BAC".KGRUITND: Independent Living Resource provides countywide services to the County's Employment and Human Services Department(EHSD)older population who are transitioning from institutionalized care to living independently at home. Services support family caregivers by providing education and training to address the needs of the care recipient who is transitioning home including access to benefits,daily living skills and techniques to transfer, bath, dress assistance and/or other personal assistance as needed. Program services include providing financial and mobility needs for the individual such as benefits counseling and assistance, equipment,home modifications and occupational and physical therapy needs of the care recipient and procuring assistive devices to modify the new home environment, evaluating wheelchair and gait requirements,providing exercise instruction to strengthen and protect the caregiver's health or instructing the caregiver in transfer and safe bathing techniques for the care recipient. Services under this contract are being extendedd to address the ongoing demand for Providing Assistance to Caregivers in Transition(PACT) services and the continuation of the federal grant. BACKGROUND/REASON(S) FOR RECONiMNDATION(S): On July 22, 2003,the Board of Supervisors approved Contract#23-320.4 with Compliance Solutions, LLC, for the period from July 1, 2003 through June 30, 2004, to provide consultation and technical assistance with regard to implementation of an Electronic Claims and Remittance System to meet the Department's objectives of automating the Accounts Receivable payer transactions and meeting Federal HIPAA regulations. Completion of this renewal contract was delayed due to the complexity of the negotiation process with the Contractor's hourly rate. Approval of Contract#23-320.5 will allow the Contractor to continue without interruptions in services through June 30, 2005. On December 2, 2003, the Board of Supervisors approved Contract #74128-3, with Charis Youth Center to provide an intensive day treatment program for Seriously Emotionally Disturbed (SED) adolescents, for the period from July 1, 2003 through June 30, 2004 (which included a six-month automatic extension through December 31, 2004). Approval of this Novation Contract #74128.4 replaces the six-month automatic extension under the prier Contract,allowing Contractor to continue providing services through June 30, 2005. Responsibility for outpatient mental health specialty services involves contracts with individual, group and organizational providers to deliver these services. Approval of Contract #74-184-24(1), will allow the Contractor to continue providing Medi-Cal mental health specialty services through June 30,2006.
