FISCAL IMPACT: $1,534,626: 21% County Funding; 35% Federal Targeted Case Management Funding (93.778); 44% State First Five Commission and Contra Costa Futures Grant. BACKGROUND: Aspiranet, formerly Moss Beach, Inc., will have a fiduciary relationship to the Welcome Home Baby Program (WHB), with responsibilities of fiscal administration including human resource management, while EHSD will remain the primary lead for program decisions and expansion. The Welcome Home Baby program provides up to three years of home-based services to families and their first newborn babies. Family Support Specialists visit the families in the home and complete a set of core activities in vital areas known to be related to healthy child development and optimal family functioning. Areas of focus include primary medical care, family planning, developmental delays, post-partum depression, parent education, substance abuse, domestic violence, literacy, basic needs and social isolation. 21-827-9 i { Ij{ I f
