M347 6/82 00 486 1 CLERICAL DEEP CLASS CLASSIFICATION QUESTIONNAIRE (New Position) Health Services Health Plan Name Department Division Work Unit Clerk - bevel B Concord Health Center, 3052 Willow Pass Job Classification Work Location & Address Percent Of Description of Werk: Time Required RECEPTIONIST: This position may be the first contact a patient has with the Health Center, either in person or on the telephone. It is, therefore, his/her duty to present a cheerful, helpful picture to set the scene for a pleasant experience. S/he must be able to solicit information in a professional manner, to explain procedures and interpret policies as they apply to a specific situation or person. S/he must have a thorough knowledge of Health Plan benefits and coverage groups, as well as the benefits of Medi-Cal and Medicare. S/he must be particularly careful and accurate in soliciting billing information and in establishing elig- ibility, since manv sources of information are not available during evening hours and patients will not be available the next day when thorough checks can be made. 20% Appointments - Makes appointments for patients both in person and by telephone. This involves helping new patients choose a physician, keeping in mind physician and patient availability, given transportation problems, work schedules, etc. , as well as the type of patients each physician usually handles. This my involve the need for some medical knowledge. Schedule return appointments and Specialty Clinic appointments as recruested by the physician, cancel and re-schedule appointrents as requested by the physician or patient. When patients request urgent care, makes an appointment if possible or refers the patient to the nurse to determine the need for urgency and/or to discuss with the doctor. 50% Registration - Solicits current information including demographics, such as name, address, telephone number, plus billing information, such as Health Plan eligibility; Medi-Cal and Medicare eligibility; insurance conpany name, benefits, numbers or name, address,. etc. of other payors, etc. Checks this .information against computer information to determing eligibility for Health Plan and Medi-Cal. During daytime hourse, any discrepancies or cniestions can be checked with Social Service Eligibility and/or Health Plan Business Office. These offices are closed during evening hours and must be checked the next day, so evening clerk must use best judgement to deter=e whether patient is eligibile. Clerk uses computer system, to enter all patient data. Clerk may use computer _ generated Admission Record, which s/he will file into the medical record , and must make a plastic identification care once a month for Health Plan Members or per visit for non-plan merbers. 5% Cash Collections - Collects cash deposits or Medial Co-Pay from Non-Plan members and premiums from Health Plan members, when appropriate. CoMletes receipt form, takes rnney, gives receipt to patient, secures money. 10% Charge forms - When a patient is fully registered, all information is correct and the iden-LiticaLion card :is made, a charge form is einhossed with the identification card and placed on the front of the medical record. The form is Meted by the physician at the time of the visit. After the visit, clerk checks the charge form for corpleteness, separates the 2 copies and sends the 2nd copy to Medical Records. The 1st copy is left on the Reception desk for the day clerk to send to Encoding. Particular attention must be paid to the imprint area to be sure all information (such as visit date) is present. 00 08'7 January, 1992 CLASSIFICATION QUESTIONNAIRE (CONTINUED) Percent Of Description of Work: Time Reclui red 5% Forms - Patients who have insurance or disability need billing forms completed and also must sign Release of Information and Assignments of Benefits forms to allow Health Services Department to bill for services. Forms completion is also required for school and work examinations, Child Health and Disability Prevention examinations, etc. 10% Medical Records - New patients or those new to Concord Health Center must have medical records initiated. This involves a ccoputer search under all possible names and spelling of names, to determine if the patient is known to the Department. If a previous record is located, identifying information must be matched carefully to be sure it is correct for this patient. If no past record is found, the computer is requested to assign a number. All patient information is entered into the computer and an Admission Record form is computer printed. The clerk assembles the medical record by: Writing patient's name and medical record number on the medical record folder (color-coded folders have partial numbers printed on them) ; Filing the Admission Record, Laboratory Accumulator Sheet, Progress Notes, Medication Record and Problem List into the folder, Stamping the forms with the patient's identification card; Stamping the Progress Notes with the date; Attaching charge forms, other necessary forms and identification card to the outside of the record. A copy of the appointment slip is placed in a "paddle" which is filed in the Medical Record files where the medical record will be permanently filed, so anyone. looking for the record will know where the record is. If any summaries, referrals, etc. are found in the Medical Record files (these are filed in a blue, red or yellow plastic pocket and placed in the file when there is no medical record in the Health Center) , this information is also filed into the medical record before giving the record to the physician. If a patient already has a medical record in the Concord Health Center, the record should be ready for use if the patient had an appointment. For urgent care visits, refills of prescriptions, telephone calls, etc., Clerk will complete an appointment slip or an "outcard". This card is filed in a "paddle", which is placed in the Medical Record file in the place of the medical record, which is pulled from the file. The Progress Notes in the record are stamped with the date, charge forms, other forms and the identification card are clipped to the front of the medical record and given to the physician. 00 088 (New Position) YNE 1'.
