(#29-127) SPECIFIC REQUEST($)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION RECOMMENDED AC IIO APPROVE and AUTHORIZE the Employment and Human Services Director, or designee, to SUBMIT the FY 2003-04 Contra Costa County Area Agency on Aging Area Pian Year End Report to the California Department on Aging and AUTHORIZE the Chair, Board of Supervisors,to sign the Letter of Transmittal. (#29-127) FINANCIAL IMPACTo_ No fiscal impact. RACKGROITND The Area Agency on Aging(AAA)provides services for older residents of the County under the Older American's Act(OAA) and Other State Funded Programs (OSFP's: previously known as"Older Californians Act Community-Based Programs"). The FY 2003-2004 Area Plan Update proposed to continue to provide through contract,Congregate Meals, Home-Delivered Meals, Legal Assistance, In-Home Services Registry, Friendly Visitor,Adult Day Care, Errand and Escort Services and Transportation under OAA. OSFP's allow AAA to provide through contract, Alzheimer's Day Care Resource Center Services and Brown Bag services. In addition,AAA directly provides Information and Assistance,Health Insurance Counseling and Advocacy, Disease Prevention and Health Promotion, Linkages, and respite services. The AAA has developed services for the Family Caregiver Support Program providing Financial, Legal and Medical Education, Respite, Multicultural Connection Program, Assistive Technology, and Grandparent Caregiver Support Program. The AAA continues to provide program development, coordination, administrate , d planning. Needs Assessment Activities Contra Costa County moved another step closer to a program of integrated long-terra medical, home, and community-based services with a 2003-2004 Development grant from the California Department Health Services, Office of Long-Term Care. The program for long-term care integration - Contra Costa Continuous Care - remains an essential partnership between the County's Area Agency on Aging (AAA), Aging and Adult Services, Health Services Department, and Contra Costa Health Plan. Community advocates, agencies, and consumers provide membership to the new Long-Term Care Advisory Committee. The Consumer Continuity Workgroup of the Committee will ensure that guidance and participation from the community is on going through the development process. The completion of the Draft Administrative Action Plan will provide the blue print for implementing Contra Costa County's long-term care integration program. To maximize resources and eliminate fragmentation, Contra Costa County continued to utilize a blend of services to address the acute and long-term medical and psychosocial needs of the frail elderly in our community. These services included interdisciplinary case conferencing, which link the client to traditional services such as in home diagnostics, medical geriatric protocols, disease education, medication management, home delivered meals, transportation to and from medical appointments, shopping, respite care, caregiver support, and 24 hour medical consultation. Case management services is provided by Aging and Adult Services through its Multipurpose Senior Services Program (MSSP), Linkages, and Adult Protective Services (APS) for adults residing in Contra Costa County. Once again, the AAA identified our community's needs through needs assessment and demographic studies. Based on the outcomes, new funds were used to further leverage additional resources and current funding was used to augment or provide new services. Title III funds were redirected to provide Adult Day Gare (ADC) and Assisted 8124/2004 3 Transportation (errand and escort) services. Title III E, Older Americans Act funds, gave us the opportunity to better serve caregivers by contracting with the following organizations: Agency Programs/Services Family Caregiver Alliance Multi-faceted Respite & Education Programs Jewish Fancily & Children's Multicultural Resource Services of the East Bay Library & Website Alzheimer's Disease & Disorders Dementia Gare, Wellness, Association Financial, Legal & Medical Education & Assistance Families First, Inc. Grandparent Caregiver Program Services Independent Living Assistive Living Devices West Coast Children's Clinic Grandparent/grandchild counseling Jewish Family & Children's Cultural Diversity Services of the East Bay Caregiver Training In FY 03-04, the AAA utilized Title III E OTO funding to add a new contractor and service to the Family Caregiver Provider Program. A grandparent/grandchild counseling and activity program was funded for six months that provided needed counseling and group support to grandparents raising grandchildren. Jewish Family and Children Services of the Greater East Bay completed their Cultural Diversity Caregiver Training program, which was funded with OTO monies. The training targeted Lao, Mein, Afgan, and Russian caregivers. AAA continued to support and participate in the planning collaborative to implement joint regional planning and program activities. AAA allocated $10,000, to each Senior Coalition for East, West, and Central County. Each coalition went through a planning and prioritizing process to determine their focus for FY 03-04. The West County Senior Coalition conducted a Town Hall Meeting, which focused on health care and transportation services for seniors. 8124/2004 4 Speakers at the event included legislators, community activists, and seniors. The Coalition also continued to organize the annually held Senior Dight Out, Senior Information Day, and Senior Rally in Sacramento. East County Senior Coalition continued its extensive Information Outreach Project that contacted seniors at senior centers, community events, and health fairs. The Coalition sponsored three Senior Information Mini-Fairs that offered health screening and resource information for seniors. In FY 03-04, the Mini-Fairs were held in the cities of Pittsburg, Oakley, and Discovery Bay. The Coalition also planned and implemented a successful and well attended Alzheimer's and Other Dementia Awareness Forum. Central County Senior Coalition established a strategic plan years 2002- 2005 to address the needs in Central Contra Costa County. It established the following working committees: Outreach, Publicity, Events and Advocacy in order to increase involvement and membership. Media contacts were developed in order to publicize the committee meetings and activities. In addition, the Coalition honored seniors from eight participating central county senior centers with the "Mary Shockley Memorial Award for Outstanding Service." The award was held in May 2004 to coincide with alder Americans' and Older Californians' Month. On September 26, 2003 a highly successful "Partners in Planning" conference was held and approximately 50 members of the local Commissions on Aging, the Advisory Council and coalitions came together for the third annual conference. The agenda consisted of "Where are we now?" a year in review (successes and challenges), how to build active membership, and an up-date on Measure C (transportation). Also, presented was a review of AAA Highlights and plans for the future. Once again, this event sponsored by AAA has proven to be an empowering tool to promote networking and assist AAA in determining services in each city/town throughout the county. During FY 03-04 AAA staff has worked to plan, organize, and coordinate the next conference that is scheduled for September 10, 2004. 8/24/2004 5 AAA continues to help address the needs of Southeast Asian elders, one of the most isolated and vulnerable groups in our community. We worked with members of the Southeast Asian Senior Access Collaborative (SEASAC) to help coordinate comprehensive and bilingual services that includes, outreach, case management, social club, and health education for Southeast Asian elders in West Contra Costa County. Along with AAA,, members of SEASAC include United Laotian Community development Inc., Lao Senior Association, City of Richmond, City of San Pablo, and West County Senior Coalition. AAA completed its 2003 Key Informant Survey. The survey was administered via focus groups, written surveys, and interviews with consumers and key informants. Health care, transportation and housing were named as the most serious problems facing older adults in the County. Results from the .survey are used to aid the AAA in developing and updating its Area Plan. The AAA also shared survey results with all participating key informants and made it available to organizations and individuals who are part of the aging network concerned about the quality of life of older adults within the County. 8/24/2004 6 . Status of Objectives GOAL #1 The AAA is committed to assisting alder individuals (who may be in danger of losing their independence) so they can lead meaningful, dignified, and independent lives in their own homes and communities as long as possible. AAA will accomplish this through an accessible ranee of options including but not limited to, collaborative efforts, decision making among public, private, voluntary, fraternal organizations, and coalitions, for continuing the development and enhancement of a comprehensive and coordinated community and home based system of care. RATIONALE The AAA conducted a needs assessment, through the analyses of the needs assessments, the AAA was requested to provide program development, coordination and technical assistance. OBJECTIVES 1. AAA staff will participate in the East County Coalition to provide a Senior Information Day for East County seniors by June 30, 2005. C ongoing. The Coalition Inas completed three senior health and information fairs throughout East County this year. 2. AAA staff will coordinate with West County organizations to provide a Senior Information day by providing I & A resource material, NICAP information and AAA staff time from July 1, 2003 through June 30, 2005. AAA staff will assist East, West, and Central County Coalitions with grant writing by providing demographic data to bring additional senior resource to Contra Costa County {CCC} through June 30, 2005. AAA staff along with the Long Term Gare (LTC) consultant will establish an 11 member Long Term Care Implementation Planning 8124/2004 7 Team with representation from Health Services and Employment and Human Services departments, providers of Social medical services and consumers by July 2001 . AAA staff and the AAA LTC consultant will provide opportunities for input for the County Long Term Care Plan from Task Force and Design Team members, LTC consumers, and providers, and representatives of the medical community by conducting focus groups, implementing surveys, and holding public forums by June 2005. AAA staff and LTC consultant will seek input from stakeholders re: issues of concern identified during planning process by publishing and distributing a LTC newsletter to provide LTC planning information and to solicit input by June 2005. PD & C completed 18. AAA staff will participate in the East County AIDS Task Force, which was convened in response to the disproportionate impact of HIV and AIDS on East County. The Task Force will develop outreach materials, resource fairs, and a CCTV program to educate the public about HIV/AIDS and services available in the community by June 30, 2005. PD & C ongoing 19. PD&C completed 8/24/2004 10 GOAL # 2 AAA will provide services to older individuals with the greatest economic and social needs including low income diverse population groups through continued service expansion, and development, and collaboration with community groups, service providers, and elected officials, in order to provide health, social, nutrition and legal services for older individuals who may be at risk of abuse or neglect. RATIONALE This is mandated by the Older Americans Act and AAA is Committed to providing Services to the population groups specified above. OBJECTIVES I, AAA staff will develop & implement a timeline including appropriate presentations to the senior community, aging network, private and public agencies. The timeline will include activities such as developing and distributing AAA newsletter, ethnically and culturally appropriate program FACT sheets, brochures, and literature regarding new and expanded services by June 2005. In addition, the AAA Mobility Manager in 03-04 implemented the "'Senior Transportation Forum'; which brings together the non-transit providers of senior transportation services in the county with the goals of improving and possibly coordinating services. 16. The Transportation Committee will advocate to local politicians and transportation officials for increased fixed-route and paratransit funds and services and less emphasis on automobiles and highways through June 2005. Alzheimer's Day Care Resource Center - Rehabilitation Services of Northern California's will provide services to benefit persons with moderate to severe Alzheimer's disease or related dementia by providing success-oriented activities and health programs designed to promote health, self-esteem, dignity, cognitive stimulation, and enjoyment for those with impaired cognitive, perceptual, behavioral, physical and emotional abilities. Outcomes will be measured by attendance, length of enrollment, health status maintenance, caregiver satisfaction and the ability for participants to remain living in their homes. Caregivers will be given respite, counseling, education, case management, and resource and referral services in order to assist them in continuing to provide care to their loved ones for as long as possible, preventing premature or inappropriate institutionalization through June 2005. ongoing 28. This program is funded under the Title III E Family Caregiver Support Program. AAA will contract with Jewish Family and Children's Services (JFCS), acting as the lead agency, to provide the development 812412004 35 and design of a program addressing the training and educational needs of caregivers who exhibit cultural barriers to accepting assistance in the care of a family member. Caregivers from different ethnic cultures bring with them cultural stigmas about care giving responsibilities. They also lack specialized training and counseling on how to deal with the daily stresses of caring for a relative as well as lack information about the resources available to them by ,June 30, 2004 completed 46. A coalition of agencies, coordinated by JFCS, will coordinate outreach in the following communities: Laotian, Vietnamese, Latino, African American, Russian, Afghan and Philippine. Focus groups will be assembled to discuss the specific cultural needs for caregivers from these cultures. Based upon information gathered from these focus groups, a curriculum will be designed to address the needs assessed. This phase will be completed by June 30, 2003. completed 47. The AAA will participate in a project sponsored by the National Association of Area Agencies on Aging (n4a) entitled: Making the Link. This program is designed to work with area physicians to identify caregivers and link them with available services provided by the Human Service organizations of Contra Costa County. The AAA will utililze Title III E FCGP OTO FY 03•-04 funding to expand services to grandparents by funding the West Coast Children's Clinic to provide counseling, support and respite to grandparents raising grandchildren through 2004. Completed 8/24/2004 36 3. Program Development and Coordination (P D, & C) Section 3. Status of objectives contains the AAA activities relating to program development and coordination for FY 03-04. "PD" or "C" is shown next to the correlating objectives. AAA. is able to provide millions of dollars worth of services, activities, and events to seniors in CCC due to the funding available for PD & C activities and the AAA staff utilizing other revenue sources by forming collaboratives. The AAA public and private partners are able to use in-kind resources to further expand, develop, and implement new services. The collaboration efforts have resulted in assisting with the development and coordination of new Senior Housing Projects, new services such as Errand and Escort, Southeast Asian Project, Fraud Fighter Forums, Adult Day Care, Alzheimer's Conferences, additional exercise programs, and many special events. As a result of this coming together, AAA's ability to utilize PD & C funds has resulted in a decrease of duplication of services throughout Contra Costa County. 8/24/2004 37 4. Targeting By continuing to use funding available to the Area Agency on Aging (AAA), the agency is able to target and serve minorities in Contra Costa County (CCC). The County's ethnic demographic profile continues to include: African- American, Hispanic, Laotian, Vietnamese, Filipino, Russian, and Ukrainian, with a small percentage of Native American and Middle-Eastern elders. In compliance with the Older Americans Act and Older Californians Act, AAA continues to require its contractors to target and serve minorities and individuals with the greatest social and economic needs. This is part of their contractual obligation. AAA continues to provide intensive outreach and targeting in the community. Community liaison Staff (outreach workers) seek out diverse and hard to reach seniors by forming collaborative working relationship with other organizations and groups, by making verbal presentations to a variety of audiences, and broadly distributing brochures, pamphlets, and other printed information about the available services. Once again, AAA is committed to providing translation services (both written and spoken information) for the community at large and its contractors. The AAA Senior Information Newsletter Health Page is printed in English as well as Russian, Spanish, Laotian and Vietnamese. Our circulation of the Newsletter throughout the County is approximately 17,000. By increasing the number of presentations made this year to various community organizations and groups, the circulation of the Newsletter was increased. Also, we increased the distribution while attending social events for outreach and rapport building. Meetings were held with leaders from the Laotian, Vietnamese, Russian, and Latino populations in Contra Costa County to better understand their culture and needs. Jewish Family and Children's Services (JFCS) was contracted to assess the needs of caregivers from multi-ethnic populations including Laotians, Russians, Afghans, African-Americans, and Vietnamese. Following the assessments, JFCS completed the Cultural Diversity Caregiver Training a/2412004 38 Program this year. The Caregiver Training Program addressed the unique needs of caregivers from diverse populations. AAA staff and nutrition staff continued to meet with CDA and Mate staff to identify funding sources for new activities to be incorporated into Nutrition Cafes (sites) and Senior Centers, Also, AAA staff continued to help organize and participate in a series of Senior Information Mini-Fairs in East Contra Costa County. The Mini-Fairs, which offered health screenings and information about resources for seniors were well attended and attracted a large number of elders from the Latino community. This year, the Mini-Fairs were held in City of Pittsburg, Bay point, and Discovery Bay. AAA continues to work with United Laotian Community Development Inc., City of San Pablo, City of Richmond, Vilest Contra Costa County Senior Coalition and Lao Senior Association in the Southeast Asian Senior Access Collaborative (SEASAC), to provide services that address the needs of Southeast Asian elders. Due to language and cultural barriers, many Southeast Asian elders are isolated and unable to access health and human services. They stay inside their apartments and houses most of the time. They are afraid to go out by themselves because they do not speak enough English to communicate with the public. A grant awarded by The California Endowment enable the Southeast Asian Senior Access Collaborative (SEASAC) to provide culturally appropriate services that help improve the lives of Southeast Asian seniors. SEASAC provided case management, community outreach, information and referral, advocacy, and transportation services to help Southeast Asian elders overcome barriers to access health and social services. In addition, SEASAC continued to offer Social Clubs where elders can participate in cultural and educational activities including but not limited to, arts and craft, field trips, English as a Second Language (ESL) classes, and health education workshops. Once again, the InfoVan enabled counties throughout California to provide needed information and assistance to individuals in their community. In Contra Costa County, the InfoVan targets isolated, homebound seniors, and diverse population groups. The InfoVan traveled throughout the county to provide information about services, how to access them, and contact information for health and social service providers. 8/2412004 39 The van is rigged with shelves that allow the transportation of brochures, pamphlets, and other materials. In consideration of this county's diverse population, informational materials are available in other languages, including Laotian, Russian, Spanish, Vietnamese, Chinese, and Korean. This year AAA staff took the infoVan to senior centers, senior housing units, health and information fairs, and other locations in Contra Costa County where seniors congregate to disseminate information. The Active Aging Community Task Force, which was established during FY 2002-2003, has continued to meet monthly in order to further enhance and provide written material to seniors in CCC. The task force continued to implement effective interventions to reduce falls and fractures through strength training programs aimed at reducing the risks of failing in older adults. In April 2004, the task force in collaboration with AAA, John Muir/Mt. Diablo Medical Center, and Mt. Diablo Adult Education, held a highly successful day- long exercise event or conference called "Going Strong; Fitness Options After 50". The event consisted of a keynote speaker, who is a published author and authority on senior fitness. Also, numerous sessions were held featuring approximately 8-9 speakers that spoke on and provided demonstrations on Chair Exercises, Low-impact Aerobics, Water Aerobics, healthy Eating, Yoga/Meditation, Strength Training, Tai Chi, Walking/Stretching and Injury Prevention. Approximately, 150 seniors attended and requested that it be held on an annual basis. This year AAA staff has continued to participate in monthly telephone conference calls with the CDhS and other California counties with similar programs; attended two conferences in Sacramento, plus one Trainers Workshops; and worked with the UCSF, to take the lead in fiscal matters pertaining to the grant. Once again, a member of our AAA staff was honored in Sacramento by CHS and UCSF for her "dedication to the health and fitness of older adults and the Active Aging Community Task Force Project".
