(dba PHFE Management Solutions) l'all, te SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND JUSTIFICATION LOW 6 REC011 N1 EN DATION(S): Approve and authorize the Health Services Director, or his designee (Wendel Brunner, M.D.) to execute on behalf of the County, Novation Contract #22-940-3 with Public Health Foundation Enterprises, Inc., dba PHFE Management Solutions, a non-profit corporation, in an amount not to exceed $128,621, to provide consultation, technical assistance, and support to County's Senior Nutrition Program, for the period from July 1, 2008 through June 30, 2009. This Contract includes a three-month automatic extension through September 30, 2009, in an amount not to exceed$32,155. FISCAL IMPACT: This Contract is 100% federally funded under Title III-C,(1) of the Older Americans Act of 1965. BACKGROUND/REASON(S) FOR RECOMMENDATION(S): This Contract meets the social needs of County's population in that it provides management and staffing for County's Senior- Nutrition Program in East Contra Costa County and provides assistance in fundraising efforts in the private sector, to purchase meals for homebound elders through the C01111tywide Meals on Wheels program. On August 7, 2007, the Board of Supervisors approved Novation Contract #22-940-2 with Public .Health Foundation Enterprises, Inc., dba PHFE Management Solutions, for the period from July 1, 2007 through June 30, 2008, which included an automatic extension through September 30, 2008, for the provision of consultation, technical assistance, and support to County's Senior Nutrition Program, including coordination and supervision of Meals on Wheels activities, delivery of meals, and producing statistical and financial reports. Approval of Novation Contract #22-940-3 replaces the automatic extension under the prior Contract and allows the Contractor to continue providing services through June 30, 2009.
