California (U.C. Davis School of Internal Medicine Department

RECO NDM ION(S) :
Approve and authorize the Health Services Director, or his designee
(Jeff Smith, M. D. ) , to execute on behalf of the County, Agreement #26-
162-19 (University SO4-00243V) with the Regents of the University of
California (U.C. Davis School of Internal Medicine Department) , in an
amount not to exceed $561, 680, for the period from July 1, 2004 through
June 30, 2005, to provide Hematology and Oncology Physician services
for Contra Costa Regional Medical Center. In accordance with the term
of the agreement, County agrees to indemnify and hold harmless
Contractor from claims, losses, damages, liabilities, costs, expenses
or obligations arising out of, or resulting from CCRMC' s wrongful or
negligent conduct in the performance of this Contract.

This Contract is funded in the Department' s Enterprise I Budget. The
services provided under this agreement are billable to patients and
appropriate third party payors .

For the past several years, the Board of Supervisors has approved
contracts with the Regents of the University of California (U.C. Davis
School of Medicine) for professional services in several medical
specialties. Physicians who are U.C. Davis School of Medicine faculty
members provide services to County' s inpatients and outpatients at
Contra Costa Regional Medical Center and the Contra Costa Health
Centers.
On May 13, 2003, the Board of Supervisors approved Contract #26-162-18
with the Regents of the University of California (U.C. Davis School of
Internal Medicine Department) to provide oncology services for
patients treated at Contra Costa Regional Medical Center. Approval of
Contract #26-162-19 will allow the Contractor to continue providing
Hematology/Oncology clinic services, through June 30, 2005.
Three sealed/certified copies of this Board Order should be returned
to the Contracts and Grants Unit.

Approve and authorize the Health Services Director, or his designee, (Jeff Smith, M.D.) to execute on
behalf of the County, Contract Amendment Agreement #26.362-5, with Dobri Kiprov, M.D., Inc, a
Professional Corporation, effective ,June 15, 2004, to amend Contract #26.362.4, to increase the
Contract Payment Limit by$37,500 from $62,500 to a new total payment limit of$100,000 with no
change in the original term through March 31, 2005.

On April 20, 2004, the Board of Supervisors approved Contract #26.3624 with Dobri Kiprov, M.D.,
Inc. (formerly Bay Area Mobile Apheresis Program), for provision of therapeutic apheresis services, for
the period from April 1, 2004 through March 31, 2005.
At the time Contract #26.362-4 was negotiated, the payment limit was based upon targeted levels of
utilization. However,the utilization of services has exceeded the Department's projections.
Approval of this Contract Amendment Agreement #26-362.5 will allow Contractor to provide
additional units of apheresis services through March 31, 2005

Approve and authorize the Health Services Director, or his designee
(Wendel Brunner, M. D. ) to execute on behalf of the County, Contract #22-
776-5 with the Speech Pathology Group, a corporation, in an amount not
to exceed $58, 220, to provide speech therapy services at the George
Miller Centers and Contra Costa Regional Medical Center (CCRMC)
Rehabilitation Services, for the period from July 1, 2004 through June
30, 2005 .
FISCAL IMPACT:
This Contract is 80% State funded and 20% County Funded.
iCK ROUND(REASON(S) FOR RE CO1 UNDATION(S) :
Many of the infants and toddlers participating in George Miller Center
(GMC) early intervention services have oral, motor, feeding and speech
delays . The GMC program services are considered comprehensive early
intervention services, which address the global developmental needs of
the children. The agency who authorizes the services, refers clients,
and provides funding (Regional Center of the East Bay) requires that GMC
provide these services .
CCRMC Rehabilitation Services occasionally receives referrals to treat
patients who are in need of a speech therapy consultation or direct
speech therapy. Typically the referrals for therapy will come from the
Contra Costa Health Plan.
On June 3, 2003, the Board of Supervisors approved Contract #22-776-4
with the Speech Pathology Group, to provide speech therapy services at
the George Miller Centers, for the period from July 1, 2003 through June
30, 2004 .
Approval of Contract #22-776-5 will allow Contractor to continue to
Provide services to the George Miller Centers and Contra Costa Regional
Medical Center (CCRMC) Rehabilitation Services, through June 30, 2005.

APPROVE and AUTHORIZE the County Administrator or designee to execute a contract with Kevin
W. Harper, CPA, for accounting services related to the Contra Costa County Public Facilities
Corporation (CCCPFC) and Contra Costa County Public Financing Authority(CCCPFA), in an amount
not to exceed $82,000 for the period from July 1, 2004 through June 30, 2006. (Budgeted)
FISC IM IkCT:
The costs associated with year-end financial statement preparation services are proportionately
allocated out to the various bonds now outstanding that have been issued by the CCCPFC and
CCCPFA, and then to the various departments for which these bonds were issued for their capital
projects. This allocation methodology results in minimal financial costs to any one department or
budget unit for these services.
BACKG NO:
As part of the County's annual financial reporting and auditing process, the County's two bonding
authorities, the Contra Costa County Public Facilities Corporation (CCCPFC) and Centra Costa
County Public Financing Authority (CCCPFA), are each required to develop stand-alone financial
statements. These financial statements are then rolled up into the County's larger audited annual
financial report.
The firm selected for this service, Kevin W. Harper, CPA, is an SBE firm familiar with
governmental mounting issues surrounding debt issuing authorities and corporations. The
proposed contract is for a two-year term.
