FROM: William Walker, M.D., Health Services Director ^i `tiz Costa By: Jacqueline Pigg, Contracts Administrator &'4 DATE: September 5, 2006 County SUBJECT: Approval of Grant Agreement 428-755 with the Kaiser Permanente (dba Kaiser Permanente Community Benefits Program) SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND JUSTIFICATION RECOMMENDATION(S): 'Approve and authorize the Health Services Director or his designee (Wendel Brunner, M.D.) to execute on behalf of the County, Grant Agreement 428-755 with the Kaiser Permanente (dba Kaiser Permanente Community Benefits Program), to pay the County an amount not to exceed $20,000, for the Bay Point Health Conductors Project, for the period from August 15, 2006 through August 15, 2007. FISCAL IMPACT: This Grant Agreement funded by Kaiser Permanente (dba Kaiser Permanente Community Benefits Program) will result in an amount not to exceed $20,000 for the Bay Point Health Conductors Project. No County funds are required: BACKGROUND/REASON(S) FOR RECOMMENDATION(S): The Bay Point Health Community Partners, in collaboration with the Public Health Division and the Public Health Outreach, Education and Collaborations (PHOEC) will work together to continue to build and deepen the Bay Point Health Conductor Project model as an effective strategy for engaging the African American community to improve its health and prevent-diseases. The Bay Point Health Conductors Project is critical to Contra Costa Health Services Department (CCHSD). The Department has begun to develop a Countywide African American Health Initiative (AAHI) to address the alarming health disparities for African Americans particularly, regarding chronic diseases. The overall goal of the African American Health Conductor Project is to improve health outcomes for approximately 2,600 African Americans living in Bay Point, by creating and sustaining an African American centered outreach, health education and health care navigation system. Approval of Grant Award Agreement #28-755 funding will support and sustain the Health Conductor strategy within CCHSD and raise awareness in the community about African American health issues, values and concerns. Three certified copies of the Board Order should be returned to the Contracts and Grants Unit.
