Dr. William.Walker, Health Services Director noted this presentation is a follow up from last week about the status of Doctors Medical Center San Pablo/Pinole. He said.during the past two weeks, the Health Services Department responded to several issues raised by the closure of Doctors San Pablo Emergency Department to ambulance traffic, as well as the financial crisis faced by the hospital. Dr. Walker said the Health Services Department's activities)for emergency medical service issues are as follows: 1. Continued to divert all ambulance patients from Doctors Medical Center in San Pablo. 2. Continued the policy of not allowing any other hospitals to go on Emergency Department diversion. 3. Met with Kaiser Richmond to assure their.availability to handle the volume of ambulance patients diverted from Doctors. Kaiser Richmond has assessed Emergency Department staff and started procedures, including transfer of patients to Kaiser Oakland when necessary, to meet added ambulance demands. 4. Continued discussions with American Medical Response (AMR) to assure their ability to meet existing response time requirements in West County. AMR has added unit hours to assure this capability. 5. Authorized AMR to respond to single-paramedic-staffed ambulances into Richmond when necessary to meet response times during periods that Richmond may be covered by an out-of-area ambulance. 6. Started daily checking of ambulance destinations.(hospitals) for patients transported from West County. Destinations are posted on the Emergency Medical Services website daily. I. 7. Started discussions withl AMR on potential long-term impacts on ambulance service if Doctors were to close. Dr. Walker explained he and the hospital staff took part in several conference calls organized by Supervisor Gioia,(which included representatives of the State Department of Health Services, the Health and Human Services Agency, the California Medical Assistance Commission, and representatives from Doctors San Pablo and the offices Senator Perata, Assemblywoman Hancock and Representative George Miller. Dr. Walker noted the possibility of an extra$2.5 million of revenue to be presented to California Medical Assistance Commission by Doctors San Pablo on October 5, and said I I I , I the financial projections have been reviewed as part of this process. In addition, Contra Costa County Health Services Department has been supportive in every way possible of Doctors San Pablo completing its proposed contract with the California Department of Corrections to provide medical services to inmates of state correctional facilities, and said he is hoping this projected line of business and revenue comes to fruition. i Chair Gioia said there is a short-term and a long-term issue: the short-term issue is the need for immediate cash infusion regularly, and is a long-term need to develop a Sustainable business plan. Supervisor Uilkema voiced concerns on the County being able to provide a short-term plan to provide flu shots in West County. Dr. Walker said flu vaccine programs outpatient in all clinics and said he does not expect that the crisis in Doctors Medical Center would impact their ability to provide flu vaccines in West County, but noted the primary concern during flu season would be i Intensive Care units. I Chair Gioia pointed out if the facility closes fully there would be an immediate need to increase urgent care or outpatient clinic hours to handle those who walk into the Emergency Room but may not need full Emergency Room care but need Urgent Care or some other form of outpatient services. Chair Gioia said he is concerned about the patients going to the Emergency,Room as "walk-ins". Chair Gioia asked the public forltheir comments and the following people spoke: I Sandy Faulk, Public Employees Union, Local 1 thanked the Board, Dr. Walker and his staff for assisting with the short and long term needs at Doctors Medical Center. She urged the Board to partners with services from the County and announced a forum being held by Local 1 on October 7, 2006 at West County; Rick Stazka, resident of El Sobr�ante, complained about the incompetence displayed at Contra Costa Regional Medical ICenter. I Dr. Walker said there is a mechanism for bringing in more federal money through intergovernmental transfers where a government entity would offer dollars as a contribution that could be matched by the federal government and made available to a hospital. He explained this is how the money has been allocated in the Distressed Hospital Fund because this is a contribution by the state to the District. He went on to say the money used now from the County's budget for Doctors Medical Center is all in support of the"Medi-Cal" clients who receive services there. He said it does not appear the County can contribute any more money without an outright gift. Chair Gioia commented there was hope if advance funds for"Medi-Cal" services could be available but that is not the case so a subsidy from the County to Doctors Medical Center could draw down extra funds with no guarantee. Chair Gioia said one of the I i . I I I issues that is important for the long tern of any facility is having some subsidy or some contribution from the property tax and parcel tax which is about $8m a year and some of that is already obligated for the next 10/20 years to pay back some prior debt. He said this is a very important issue and at some point one needs to be aware of whether short- term decisions to incur cost defeats the long-term interest of having revenue for a hospital. Addressing Dr. Walker, Supervisor DeSaulnier requested continued oversight and a return to the Board if the diversions, the cost, or the response time changes, and to make sure the costs do not increase with the hospital administration or the Receiver. Addressing Ms. Faulk of Local One, Chair Gioia said it is important at any community discussion the facts be put out there without assumptions. He said he is aware the Health Services Department would look at whatever discussion takes place today at the Hospital District Board Meeting. He suggested keeping this on the Agenda in the event action needs to be taken. By a unanimous vote with none absent, the Board of Supervisors too the following action. ACCEPTED the update. We have initiated discussions with AMR regarding potential long-term impacts on ambulance service if Doctors were to close. We have also participated in a number of conference calls organized by Supervisor Gioia, which included representatives of the State Department of Health Services, the Health and Human Services Agency, the California Medical Assistance Commission, and representatives from Doctors San Pablo and the offices Senator Perata, Assembly- woman Hancock and Representative George Miller. To date, we'have explored the use of intergovernmental transfers and Federal matching dollars to assist Doctors San Pablo. The possibility of an additional $2.5 million of revenue will be presented to CMAC by Doctors San Pablo on October 5. In addition, we have been supportive in every way possible of Doctors San Pablo completing its proposed contract with the California Department of Corrections to provide medical services to inmates of state correctional facilities.
