RECOMMENDED ACTION:
APPROVE AND AUTHORIZE the Employment and Human Services Director, or designee,to Apply For
and If Granted, Accept AND Execute a contract with California State Department of Transportation(CalTrans)
for the Environmental Justice Grant for the Welfare-to-Work Transportation Action Plan Update and Expansion
project, in the amount of$36,000 for FY 2003-04. (29-129-0)
FINANCIAL IMPACT!
The County will receive $36,000. Grant requires a$2,000 in-kind services match and a$2,000 cash match,
which will come from Workforce Investment Act(WIA) Transportation funds.
RACKGROUND•
On July 20, 2004(Item C.19), the Board approved Employment and Human Services Department(EHSD)to
apply for and accept a grant for FY 2003-04 from the California State Department of Transportation(CalTrans)
Environmental Justice Grant for the Welfare-to-Work Transportation Action Plan Update and Expansion
project. Funds from this grant will be used for consultant services to do research and outreach to address the
employment transportation needs of the County's disabled population as well as update the County's Welfare-
to-Work Transportation Plan. EHSD recently received notification of the grant award. This Board Order is being
written to conform to CalTrans' contracting language requirements and to allow EHSD to enter into and execute
a contract with the California Department of Transportation to implement the planning grant.

CHILDREN'S IMPACT STATFMF,NT
This funding for home visiting will support two of the five of the community outcomes established in the
Children's Report Card, 4) "Families that are Safe, Stable and Nurturing", 5) "Communities that are Safe and
Provide a High Quality of Life for Children and Families",by providing services for young children to be
healthy, ready to learn and supported in safe, nurturing families and communities.
BACKGROUND!
The FIRST 5 Contra Costa Children and Families Commission has made funding available for Home Visiting in
Contra Costa County as part of their mission to foster optimal development of children zero to five years of age.
The funding will be used to support the Welcome Home Baby Program(WHB) to include a social caseworker
specialist as a member of the Consultation and Response Team for Home Visiting. WHB is a prevention and
early intervention program providing home visiting for first-time families. The identification of first-time
families and provision of weekly home visits emphasizing infant bonding and nurturing child development,
health education and medical care, and assistance with employment, housing, transportation an ildcare can
provide appreciable reductions in child abuse and neglect and other significa4hhe "and e c on concerns.

RECOMMENDATION:
Authorize the Health Services Director to transfer a maximum of$10,000,000 to the State of California
as authorized under SB-1255.
FISCAL IMPACT:
Contra Costa Regional Medical Center(CCRMC)will be eligible for a maximum of$10.0 million in
matching Federal funds. See background information for full discussion.
BACKGROUND:
Federal grants to the State of the Medicaid program (Medi-Cal in California) are authorized under Title
XIX of the Social Security Act to provide medical assistance to certain persons with low income. These
Medicaid programs are jointly financed by the Federal and State governments and administered by the
States. State Medicaid agencies conduct their programs according to the Medicaid State plan approved
by the Administrator of the Health Care Financing Administration (HCFA), To carry out the Medicaid
Program, the State agency pays providers for medical care and services provided to eligible Medicaid
recipients.
The Federal government pays its share of Medicaid program expenses to the State on a quarterly basis
according to a formula. The State share of Medicaid program expenses is paid from "State funds,"that
is, those funds over which the State legislature has unrestricted power of appropriation. In California,
the program "split" is 50% Federal financing, 50% State financing.
SB-1255 (Chapter 996, Statutes of 1989)added Section 14085.5 to the Welfare and Institutions Code
and established a program whereby certain voluntary transfers and donations will be utilized by the
State to provide funds to disproportionate share hospitals to assist them in enhancing or maintaining
access for Medi-Cal beneficiaries. This program is similar in nature to those established in Tennessee
and other states.

These voluntary transfers would be used to match Federal dollars under the Medi-Cal program,
thereby increasing the total revenue base available for distribution (i.e., $1 donation will generate $1
in Federal funding back to the disproportionate hospitals).
CCRMC is a disproportionate hospital eligible for reimbursement under this program.
The California Medical Assistance Commission (CMAC) is responsible for administration of SB-1255.
The Commission solicits the donations and distributes the funds collected through the Medi-Cal
contracting processing.
ISSUES:
Issue 1: SB-1255 payments are subject to Federal "Upper Payment Limits" imposed on the
State of California's Selective Contracting Program waiver.
Discussion: Federal approval of the State of California's most recent Selective Contracting
Program waiver included a provision that on a statewide basis, the aggregate amount
paid to hospitals through their contracts with CMAC, including SBA 255 payments,
may not exceed the aggregate amount that would have been paid for those same
services under the Medicare Program. While the Federal limit is not hospital specific,
at this time the State has not issued regulations specifying how they intend to perform
the required calculation and implement the Federal limitation, nor have they specified
how they would recoup any such overage should one occur.
Issue 2: Facilities which make a"voluntary"donation are not guaranteed of return of either the
matching Federal funds or the donation itself.
Discussion: SBA 255 is intended to provide a workable program. If the most"needy'facilities in
the State make donations and do not receive back, at a minimum$1.50 for each
$1.00 donation, the program will fail. While, because of Federal restrictions, the
State cannot guarantee the return of the transferred funds, it is the intent of the
program that no"donating„facility will be disadvantaged.

RECOMMENDATIONS
Authorize the Community Development Director, or his designee, to execute a contract
amendment effective September 29, 2004 to extend the term from September 30, 2004 to
March 31, 2005 to allow for additional public outreach in developing a shortcut path for
Pleasant Hill BART as part of the contract with Moore Iacofano Goltsman, Inc. (MIG)with no
increase to the payment limit of$50,000.
FISCAL IMPACT
Costs will be covered by revenues received from the Redevelopment Agency, from Specific
Pian fees and a Transportation for Livable Communities Grant from the Metropolitan
Transportation Commission.
BACKGROUND/REASONS FOR RECOMMENDATIONS
The consultant (MIG)was chosen through an RFP process and a collaborative review process
including staff from the County Community Development and Public Warks Departments,the
City of Walnut Creek, The City of Concord, and the East Bay Regional Park District. This
contract extension will allow staff and the consultant the time necessary to complete the
outreach. &preliminary design process.
