amount not to exceed $15,000, for the Independent Living Skills Program, Senior

contracts with the cities of Richmond, Concord, Pittsburg, and Antioch in an
aggregate amount not to exceed $522,000 to provide AB 109 police services for

C. 16 APPROVE and AUTHORIZE the Public Works Director, or designee, to apply for
and accept a grant in the amount of $35,000 from the Contra Costa County Keller
Canyon Mitigation Trust Fund for the roadside cleanup of unincorporated areas of
District V in order to provide cleaner, safer communities to residents for the period
of July 30, 2016 through June 30, 2017, Bay Point area. (100% Keller Canyon

C. 17 APPROVE and AUTHORIZE the County Clerk-Recorder, or designee, to apply
for and accept a grant in an amount up to $10,000 from the Contra Costa County
Keller Canyon Mitigation Trust Fund for the Neighborhood Engagement Project.

C. 18 APPROVE and AUTHORIZE the Employment and Human Services Director, or
designee, to apply for and accept California Governor's Office of Emergency
Services grant funding, in an amount not to exceed $957,742, for Elder Abuse

C. 19 APPROVE clarification of Board action of January 5, 2016 (Item C.38), which
approved and authorized the Employment and Human Services Director, or

Edward Byrne Memorial Justice Assistance Grant (JAG), to change the funding
payment limit from $983,971 to $1,786,917, with no change in the term dates and
ADOPT Resolution No. 2016/399 rescinding and superseding Resolution No.

C. 20 APPROVE and AUTHORIZE the Health Service Director, or designee, to accept
Transportation Development Act Grant funds from the Metropolitan
Transportation Commission for the Bicycle and Pedestrian Safety Education
Project, to pay County an amount not to exceed $40,000 for the period July 1,
2016 through June 30, 2017. ($17,624 General Fund match required)
APPROVE and AUTHORIZE execution of agreement between the County and the
following parties as noted for the purchase of equipment and/or services:
C. 21 APPROVE and AUTHORIZE the Employment and Human Service Director, or
designee, to reimburse employers for participating in the CCWORKS Subsidized
Employment Program, including contractual agreements with employers to
reimburse no less than $10 per hour, and not to exceed $20 per hour for those
CCWORKS clients working with employers during the period July 1, 2016
through June 30, 2017. (100% Federal)
C. 22 APPROVE and AUTHORIZE the Employment and Human Services Director, or
designee, to execute a contract with the California State Association of Counties in
an amount not to exceed $818,324 to administer computerized data processing
services for the period July 1, 2016 through June 30, 2019. (10% County, 45%

C. 23 APPROVE and AUTHORIZE the County Administrator to execute a contract
amendment with KMI Human Resources Consulting, Inc., to extend the term from
June 30, 2016 to June 30, 2017 and increase the payment limit by $575,000 to a
new payment limit of $1,150,000 to continue providing specialized administrative
services. (100% General Fund)
C. 24 APPROVE and AUTHORIZE the Purchasing Agent, or designee, on behalf of the
Director of Child Support Services, to execute a purchase order with R-Computer,
in an amount not to exceed $200,000, to lease/purchase network routing and
switching hardware. (66% Federal, 34% State)
C. 25 APPROVE and AUTHORIZE the Purchasing Agent to execute, on behalf of the
Public Works Director, a contract amendment with Crusader Fence Co. Inc., to
increase the payment limit by $130,000, to a new payment limit of $430,000 for
fencing installation and repairs, with no change to the original term of July 9, 2013
through June 30, 2016, Countywide. (100% General Fund)
C. 26 APPROVE and AUTHORIZE the Chief Information Officer-Department of
Information Technology, or designee, to execute a premier support services
agreement with Microsoft Corp., in an amount not to exceed $299,608 to provide
Microsoft premier support services for the period June 13, 2016 through June 12,

designee, to execute a contract with Steve Gray, dba Steve Gray Consulting and
Training, in an amount not to exceed $100,000, to provide domestic violence
consultation services for the period July 1, 2016 through June 30, 2018. (10%
County; 45% State; 45% Federal)
C. 28 APPROVE and AUTHORIZE the Employment and Human Services Director, or
designee, to execute a contract with Goodwill Industries of the Greater East Bay in
an amount not to exceed $1,486,248 for the Subsidized Temporary Experience
with Pay for the Under-Employed Program (STEP-UP) for the period July 1, 2016
through June 30, 2017. (100% Federal)
C. 29 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Merritt, Hawkins and Associates, LLC, in an amount not to
exceed $300,000, to recruit family practice physicians at Contra Costa Regional
Medical and Health Centers, for the period July 1, 2016 through June 30, 2017.

C. 30 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Concord Yellow Cab, Inc., in an amount not to exceed
$300,000, to provide non-emergency taxicab transportation services to patients to
and from Contra Costa Regional Medical, Contra Costa Health Centers and other
County facilities for the period from July 1, 2016 through June 30, 2017. (100%
Hospital Enterprise Fund I)
C. 31 APPROVE and AUTHORIZE the Purchasing Agent, on behalf of the
Employment and Human Services Department, to execute a purchase order with
R-Computer, Inc., in the amount not to exceed $123,911, to procure computer
hardware required to implement an intake and interview booth; client review and
touch screen signature program for Social Services Benefit applications during the
period of June 1, 2016 through December 31, 2016. (100% County Venture
Capital Funds)
C. 32 APPROVE and AUTHORIZE the Conservation and Development Director, or
designee, to execute a contract amendment with McHale's Environmental
Insulation, Inc. to increase the payment amount by $105,000 to a new payment
limit of $200,000 to purchase items and/or services needed to provide home
weatherization services to low income residents throughout Contra Costa County,
and extend the contract period from December 31, 2016 through December 31,
2017. (100% State and Federal Weatherization Program funds)
C. 33 APPROVE and AUTHORIZE the Employment and Human Services Director, or
designee, to execute a contract with Aspiranet in an amount not to exceed
$396,418, to provide emergency shelter receiving center services in Central
Contra Costa County for children taken into protective custody or transitioning
through foster placements for the period July 1, 2016 through June 30, 2017. (70%
State; 30% County)

C. 34 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a novation contract, containing modified indemnification language, with
Meals on Wheels and Senior Outreach Services of Contra Costa in an amount not
to exceed $177,921 to provide congregate meals for the Senior Nutrition Program
for the period from July 1, 2016 through June 30, 2017, with a three-month
automatic extension through September 30, 2017 in an amount not to exceed
$44,480. (100% Federal Title III-C1 of the Older Americans Act of 1965)
C. 35 APPROVE and AUTHORIZE the Purchasing Agent, on behalf of the Health
Services Department, to execute a purchase order with Carefusion Inc. in the
amount of $127,465 for medication delivery equipment for the Contra Costa
Regional Medical Center and Health Centers for the period from May 1, 2016
through November 30, 2016. (100% Hospital Enterprise Fund I)
C. 36 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a novation contract with Meals on Wheels and Senior Outreach Services
in an amount not to exceed $366,495 to provide home-delivered meals for the
Senior Nutrition Program, for the period July 1, 2016 through June 30, 2017, with
a three-month automatic extension through September 30, 2017, in an amount not
to exceed $91,624. (100% federal Title IIIC 2 of the Older Americans Act of 1965)
C. 37 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a novation contract, containing modified indemnification language, with
Neighborhood House of North Richmond in an amount not to exceed $13,000 to
provide congregate meals for the Senior Nutrition Program for the period from
July 1, 2016 through June 30, 2017, with a three-month automatic extension
through September 30, 2017 in an amount not to exceed $3,250. (100% Federal
Title III C-1 of the Older Americans Act of 1965)
C. 38 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Public Health Foundation Enterprises, Inc., in an amount
not to exceed $252,629 to provide coordination, supervision, and support to the
County’s Senior Nutrition Program for the period July 1, 2016 through June 30,
2017, including a three-month automatic extension through September 30, 2017,
in an amount not to exceed $63,157. (12% Federal Title IIIC (1) of the Older
Americans Act of 1965, 88% Federal Title IIIC (2) of the Older Americans Act of
1965)
C. 39 APPROVE and AUTHORIZE the Employment and Human Services Director, or
designee, to execute a contract with Youth Homes Incorporated, in an amount not
to exceed $200,000, to provide group home placement services to foster youth for
the period July 1, 2016 through June 30, 2017. (70% State; 30% County)
C. 40 APPROVE and AUTHORIZE the Employment and Human Services Director, or

C. 40 APPROVE and AUTHORIZE the Employment and Human Services Director, or
designee, to execute a contract with Lutheran Social Services of Northern
California, in an amount not to exceed $172,159, to provide transitional housing
assistance for emancipated youth, for the period July 1, 2016 through June 30,
2017. (100% State)
C. 41 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract amendment with Docustream, Inc., effective June 1, 2016, to
increase the payment limit by $223,000 to a new payment limit of $870,000 to
provide additional electronic claims processing services due to an increase in
CCHP membership, changes in forms, processing of ambulance claims and
meeting additional state requirements, with no change in the original term of
November 1, 2015 through October 31, 2016. (81% CCHP Enterprise Fund II;
19% Mental Health Services Act)
C. 42 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Bailey T. Lee, M.D., in an amount not to exceed $564,000
to provide radiology services at Contra Costa Regional Medical Center and Health
Centers for the period July 1, 2016 through June 30, 2019. (100% Hospital
Enterprise Fund I)
C. 43 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Advanced Medical Personnel Services, Inc. in an amount
not to exceed $1,200,000 to provide temporary therapists for Contra Costa
Regional Medical and Contra Costa Health Centers for the period July 1, 2016
through June 30, 2017. (100% Hospital Enterprise Fund I)
C. 44 APPROVE and AUTHORIZE the Chief Information Officer-Department of
Information Technology, or designee, to execute a contract amendment with
Genesis Data, Inc., effective June 14, 2016, to increase the payment limit by
$606,576 to a new payment limit of $1,386,576 to provide data entry services for
the period July 1, 2016 through June 30, 2018. (100% Department User fees)

C. 45 APPROVE and AUTHORIZE the County Clerk-Recorder, or designee, to
authorize expenditures and reimbursements for the Civic Education and
Engagement Program for a total amount not to exceed $15,000 annually. (100%

C. 46 APPROVE and AUTHORIZE the Employment and Human Services Director, or
designee, to continue the expanded CalWORKs Subsidized Employment Program
to provide reimbursement in an amount not to exceed $1,000,000 to employers
who hire CalWORKs participants for the period July 1, 2016 through June 30,

C. 47 DECLARE as surplus and AUTHORIZE the Purchasing Agent, or designee, to

dispose of fully depreciated vehicles and equipment no longer needed for public

Request for Qualifications for architectural services to conduct a planning study in
an amount not to exceed $750,000, for capital facility projects including the
potential construction of new County Buildings, a new Emergency Operations
Center and other significant facility construction and/or renovation. (100%

C. 49 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute an unpaid student training agreement with Diablo Medical Training, to
provide supervised field instruction to medical residency students at Contra Costa
Regional Medical Center and Health Centers for the period May 1, 2016 through
April 30, 2017. (Non-financial agreement)
C. 50 AUTHORIZE discharge from accountability for certain Public Works Department
accounts totaling $105,440, that have been determined to be uncollectible, as
recommended by the Public Works Director, Countywide. (29% Local Road
Funds and 71% Land Development Funds)
C. 51 ALLOCATE $1,432,830 from the Livable Communities Trust to the Heritage
Point Affordable Housing Project in North Richmond and DIRECT the Director of
the Department of Conservation and Development to incorporate the terms and
conditions of the funds into a Disposition and Development Agreement between
the County and Community Housing and Development Corporation of North
Richmond for the project, as recommended by Supervisor Gioia. (Consider with
C.53)
C. 52 RECEIVE Civil Grand Jury Report No. 1610, entitled "Contra Costa County
Office of the Public Defender: Who is Entitled to Public Legal Representation?"
(attached), and FORWARD to the County Administrator for response. (No fiscal
impact)
C. 53 ADOPT Resolution No. 2016/411 approving and authorizing the Conservation
and Development Director, or designee, to submit a grant application to the
Strategic Growth Council/California Department of Housing and Community
Development Affordable Housing and Sustainable Communities program in an
amount not to exceed $12,094,780 to construct the Heritage Point Multiple-Family
Housing Project in North Richmond. (Consider with C.51)
C. 54 APPROVE and AUTHORIZE the Health Services Director, or designee, to
June 14, 2016 Contra Costa County Board of Supervisors 10
C. 54 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute an unpaid student training agreement with San Diego State University
Foundation for its Women, Infant and Children Dietetic Internship Program, to
provide supervised field instruction to dietitian students, for the period from
January 1, 2016 through December 31, 2016. (Non-financial agreement)
C. 55 RECEIVE Civil Grand Jury Report No. 1606, entitled "Reclaiming Our Water"
and refer it to the County Administrator, as recommended by the County
Administrator.
C. 56 ACCEPT and APPROVE the 2016 program year annual Head Start
self-assessment as submitted and as recommended by Employment and Human
Services Director.

The Board meets in all its capacities pursuant to Ordinance Code Section 24-2.402, including as the
Housing Authority and the Successor Agency to the Redevelopment Agency. Persons who wish to
address the Board should complete the form provided for that purpose and furnish a copy of any
written statement to the Clerk.
Any disclosable public records related to an open session item on a regular meeting agenda and
distributed by the Clerk of the Board to a majority of the members of the Board of Supervisors less

All matters listed under CONSENT ITEMS are considered by the Board to be routine and will be
enacted by one motion. There will be no separate discussion of these items unless requested by a
member of the Board or a member of the public prior to the time the Board votes on the motion to
adopt.
Persons who wish to speak on matters set for PUBLIC HEARINGS will be heard when the Chair
calls for comments from those persons who are in support thereof or in opposition thereto. After
persons have spoken, the hearing is closed and the matter is subject to discussion and action by the
Board. Comments on matters listed on the agenda or otherwise within the purview of the Board of
Supervisors can be submitted to the office of the Clerk of the Board via mail: Board of
