
Costa Health Plan members, for the period June 1, 2014 through May 31, 2016.

C. 36 AWARD and AUTHORIZE the Public Works Director, or designee, to execute a
contract in the amount of $656,000 with Sea Pac Engineering, Inc., the lowest
responsive and responsible bidder for the Boiler Replacement at 651 Pine Street,
Martinez Project, Countywide. (100% General Fund)
C. 37 APPROVE and AUTHORIZE the Purchasing Agent to execute, on behalf of the
Employment and Human Services Department, a purchase order with
ComputerLand of Silicon Valley in an amount not to exceed $161,848 to procure
new Cisco 3750X switches. (10% County; 45% State; 45% Federal)
C. 38 APPROVE and AUTHORIZE the Purchasing Agent to execute, on behalf of the
Employment and Human Services Department a purchase order with Carousel
Industries in the amount not to exceed $180,000 for a call center telephone system
software upgrade. (10% County; 45% State; 45% Federal)
C. 39 APPROVE and AUTHORIZE the Public Works Director, or designee, to execute
a contract with Estrada Consulting, Inc., in an amount not to exceed $230,000 to
address the issue of non-standardized and inaccessible asset management data, for
the period May 1, 2014 through April 30, 2016, Countywide. (73% General Fund
Venture Capital, 27% Various Special Revenue Funds)
C. 40 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Paul Reif, M.D., in an amount not to exceed $200,000 to
provide gastroenterology services at Contra Costa Regional Medical and Health
Centers, for the period June 1, 2014 through May 31, 2015. (100% Enterprise
Fund I)
C. 41 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Matthew Littlefield, M.D., in an amount not to exceed
$274,400 to provide professional psychiatric services for mentally ill adults in East
County, for the period July 1, 2014 through June 30, 2015. (100% Mental Health
Realignment)
C. 42 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Indra Singh, M.D., in an amount not to exceed $199,680,
to provide professional outpatient psychiatric services, for the period May 1, 2014
through April 30, 2015. (100% Mental Health Services Act)
C. 43 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with VVS Primary Care Medical Group, Inc., in an amount not
to exceed $200,000, to provide professional primary care services, for the period
May 1, 2014 through April 30, 2016. (100% Contra Costa Health Plan member
premiums)
C. 44 APPROVE and AUTHORIZE the Health Services Director, or designee, to

C. 44 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract amendment with Lesley Cantor, MFT, effective April 1, 2014,
to increase the payment limit by $42,000 to a new payment limit of $152,000 to
provide additional Medi-Cal specialty mental health services, with no change in
the original term of July 1, 2013 through June 30, 2015. (50% Federal Financial
Participation, 50% State General Fund)
C. 45 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract amendment with Contra Costa ARC, effective April 1, 2014, to
increase the payment limit by $25,500 to a new payment limit of $107,500 to
provide additional mental health services to CalWORKs recipients, with no
change in the original term of July 1, 2013 through June 30, 2014; and to increase
the automatic extension payment limit by $11,000 to a new payment limit of
$52,000, with no change in the term of the automatic extension through December
31, 2014. (100% CalWORKs)
C. 46 APPROVE and AUTHORIZE the Purchasing Agent to execute, on behalf of the
Health Services Department, a purchase order with Omnicell, Inc., in an amount
not to exceed $438,316 for Omnicell Pharmacy Automated Dispensing Systems at
Contra Costa Regional Medical and Health Centers, for the period of five years
from delivery of equipment. (100% Enterprise Fund I)
C. 47 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract amendment with Rubicon Programs Incorporated, effective
April 1, 2014, to increase the payment limit by $31,000 to a new payment limit of
$105,000 to provide additional mental health services to CalWORKs recipients,
with no change in the original term of July 1, 2013 through June 30, 2014; and to
increase the automatic extension payment limit by $6,000 to a new payment limit
of $43,000, with no change in the term of the automatic extension through
December 31, 2014. (100% CalWORKs)
C. 48 APPROVE and AUTHORIZE the Chief Information Officer, or designee, to
execute a Memorandum of Agreement with The Aquatic Science Center Institute
in an amount not to exceed $137,975 to provide funding for the purchase of
electronic aerial imagery (orthophotos) of Contra Costa County that will be made
available to the County's Geographic Information System via Photo Science, a
Quantum Spatial company, under its contract with the San Francisco Estuary
Institute. (100% County General Fund, budgeted)
Other Actions
C. 49 ADOPT Resolution No. 2014/92 supporting the need for reforms for the
intelligence community to support privacy, open government, government
transparency, and accountability, as recommended by the Contra Costa County
Library Commission. (No fiscal impact)
C. 50 APPROVE the list of providers recommended by the Contra Costa Health Plan's
May 6, 2014 Contra Costa Board of Supervisors 9
C. 50 APPROVE the list of providers recommended by the Contra Costa Health Plan's
Peer Review and Credentialing Committee on April 8, 2014 and by the Health
Services Director, as required by the State Departments of Health Care Services
and Managed Health Care, and the Centers for Medicare and Medicaid Services.

C. 51 REFER to the Internal Operations Committee an evaluation of a Private Parking
Facility ordinance in the unincorporated areas of Contra Costa County as
recommended by Supervisor Mitchoff.
C. 52 APPROVE allocation of fiscal year 2014/15 Community Development Block
Grant, HOME Investment Partnerships Act, Housing Opportunities for Persons
with AIDS, and Emergency Solutions Grant Program funds as recommended by
the Family and Human Services Committee and the Conservation and
Development Director, ADOPT related California Environmental Quality Act
findings, and DIRECT the Conservation and Development Director to execute
related administrative actions implementing each program. (100% Federal funds)
C. 53 CONTINUE the emergency action originally taken by the Board of Supervisors on
November 16, 1999 regarding the issue of homelessness in Contra Costa County,
as recommended by the Health Services Director. (No fiscal impact)
C. 54 REFER to the Finance Committee of the Board of Supervisors a review of the
proposed Regional Renewable Energy Procurement Solar Project, as
recommended by the County Administrator.
Successor Agency to the Contra Costa County Redevelopment Agency
C. 55 AUTHORIZE a “yes” vote for the proposed property-related fee to be assessed
under the Fire Protection and Emergency Response Services Assessment by the
Rodeo-Hercules Fire Protection District for four housing asset parcels owned by
the former Contra Costa County Redevelopment Agency, as recommended by the
Conservation and Development Director.
GENERAL INFORMATION
The Board meets in all its capacities pursuant to Ordinance Code Section 24-2.402, including as the
Housing Authority and the Successor Agency to the Redevelopment Agency. Persons who wish to
address the Board should complete the form provided for that purpose and furnish a copy of any
written statement to the Clerk.
Any disclosable public records related to an open session item on a regular meeting agenda and
distributed by the Clerk of the Board to a majority of the members of the Board of Supervisors less
than 72 hours prior to that meeting are available for public inspection at 651 Pine Street, First
Floor, Room 106, Martinez, CA 94553, during normal business hours.
All matters listed under CONSENT ITEMS are considered by the Board to be routine and will be
enacted by one motion. There will be no separate discussion of these items unless requested by a

enacted by one motion. There will be no separate discussion of these items unless requested by a
member of the Board or a member of the public prior to the time the Board votes on the motion to
adopt.
Persons who wish to speak on matters set for PUBLIC HEARINGS will be heard when the Chair
calls for comments from those persons who are in support thereof or in opposition thereto. After
persons have spoken, the hearing is closed and the matter is subject to discussion and action by the
Board. Comments on matters listed on the agenda or otherwise within the purview of the Board of
