(„ . 75 SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION RECOMMENDED ACTION- Approve C I"ION:Approve and authorize the Health Services Director, or his designee (Rich Harrison), to execute on behalf of the County, Contract#27-1.59-4 with Abbas Mandavi,M.D.,Inc.,a professional corporation,in an amount not to exceed . $175,000, for provision of professional primary care services for Contra Costa Health Plan members,for the period from September 1, 2004 through August 31, 2005. FISCAL IMPACT: This Contract is funded by Contra Costa Health Plan member premiums. Costs depend upon utilization. As appropriate, patients and/or third party payors will be billed for services. BACKGR.01`.iI4T/REASON(S) FOR RECOMMENDATION(S): On February 1, 1997 the Local Initiative for Medi-Cal managed care in Contra Costa County was implemented. Local Initiatives are required to include traditional Medi-Cal providers from the community in their provider networks.This Contract is necessary to meet State mandates to expand the number of community providers for the Local Initiative, along with a Department of Corporations audit finding that requires formal contracts with low volume providers. On September 9, 2003,the Board of Supervisors approved Contract#27.1.59.3 with Abbas Mandavi,M.D.,Inc.,for the period from September 1, 2003 through August 31, 2004. Approval of Contract #27-1.59.4 will allow the Contractor will provide professional primary care services to Health Plan members through August 31, 2005. FISCAL IMPACT: These Contracts are funded by 49% State and 51% Federal FFP Medi-Cal Funds. BACKGROUND/REASON(S)FOR RECOMMENDATION(S): On January 14, 1997, the Beard of Supervisors adapted Resolution#97/17, authorizing the Health Services Director to contract with the State Department of Mental Health to assume responsibility for Medi-Cal mental health specialty services. Responsibility for outpatient mental health specialty services involves contracts with individual, group and organizational providers to deliver these services. Approval of this Board Order will allow the Contractors to continue providing Medi-Cal mental health specialty services through June 30,2006. White, M. D. 77_ SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION RECO LADED ACTION: Approve and authorize the Health Services Director, or his designee (Jeff Smith, M. D. ) , to execute, on behalf of the County, Contract Cancellation Agreement #26-931-7, to cancel Contract #26- 931-6 with Thomas E. White, M. D. , (Specialty: Otolaryngology) , effective at the close of business on August 31, 2004 . FISCAL IMPACT: This Contract was based on cost to the County depending upon utilization. As appropriate, patients and/or third party payors was billed for services. BACKQROtFNT11REAS0N(S) FOR RECOM[MNOATION(S) : On September 10, 2002, the Board of Supervisors approved Contract #26- 931-6 with Thomas E. White, M. D. for the provision of services in Contractor' s medical specialty, Otolaryngology, including but not limited to, consultation, on-call coverage, training, administration, medical and/or surgical procedures, for the period from October 1, 2002 through September 30, 2005.
