On-Call Contract(s) for Various Road and Flood Control Maintenance Work,

C. 2 APPROVE and AUTHORIZE the Public Works Director, or designee, to execute
a contract with Vali Cooper & Associates, Inc., in an amount not to exceed
$200,000 for construction management services for the Camino Tassajara
Shoulder Widening Project, for the period of May 24, 2016 through project
closeout, San Ramon area. (46% Highway Safety Improvement Funds, 54% South

a contract amendment with LSA Associates, Inc., effective June 30, 2016, to

associated with the Marsh Creek Road Bridge Replacement-Environmental
Technical Studies Project, Clayton area. (89% Federal Highway Bridge Program

C. 9 ACCEPT the Grant Deed for a 120-acre land donation from Wildlands, Inc., to

Director of Airports, a purchase order amendment with Ascent Aviation Group,
Inc., to increase the payment limit by $75,000 to a new payment limit of $305,000
for aviation fuel at the Byron Airport, with no change to the original term
September 1, 2015 through August 31, 2016. (100% Airport Enterprise Fund)

County, a partial settlement in the class action lawsuit, In Re Lithium Ion Batteries
Antitrust Litigation (U.S.D.C. Case No. 13-md-02420-YGR, N.D. Cal.), as
recommended by the Purchasing Agent. (100% General Fund)
C. 13 RECEIVE report concerning the final settlement of Kim Haramaki vs. Contra
Costa County Fire Protection District; and AUTHORIZE payment from the
Workers' Compensation Internal Service Fund in an amount not to exceed $85,000
less permanent disability payments, as recommended by the Risk Management

C. 22 REAPPOINT Thomas Weber to the BOS Appointee 1 seat on the Airport Land
Use Commission, as recommended by the Internal Operations Committee.
C. 23 APPOINT Bijal Patel to the Community #2 seat, and REAPPOINT Lisa Caronna
to the Community #3 seat and Tom Shephard to the County #3 seat on the

Appropriation Adjustments
C. 26 Employment and Human Services (0503)/Plant Acquisition (0111): APPROVE
FY 2015/16 Appropriation Adjustment No. 5076 authorizing the transfer of
appropriations in the amount of $938,000 from EHSD - Aging & Adult Services to
the Public Works Department for four capital projects, as recommended by the
Employment and Human Services Director.
Intergovernmental Relations
C. 27 ADOPT a "Support in concept" the "No Place Like Home" proposal developed in
the Senate to redirect a portion of Mental Health Services Act (MHSA, or
Proposition 63) funding from counties to help securitize up to $2 billion in
affordable housing bonds to address the persistent issue of homelessness through
the State, as recommended by the County Administrator.

C. 28 ADOPT Position Adjustment Resolution No. 21865 to establish the class of

Public Health Lead Mobile Clinic Operator (represented) and reclassify one (1)

C. 29 ADOPT Position Adjustment Resolution No. 21860 to add two Eligibility
Supervisor I positions (represented) and cancel two vacant Social Work

Grants & Contracts
APPROVE and AUTHORIZE execution of agreements between the County and the
following agencies for receipt of fund and/or services:
C. 30 APPROVE and AUTHORIZE the County Administrator or designee to apply for
and accept, on behalf of the Library, a grant in the amount of $26,250 from Keller
Canyon Mitigation Trust Fund to provide security services at the Antioch
Libraries, for the period July 1, 2016 through June 30 2017. (No Library Fund
match)
C. 31 APPROVE and AUTHORIZE the Health Services Director, or designee, to accept
a grant award from the U. S. Department of Health and Human Services (Health
Resources & Services Administration) to pay the County an amount not to exceed
$111,449 for the continuation of Ryan White, Part C, HIV early intervention
services, for the period May 1, 2016 through April 30, 2017. (No County match)
C. 32 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract containing modified indemnification language with the County
of Alameda Health Care Services Agency, to allow Contra Costa County to
receive an amount up to $1,379,786 in funding for coordination of essential
services to Contra Costa County residents with HIV Disease and their families, for
the period March 1, 2016 through February 28, 2017. (No County match)
C. 33 APPROVE and AUTHORIZE the Health Services Director, or designee, to submit
a funding application to the California Department of Resources Recycling and
Recovery (CalRecycle) to pay County an amount not to exceed $30,000 for the
continuation of the Local Enforcement Agency assistance funds for the
Department's Environmental Health Division Solid Waste Program, for the period
July 1, 2016 through October 30, 2017. (No County match)
C. 34 APPROVE and AUTHORIZE the County Veterans Services Officer or designee
to apply for, and execute a contract to accept, grant funding from the California
Department of veterans Affairs in an amount not to exceed $22,500 to provide
mental health outreach and support services for a period July 1, 2016 through June
30, 2017. (No County match)

C. 35 APPROVE and AUTHORIZE the Employment and Human Services Director, or
designee, to execute a contract containing modified indemnification language with
California Department of Community Services and Development, to pay the
County an amount not to exceed $115,014 to provide Department of Energy
Weatherization Assistance Program for the period July 1, 2016 through January
31, 2017. (No County match)
C. 36 APPROVE and AUTHORIZE the Child Support Services Director, or designee, to
execute a contract with Riverside County, to pay the County an amount not to
exceed $85,000 for delinquent payment early intervention services for the period
May 1 through September 30, 2016. (No County match)
C. 37 APPROVE and AUTHORIZE the Employment and Human Services Director, or
designee, to submit an application to and accept funding from the Department of
Health and Human Services Administration for Children and Families in an
amount not to exceed $280,282 for Head Start supplemental funding for the period
January 1 through December 31, 2016. (20% County in-kind match)
C. 38 APPROVE and AUTHORIZE the Employment and Human Services Director, or
designee, to submit an application to and accept funding from the Department of
Health and Human Services Administration for Children and Families in an
amount not to exceed $60,496 for Early Head Start supplemental funding for the
period January 1 through December 31, 2016. (20% County in-kind match)
C. 39 APPROVE and AUTHORIZE the Employment and Human Services Director, or
designee, to submit an application to and accept funding from the Department of
Health and Human Services Administration for Children and Families in an
amount not to exceed $18,653 for Early Head Start supplemental funding for the
period January 1 through December 31, 2016. (20% County in-kind match)
C. 40 APPROVE and AUTHORIZE the Employment and Human Services Director, or
designee, to execute a contract amendment with California Department of
Education, effective February 1, 2016, to increase the payment limit by $23,000
for a new payment limit of $4,709,720, to provide for childcare and development
programs (CalWORKS Stage 2), with no change to the term of July 1, 2015
through June 30, 2016. (No County match)
C. 41 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with the West Contra Costa Unified School District, to pay the
County an amount not to exceed $5,000 to provide mental health services to
students and their families for the period July 1, 2016 through June 30, 2017. (No
County match)
APPROVE and AUTHORIZE execution of agreement between the County and the
following parties as noted for the purchase of equipment and/or services:

C. 42 APPROVE and AUTHORIZE the Conservation and Development Director, or
designee, to execute a contract amendment with Circlepoint, to extend the term
from December 31, 2016 through December 31, 2017 and increase the payment
limit by $36,000 to a new payment limit of $160,990 to provide continued service
to complete the Environmental Impact Report for the Ball Estate Subdivision.

C. 43 APPROVE and AUTHORIZE the Agricultural Commissioner, or designee, to
execute a contract with the Regents of the University of California in an amount
not to exceed $48,000 for Master Gardener Program coordination services, for the
period July 1, 2016 through June 30, 2017. (100% County)
C. 44 APPROVE and AUTHORIZE the Interim County Probation Officer, or designee,
to apply for and accept a grant from the U.S. Department of Justice, Office of
Justice Programs, Bureau of Justice Assistance, for a Second Chance Act Smart
Reentry Program for Planning and Demonstration Projects in an amount not to
exceed $1,000,000 for the development and implementation of Evidence-based
Strategies for Successful Reentry from Incarceration to Community for the period
October 1, 2016 through September 30, 2019. (100% County match)
C. 45 RATIFY the agreement reached between East Bay Perinatal Medical Associates
and the Health Services Director to terminate contractual obligations for the
provision of Maternal-Fetal Medicine services effective December 31, 2015, as
recommended by the Health Services Director. (100% Contra Costa Health Plan
Enterprise Fund II)
C. 46 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract containing modified indemnification language with Irvington
Group, Inc. (dba IgMedical), in an amount not to exceed $90,000 to provide
recruitment services for the pharmacy department at Contra Costa Regional
Medical and Health Centers, for the period May 1, 2016 through April 30, 2017.

C. 47 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract containing modified indemnification language with the Regents
of the University of California, on behalf of its University of California San
Francisco campus, in an amount not to exceed $7,524 to provide family-based
treatment training for eating disorders for the period December 1, 2015 through
November 30, 2016. (100% Mental Health Realignment funds)
C. 48 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Dayana Carcamo-Molina, M.D., Inc., in an amount not to
exceed $1,620,000 to provide gastroenterology services at Contra Costa Regional
Medical and Health Centers for the period June 1, 2016 through May 31, 2019.

C. 49 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Per Diem Staffing Systems, Inc., in an amount not to
exceed $1,100,000 to provide temporary respiratory therapists, nursing services
and other medical classifications at Contra Costa Regional Medical and Health
Centers for the period July 1, 2016 through June 30, 2017. (100% Hospital
Enterprise Fund I)
C. 50 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Infectious Disease Doctors Medical Group, APC, in an
amount not to exceed $250,000 to provide infectious disease consulting services
and training at Contra Costa Regional Medical and Health Centers for the period
May 1, 2016 through April 30, 2017. (100% Hospital Enterprise Fund I)
C. 51 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract amendment with Bailey T. Lee, M.D., effective May 1, 2016, to
increase the payment limit by $28,000 to a new payment limit of $528,000 to
provide additional hours of radiology services at Contra Costa Regional Medical
and Health Centers, with no change in the original term of July 1, 2013 through
June 30, 2016. (100% Hospital Enterprise Fund I)
C. 52 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with the City of San Ramon, to pay the County an amount not
to exceed $10,000 to provide home-delivered and congregate meal services for
County’s Senior Nutrition Program for the period July 1, 2016 through June 30,
2017, with a three-month automatic extension through September 30, 2017 in an
amount not to exceed $2,500. (No County match)
C. 53 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Golden Rain Foundation of Walnut Creek, to pay the
County an amount not to exceed $16,142, to provide congregate meal services for
the County’s Senior Nutrition Program for the period July 1, 2016 through June
30, 2017, with a three-month automatic extension through September 30, 2017 in
an amount not to exceed $4,036. (No County match)
C. 54 APPROVE and AUTHORIZE the Interim County Probation Officer, or designee,
to execute a contract amendment with Rubicon Programs to extend the term from
July 31, 2015 through June 30, 2016 with no change to the payment limit, to
provide reentry services to probationers enrolled in the Second Chance Reentry
Initiative. (100% Federal)
C. 55 APPROVE and AUTHORIZE the County Administrator, or designee, to execute
contract amendments with Reach Fellowship International and Men and Women
of Purpose, to extend the term from June 30, 2016 to June 30, 2017 and increase
the total payment limit by $100,000 to a new total payment limit of $200,000 for
employment/education liaison services in the Central/East Network Reentry
System of Services for the County’s AB 109 Realignment Program. (100% Public
Safety Realignment)

C. 56 APPROVE and AUTHORIZE the Employment and Human Services Director, or
designee, to execute a contract with Language Line Services, Inc., in an amount
not to exceed $1,500,000 to provide telephone interpretation, on-site
interpretation, and document translation services for the period July 1, 2016
through June 30, 2017. (10% County, 45% State 45% Federal)
C. 57 APPROVE and AUTHORIZE the Purchasing Agent or designee to execute, on
behalf of the Public Works Director, a purchase order amendment with Royal
Wholesale Electric Co., to increase the payment limit by $230,000 to a new
payment limit of $239,900 for will-call electrical parts and supplies, with no
change to the original term of May 1, 2016 through April 30, 2019, Countywide.

C. 58 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Discharge Resource Group (dba DRG Health Care
Staffing) in an amount not to exceed $190,000 to provide temporary occupational
and physical therapy services for the County’s California Children Services
Program, for the period July 1, 2016 through June 30, 2017. (50% State California
Children’s Services; 50% County funds)
C. 59 APPROVE and AUTHORIZE the Employment and Human Services Director, or
designee, to execute a contract with Child Abuse Prevention Council in an amount

annual Fall Forum event for law enforcement agencies to discuss fish and wildlife
conservation and enforcement of regulations in Contra Costa County and
AUTHORIZE the Auditor-Controller to release up to $2,000 from the Fish and
Wildlife Propagation Fund to offset costs of the event, as recommended by the
Fish and Wildlife Committee. (100% Fish and Wildlife Propagation Fund)
C. 96 RECEIVE Civil Grand Jury Report No. 1603, "Pension Reform - If Not Now,
When?", and FORWARD to the County Administrator for response, as
recommended by the County Administrator.

distributed by the Clerk of the Board to a majority of the members of the Board of Supervisors less

All matters listed under CONSENT ITEMS are considered by the Board to be routine and will be
enacted by one motion. There will be no separate discussion of these items unless requested by a
member of the Board or a member of the public prior to the time the Board votes on the motion to
adopt.
Persons who wish to speak on matters set for PUBLIC HEARINGS will be heard when the Chair
calls for comments from those persons who are in support thereof or in opposition thereto. After
persons have spoken, the hearing is closed and the matter is subject to discussion and action by the
