(per week) This position provides on-call relief coverage (sick leave, vacation holiday)

' at the County Hospital as well as the Central County CMHC. Job varies
according to location.
On Inpatient Units:
5 hrs. Attend ward rounds 0 th treatment team to conference each patient; present
`;'... relevant patient information; develop treatment and disposition Plan.
8 hrs. Evaluate patients upon intake to assess treatment and disposition needs.
8 hrs. Individual therapy/counseling y/counseling sessions with

of disposition plans.
6 hrs.. Charting of patient progress/disposition plans/paperwork related to disposition.
S 5 hrs. Work with resource specialist/community agencies regarding placement.
3 hrs. Lead psychotherapy groups and community meetings on ward.
J 3 hrs. Meet with patient families for disposition conferences/family therapy.
2 hrs. Represent hospital in Probable Cause (Gallinot) Hearings.
On Mental Health Screening (requires licensed clinician):
18 hrs. Interview patients in crisis, who come in on voluntary or involuntary
basis. Evaluate immediate and longer-term psychiatric/placement needs.
Meet with families and collateral workers (or initiate phone contact)
to obtain necessary information regarding patient history and behavior.
Conduct crisis intervention with patient and family. Initiate disposition
plan: referral to other agency, psychiatrist for medication, inpatient unit, etc.
\ 8 hrs. Phone calls/conferences/paperwork required to obtain appropriate disposition
C_Jand continued treatment for patient.
8 hrs. Write up comprehensive intake evaluations of all patients seen/misc. paperwork.
3 hrs. Follow-up therapy sessions with patients.
3 hrs. Interview patients on medical wards regarding psychiatric needs and consult

(per 4 hr. At Crossroads Center-Concord CMHC (requires licensed clinician):_
shift)
2y hr. Interview drop-in patients seeking therapy or in crisis; evaluate
patients; make appropriate referrals to outpatient therapy, psychiatrist
for medication evaluation, hopsitalization, community placement, other
resources. Conduct psychotherapy with ongoing patients.
3/4 hr. Paperwork - charting of client contacts, paperwork related to referrals.
3/4 hr. Phone contact with county/community agencies to initiate appropriate
aftercare referrals.
