SUBJECT: Update on Services Provided by Crestwood
Behavioral Health, Inc. for the Mental Health Division Oe-120

RECOMMENDATION:
ACCEPT update from the County Administrator and the Department of Health Services,
Mental Health Division, regarding the management and security of the Crestwood facility
and the status of its operation.
FISCAL IMPACT:
No financial impact.
BACKGROUND:
This is the fourth quarterly report on the services provided, management and security of
the Crestwood Behavioral Health Center located in Pleasant Hill. The center opened and
received the County's first three consumers on October 10, 2003. By the end of
November, there were fifteen consumers residing at the facility and on May 25, 2004 there
were 35 consumers residing at the facility. As of August 31, 2004 there are 42 residents
in two distinct programs. Of these, 9 consumers are in enrolled at Diablo Valley College,
4 consumers are working part-time, and 12 are in vocational training.

Since Crestwood opened last year, there have been a total of thirteen incidents. We last
reported to the Board of Supervisors in June and, at that time, there had been eight
incidents. Since that report, the following has occurred:
• On May 11, 2004 there was an accidental fire in a clothes dryer. Staff on site
extinguished the fire. The Fire Department was notified and inspected the area.
• On June 10, 2004 a staff member took five consumers to the convenience store. One
consumer did not return with the group. Police were notified of her absence. The
consumer had returned to the former board and care home.
• One July 18, 2004 a consumer, who was present at the 4:00 head count, was
unaccounted for at the 5:00 head count. A search of the neighborhood was conducted
and the police were notified. The consumer was located at an apartment where she
had lived prior to admission. The consumer was immediately transferred and admitted
to Contra Costa Regional Medical Center.
• A consumer signed out on independent pass to a local convenience store at 6:15 p.m.
on July 13, 2004. When the consumer had not returned by the 8:30 curfew, staff
conducted a search of the community and contacted family. The police were notified.
The consumer returned on his own at 11:00 p.m. after using BART to attend an A's
game in Oakland. The police were notified of his return.
• On August 4, 2004 two consumers were escorted to a local department store. While
one consumer was in the dressing room, the staff member took the second consumer
to the book section. When the staff member returned to the dressing area, the
consumer could not be located. An additional staff member was sent to assist with the
search. The police were notified. The consumer was subsequently located at her
former apartment. She refused to return to Crestwood, but went to Contra Costa
Regional Medical Center the following morning.
The Neighborhood Oversight Advisory Committee has met several times. However, they
are currently struggling with communication issues and coordination of meeting times that
are convenient to all parties. Health Services Department, Mental Health staff continue to
make themselves available to attend these Committee meetings when invited.
Crestwood has recently hired an additional staff member to cover the lobby between the
hours of 5:00 p.m. and 10:00 p.m.

Supervisor DeSaulnier noted for the record that as of August 31, 2004,there are now 42 residents in two
distinct programs at the Crestwood Facility. Of those 42 residents, 9 area enrolled in Diablo Valley
College, 4 are working part time, and 12 are in vocational training. There have been a total of 13 incidents,
five of which are noted in the Board material-those occurring since the last report.
He further noted the importance of continued work the make the facility a success for clients and the
neighborhood, and that the monitoring of the, program continue to be public and transparent.
The Supervisor reiterated the Board's commitment to work with the neighbors on legitimate concerns and
the success of the program.
Supervisor DeSaulnier moved the recommendations with the added direction that:
From here forward the last quarterly report of the calendar year include a total of all reports for the
preceding calendar year,that any violations or serious potential violations to state, County or city laws or
contracts be included in that report, and that whenever there is an incident, to notify the Board and to
include corrective actions taken either by Crestwood or by the Division, that these may be handled in a
timely fashion;
and that the Senior Deputy County Administrator Dorothy Sansoe work with Supervisor DeSaulnier's
office to set up a meeting with Ben Paredes of the Neighborhood Oversight Committee(Pleasant Hill)..
The recommendations and added direction were adopted by unanimous vote with all Supervisors present.
