Approval of Contract #23-338-2 will allow the Contractor to continue providing services through

BACKGROLTND/REASON(S) FOR RECOMMENDATION(S):
On October 7, 2003,the Board of Supervisors approved Contract#26.305.10(as amended by Contract
Amendment Agreements 426-305.11 and#26-305.12)with Vista Staffing Solutions,for the period from
October 1, 2003 through September 30,2004,to present locum tenens physicians to work as temporary
employees to ensure appropriate medical staff coverage to the inpatient psychiatric unit at Contra Costa
Regional Medical Center, and the County's Main Detention Facility.
Approval of Contract Amendment Agreement #26-3035.13 will modify the rate schedule to allow the
Contactor to provide additional classifications of temporary physician services with no change in the
original payment limit and extend the term of the Contract through September 30, 2005.

For many years the County has contracted with registries to provide temporary qualified personnel to
assist the Department during peak work loads,temporary absences and emergency situations.
On December 17, 2002, the Board of Supervisors approved Contract #26-294-8 (as amended by
Contract Amendment Agreement #26-294-9) with Staff Care, Inc., for the period from January 1,
2003 through December 31, 2004, to provide locum tenens physicians to work as temporary
employees to provide vacation, sick leave,and extended leave relief for County-employed physicians
at Contra Costa Regional Medical Center and Contra Costa Health Centers, including the specialty of
Radiology services.
Approval of Contract Amendment Agreement #26-294-10 will allow the Contractor to provide
additional temporary physicians services,through December 31,2004.

On December 2, 2003, the Board of Supervisors approved Contract 926-429-4, with J & C
Nationwide, for the recruitment of temporary physician services to cover vacation, sick leave, and
extended leave relief for County-employed physicians at the Contra Costa Regional Medical Center
and Main Detention Facility for the period from January 1,2004 through December 31,2005.
At the time of negotiations, the payment limit was based upon target levels of utilization. However,
the utilization during the term of the agreement was actually higher than originally anticipated and
has exceed more than 92.3%of its actual payment limit,
Approval of Contract Amendment Agreement #26-429-5 will allow Contractor to continue to
provide services, to Contra Costa Regional Medical Center and Contra Costa Health Centers to
cover vacations,sick leave and extended leave,through December 31, 2005.

For a number of years the County has contracted with Medical and Dental Specialists to provide
specialized professional services which are not otherwise available in its hospital and clinics.
On October 28, 2003,the Board of Supervisors approved Contract#26-9773 with Pacific Urology,
for the period from November 1, 2003 through October 31, 2005, to provide Urology services at
Contra Costa Regional Medical Center, including consultation, training, and medical and/or surgical
procedures.

additional level of services, including weekend and holiday on-call Urology services, through

consultation and technical assistance with regard to the Health Plan's Quality Improvement Program including,but
not limited to, formulate the Quality Improvement Plan and present it to the Health Plan's Quality Council and
act as a liaison between.the Health Plan's partners and providers.

behalf of the County, Contract #26-391-3 with Maxim Health Care Services, a corporation, in an
amount not to exceed $500,000, to provide nursing registry services at Contra Costa Regional Medical
Center,for the period from September 1,2004 through August 31,2005.

RACKGRQ11NDIREASON(S)FOR RECOMMEN DATTON4S):
For several years the County has contracted with nursing registries to provide temporary licensed
nursing personnel to assist Contra Costa Regional Medical Center and Contra Costa Health Centers
during peak loads,temporary absences and emergency situations in critical areas. There is a nationwide
nursing shortage, and in spite of persistent efforts to recruit nurses, the Department has experienced
great difficulty in filling any vacant positions. Therefore, the Department has had to rely heavily on the

On September 9, 2003,the Board of Supervisors approved Contract#26-391-2 with Maxim Health Care
Services, Inc., for the period from September 1, 2003 through August 31, 2004, to provide nursing
registry services to Contra Costa Regional Medical Center.
Approval of Contract #26-391-3 will allow the Contractor to continue to provide services, through
August 31,2005.

Ratify purchase of services from tindrix Transcription,Inc., a Corporation, for the period from July 1, 2003
through June 30, 2004, and authorize the County Auditor-Controller to pay $9,449.30 for provision of
medical transcription services for Contra Costa Mental Health Outpatient Psychiatric Services.
FISCAL IMPACT:
This contract is included in the Health Services Department Enterprise I Budget. As appropriate, patients
and third party payors will be billed for services.
REASONS FOR RFICOMMENDAT C3MMACKGROI1ND:
On June 17, 2003, the Board of Supervisors approved Contractor#74-185-1 for the period from July 1, 2003
through June; 30, 2004, for provision of medical transcription services for Contra Costa Mental Health
Outpatient Psychiatric services.
Services were requested and provided beyond the payment limit and by the end of June 30, 2404, charges of
$39,449.30 had been incurred,of which$30,000 had been paid and$4,449.30 remains outstanding.
Medical transcription services were both requested by County staff and provided by the Contractor in good
faith. Because of administrative oversight by both the County and Contractor, use of transcription services
exceeded the authorized limits.
The Department is requesting that the amount due the Contractor be paid. This can be accomplished by the
Board of Supervisors ratifying the actions of the County employees in obtaining provision of outside nursing
registry services of a value in excess of the contract payment limit. This will create a valid obligation on the
part of the County retroactively authorizing all payments made by the Auditor-Controller up to now, and
authorizing payment of the balance.
