Non-Profit Corporation, effective August 15, 2004, to amend Contract #26-338-4 to increase the total
Contract Payment limit by$300,000 from$400,000 to a new total payment limit of$700,000 with no change
in the original'tenn through December 31, 2004.
FISCAL,TMPACI:
Funding for this contract is included in the Health Services Department Enterprise I budget. Cost to the
County depends upon utilization. As appropriate, patients and/or third-party payors will be billed for
services.
RAC GROUNDIREASON(S)FOR RECOMMEND
On January 20, 2004, the Board of Supervisors approved Contract #26-338-4 with American Red Cross
Blood Services, for the period from January 1, 2004 through December 31, 2004,to provide total blood and
blood component products and associated services as needed for patients at the Contra Costa Regional
Medical Center.
Approval of Contract Amendment Agreement #26-338-5 will allow Contractor to provide additional
services through December 31,2404.

exceed $750,000, to provide temporary nursing services for Contra Costa Regional Medical Center and
Health Center patients, for the period from September 1,2004 through August 31, 2005.

REASONS FOR RECOMMENDAITONSMACKGROUND:
For several years the County has contracted with registries to provide temporary medical professionals
to assist Contra Costa Regional Medical Center and Contra Costa Health Centers during pear loads,
temporary absences, and emergency situations. There is a nationwide nursing and healthcare staff
shortage,and in spite of persistent efforts to recruit nurses and other healthcare staff,the Department has
experienced great difficulty in filling any vacant positions. Therefore, the Department has had to rely
heavily on the registry services in order to provide quality nursing care for patients.
On August 12, 2003, the Board of Supervisors approved Contract #26-458-2 with Access Nurses to
provide temporary nursing services, as requested by the Department, for the period from September 1,
2003 through August 31, 2004.
Approval of Contract #26-458-3 will allow the Contractor to continue to provide temporary nursing
coverage services through August 31,2405.

REASONS MR RECOMMENDATIONN/RACKGROUND:
For several years the County has contracted with registries to provide temporary medical professionals
to assist Contra Costa Regional Medical Center and Contra Costa Health Centers during peak loads,

August 31,2005.

behalf of the County, Contract Amendment Agreement #27-277--6 with Kaiser Foundation Health
Plan, Inc., a non-profit corporation, effective August 1, 2004, to amend Contract #27-277---4 (as
amended by Contract Amendment Agreement#27-237-5)to increase the Contract payment limit by
$720,415, from $7,200,000 to a new total of $7,920,415 with no change in the term through
September 30,2004.
FISCAL IMPACT:
This Contract is funded 100% by Contra Costa, Health Plan member premiums. Costs depend
upon utilization. As appropriate,patients and/or third party payors will be billed for services.
BACKGItC3UNDfREASON S) FSR RECpMIV[FNI)ATION(S):
On January 27, 2004, the Board of Supervisors approved Contract #27-277-4 (as amended by
Contract Amendment Agreement #27-277-5) with Kaiser Foundation Health Plan, Inc., for the
period from October 1, 2003 through September 30, 2004, for the provision of health care
services for Medi-Cal recipients enrolled in Kaiser Foundation Health Plan.
Kaiser Foundation Health Pian has been a traditional provider of health care services to Medi-Cal
beneficiaries in Contra.Costa County. With the implementation of the Local Initiative for Medi-Cal
managed care,Kaiser,in order to continue to enroll Medi-Cal members,roust affiliate with the Local
Initiative. The affiliation arrangement allows Medi-Cal recipients to choose Contra Costa Health
Plan as their managed care plan,and.designate Kaiser as their provider.
Approval of Contract Amendment Agreement #27-277-6 will allow the Contractor to provide
services to additional Medi-Cal recipients through September 30,2004.

behalf of the County, Contract#27-541-1 with Language Line Services, a Corporation, in an amount not
to exceed $75,000, for the provision of over-the-phone interpreter services for Contra Costa Regional
Medical Center and Contra Costa Health Plan patients, for the period from September 1, 2004 through
August 31,2005.

RACKGRQT1ND/RFASQX(S�FOR RECOMMENDATION(S):
For several years Language Line Services has provided foreign language interpretation services via over-
the-phone to Contra Costa Regional Medical Center (CCRMC). The Contractor shall recruit, screen,
train, supervise, and otherwise provide qualified interpreters to provide over-the-phone foreign language
translation services, with over 100 languages from Vietnamese to Portuguese, for CCRMC and the
Contra Costa Health Plan patients.
On September 10, 2002, the Board of Supervisors approved Contract 427-541 with Lanugage Line, LLC
(dba Language Line Services) to provide imperative translation services for CCRMC and Health Plan
patient's who need access facilities and healthcare, when a translator is not physically available, as
requested by Department, for the period from September 1,2002 through August 31,2004.
Approval of Contract #27-541-1 will allow the Contractor to continue to provide services, through
August 31, 2005.

behalf of the County, Contracts specified on the attached Addendum, in an amount not to exceed
$50,000,to continue to provide Medi-Cal mental health specialty services for the period from July 1,
2004 through June 30,2006.
FISCAL IMPACT:
These Contracts are funded by 49%State and 51%Federal FFP Medi-Cal Funds.
BACKGROUND/REASON(S)FOR RECOMMENDATION($):
On January 14, 1997, the Board of Supervisors adopted Resolution#97/17, authorizing the Health
Services Director to contract with the State Department of Mental Health to assume responsibility
for Medi-Cal mental health specialty services. Responsibility for outpatient mental health
specialty services involves contracts with individual, group and organizational providers to
deliver these services.
Approval of this Board Order will allow the Contractors to continue providing Medi-Cal mental
health specialty services through June 30,2006.

on behalf of the County, Contract #24-794-7(6) with St. Helena Hospital and its California
Specialty Hospital, a non-profit corporation, in an amount not to exceed $250,000, to provide
inpatient psychiatric hospital services for the period from July 1, 2004 through June 30, 2005.
FISCAL. IMPACT:
This Contract is 100% funded by State Managed Care Allocation Funds.
BACKGROUND:
Chapter 633, Statutes of 1994, AB 757, authorized the transfer of state funding for Fee-For-
Service/Medi-Cal (FFS/MC)acute psychiatric inpatient hospital services from the Department of
Health Services to the Department of Mental Health (DMH). On January 1, 1995, the DMH
transferred these funds and the responsibility for authorization and funding of Medi-Cal acute
psychiatric inpatient hospital services to counties that chase to participate in this program.
On December 16,2003,the Board of Supervisors approved Contract#24-794-7(5)with St Helena
Hospital and its California Specialty Hospital for the period from July 1,2003 through June 30,2004,
for the provision of inpatient psychiatric hospital services.
Approval of Contract#24-794 -7 (5) will allow the Contractor to continue providing services
through June 30, 2006.

Approve and authorize the Health Services Director, or his designee (Donna Wigand) to execute on
behalf of the County, Contract #24-794-10(1) with Telecare Corporation, a corporation, in an
amount not to exceed$125,000,to provide inpatient psychiatric hospital services, for the period from
July 1, 2004 through June 30,2005.
FISCAL IMPACT:
This Contract is funded 50%by State Medi-Cal and 50%Mental Health Realignment,
BACKGROUNDIREASON(S)FOR RECOMMENDATION(S).
Chapter 633, Statutes of 1994, AB 757, authorized the transfer of state funding for Fee-For-
Service/Medi-Cal (FFS/.N4C) acute psychiatric inpatient hospital services from the Department of
Health Services to the Department of Mental Health (DMH). On January 1, 1995, the DMH
transferred these funds and the responsibility for authorization and funding of Medi-Cal acute
psychiatric inpatient hospital services to counties that chose to participate in this program.
On December 2, 2003, the Board of Supervisors approved Contract#24-794-10 for the period from
July 1, 2003 through June 30,2004 for inpatient psychiatric hospital services.
Approval of Contract #24-794-10(1) will allow Telecare Corporation to continue to provide
inpatient psychiatric hospital services to County-referred residents, ages eighteen and over, through
June 30,2005.

Approve and authorize the Health Services Director, or his designee (Haven Feam) to execute on
behalf of the County,Novation Contract#24-429-34 with Ujima Family Recovery Services, a non-
profit corporation, in an amount not to exceed $1,336,163, to provide perinatal substance abuse
treatment services for the period from July 1, 2004 through June 30,2005.
FISCAL IWACT:
This Contract is funded 70.2% by Federal Perinatal Set-Aside and Medi-Cal, and 29.8% State
funded.
CWLDREN'S IMPACT STATEMENT:
This Alcohol and Drug Abuse program supports the Board of Supervisors' "Families that are Safe,
Stable, and Nurturing" cornmunity outcome by providing parenting education, stability, and safety
for mothers (and their children) and pregnant women who are alcohol and drug dependent, while
they are in substance abuse treatment. Expected outcomes include delivery of drug-free babies,
decreased use of alcohol, tobacco and other drugs, reduction in the number of relapses, and creation
of a sober social network.
RAcKGRouND/REASON(S)FOR REcoMMENDATION(S):
This Contract meets the social needs of County's population in that it provides family-centered
alcohol and drug treatment services to pregnant and parenting women, in order to prevent perinatal
substance abuse and improve birth outcomes.
On December 16, 2003, the Board of Supervisors approved Novation Contract #24-429-32 (as
amended by Contract Amendment Agreement #24429-33) with Ujima Family Recovery Services
for the period from July 1, 2003 through June 30, 2004, (which included a six-month automatic
extension through December 31, 2004) for the provision of perinatal intensive day treatment,
residential recovery progran-4 and transitional housing for women.
Approval of Novation Contract #24-429-34 replaces the automatic extension under the prior
Contract and allows the Contractor to continue providing services through June 30, 2005.
