RECOMMENDATIONS)
ADOPT Resolution No. 2008/593 approving and authorizing the Employment and Human Services Director,
or designee, to execute a contract with the State of California, Department of Aging (MS-0809-27), in the
amount of$321,375, for the Multipurpose Senior Service Program for the period July 1, 2008 through
November 30, 2008.
FISCAL IMPACT
Per State of California, Department of Aging Agreement (MS-0809-27) for the Multipurpose Senior Service
Program (MSSP), County to receive funding in the amount not to exceed $371,375: No County match
required.

BACKGROUND
The State of California, Department of Aging funding is yearly funding accepted by the Department on behalf
of the County to provide case management on a countywide basis for Contra Costa County residents age 65
years and over, Medi-Cal eligible, and certifiable for skilled nursing facility care.
The Multipurpose Senior Service Program (MSSP) provides case management, assessment, care planning,
service arrangements, and Client monitoring through a team of health and social service providers. The
provision of case management services to this senior population allows individuals who may otherwise require
institutionalization, to remain safely in their own residences.
