SCAT. TMPACT: Approval of this Standard Agreement will result in an amount not to exceed $125,000 for the period from July 1, 2004 though June 30, 2009, for the Chlamydia Awareness and Prevention Project (CAPP). No County funds are required. RACKGROUNDIREASON(S) FOR RECOMMENDATION(S): More than 68,700 cases of Chlamydia (CT) were reported in California in 1997. However, due to underreporting and undiagnosed infections, experts estimate that closer to 300,000 women and their partners are infected annually. The purpose of this project is to facilitate, develop, and enhance local capacity for Chlamydia (CT) awareness and prevention through a process of community-health department collaboration and partnership. Approval of Standard Agreement #28-643-3 will continue funding for this Chlamydia Awareness and Prevention Project (CAPP)to strengthen the infrastructure of agencies serving young people, at risk for sexually transmitted disease,through June 30, 2009. FISCAL IMPACT: Approval of this Agreement will result in $444,000 from the Contra Costa Children and Families Commission for the Home Visiting Program. No County funds are required. ICAC"IAC RO11NII[REASONfS)FOR RE+<:`,OMM NDATI{7N(Sl: The Children & Families Commission has adopted a Home Visiting strategy which has been developed through extensive collaboration and planning with representatives of nine home visiting programs in Contra Costa County. The goal of the Home Visiting Program plan is to assure that children, ages 0-5, in our communities are healthy and have access to quality preventive well-child primary care. On October 28, 2003, the Board of Supervisors approved Contract #28-702-1 (as amended by Amendment Agreement #28-702-2) with Contra Costa Children & Families Commission, now known as First 5 Contra Costa Children &Families Commission, for the period from July 1, 2003 through June 30, 2004. Approval of Contract #28-702-3 will allow the continuation and expansion of the Horne Visiting Program through June 30, 2005. This Project will coordinate existing tobacco cessation efforts and develop collaborative countywide efforts to reduce environmental tobacco smoke exposure among children, ages 0-5. FISCAL IMPAC : Approval of this agreement by the State will result in an amount not to exceed $142,149 of State funding for the Regional Medical Disaster Planning Project for FY 2004-2005. No County funds are required. 4 RACKGROIJMIRFASONkS FOR R .COMMENDATION S): On August 12, 2003, the Board of Supervisors approved Standard Agreement#29-392-20, with the State Emergency Medical Services Authority, for the period from July 1, 2003 through June 30, 2004, for the Regional Medical Disaster Planning Project. Approval of Standard Agreement #29-392-21 will continue State funding for the Medical Disaster Planning Project services through June 30, 2005, and allows staff to enhance the County's disaster preparedness by improving coordination between counties, cities, hospitals, Emergency Medical Services agencies and the State Emergency Medical Services Authority, in the event of an earthquake or other disaster. DEPUTY State EMS Authority TO:` BOARD OF SUPERVISORS Contra FROM: William Walker, M.D., Health Services Director Costa DATE: County SEPTEMBER21, 2004 , ,�,n/ SUBJECT: Accept Contributions to County Home Delivered Meals Program (Meals on Wheels) SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION RECOMMENDATION: Accept a contribution of $66,200 from Meals on Wheels of Contra Costa, Inc., on behalf of the County Home Delivered Meals Program(Meals on Wheels),administered by the Public Health Division of County Health Services, to subsidize 30,000 additional Meals delivered to the residences of homebound elders in Contra Costa County. FISCAL IMPACT: The majority of this contribution, $60,000, subsidizes approximately 30,000 additional nutritious meals delivered to approximately 480 additional homebound elders at their residences each day this calendar quarter through the County Home Delivered Meals Program. Substantial voluntary contributions from program clients provide sufficient financial leverage to allow one additional meal to be delivered to a homebound elder for each $2.00 received in additional private or public sector contributions or grants. A portion of this contribution, $6,250, reimburses the county for fundraising development costs incurred on behalf of the program . BACKGROUND: Meals on Wheels of Contra Costa,Inc. is a charitable private non-profit coalition of public and private organizations and individuals that was founded in 1990, by two County managers from the Health Services and Employment and Human Services Departments,to develop on-going private sector funding resources to bridge the gap between available government funding, and demand for Meals on Wheels by the county's homebound elders. With this contribution, Meals on Wheels of Contra Costa,Inc. has donated$1,160,400,in meals contributions since its inception in 1990,to subsidize approximately 673,000 meals delivered to the residences of homebound elders throughout Contra Costa County. Much of the success of this effort in the past five years has been due to a multi-year seed grant from the County Administrator's Grant Revenue Improvement Program(GRIP)which precipitated a dramatic increase in private sector program contributions, which eventually allowed the program to eliminate waiting lists for services. Organizations represented on the Meals on Wheels of Contra Costa,Inc.Board of Directors include:Aging and Adult Services Bureau(Employment&Human Services Department); County Senior Nutrition Program(Health Services- Public Health);County Advisory Council on Aging;Senior Nutrition Program Advisory Council;West County Meals on Wheels,Inc;Family&Community Services,Inc(dba Senior Outreach Services);and Martinez Shell Oil Refinery. The County Home Delivered Meals Program is the primary provider of Meals on Wheels services in Contra Costa County, delivering over 900 meals each day to the residences of homebound elders throughout the county via its network of more that 65 delivery routes almost entirely staffed by volunteers. Service delivery is coordinated regionally in much of the county through contracts with private non-profit community based agencies.
