This Contract supports all five of the community outcomes established in the Children's Report Card:
1) "Children Ready for and Succeeding in School"; 2) "Children and Youth Healthy and Preparing for
Productive Adulthood"; 3) "Families that are Economically Self Sufficient"; 4) "Families that are Safe, Stable
and Nurturing"; 5)"Communities that are Safe and Provide a High Quality of Life for Children and Families"
by assisting individuals and families in times of emergency and removing barriers to self sufficiency.
BACKGROUND!
Volunteer and Emergency Services Program of the Employment and Human Services Department(EHSD)
provides emergency supplemental resources to stabilize low-income families and individuals in crisis.
Resources such as food, furniture, clothing, toys, and education supplies assists families to remove barriers to
employment,housing, and other basic living needs empowering families to move to self-sufficiency. Additional
program services include summer camperships, holiday food baskets, and tickets to cultural and sporting events,
which support the entire families' quality of life experience. The program receives a grant from the San
Francisco Chronicle's Season of Sharing (SOS) campaign, which provides housing assistance for families in
crisis.
Ms. McAlister will provide West County Season of Sharing(SOS) Coordinator services for FY 2004/2005.
Additionally, she will network with community-based organizations,make presentations to EHSD units, solicit
financial support through cash and in-kind donations, and manage special projects for Volunteer and Emergency
services.

RACKGROUNI)o
The contract will continue the services of Ms. Donna Bledsoe as clerical supervisor for a total of ten months
after her county retirement,which was effective February 29, 2004. Having her as contractor continue will
facilitate the smooth transition of the new Clerical Supervisor. Ms. Bledsoe will continue to assist in the
development of new clerical policies and office procedures as it pertains to forms, publications and other new
issues brought on by automation. The goal is to develop a new department-wide manual for clerical policies
and procedures. She will continue to assist management in defining problem issues within the clerical unit and
recommend possible solutions.

Approve and authorize the Health Services Director, or his designee (Donna Wigand)to execute on
behalf of the County, Novation Contract #74-190-3 with Mental Health Management 1, Inc. (dba
Canyon Manor), a corporation, in an amount not to exceed $147,613, to provide Mental Health
subacute care and treatment services for the period from July 1, 2004 through June 30, 2005. This
Contract includes a six-month automatic extension through December 31, 2005, in an amount not to
exceed$73,806.
FISCAL IMPACT:
This Contract is funded 100%by Mental Health Realignment.
BACKGROUND REASON(SS)FOR RECOMMENDATION(S):
This Contract meets the social needs of County's population in that it provides long-term care for
adults with serious mental illness who require skilled nursing inpatient psychiatric care.
On July 15, 2003, the Board of Supervisors approved Novation Contract #74-190-2 with Mental
Health Management 1, Inc. (dba Canyon Manor) for the period from July 1, 2003 through June 30,
2004, (which included a six-month automatic extension through December 31, 2004) for the
provision of Mental Health subacute care and treatment services.
Approval of Novation Contract#74®190-3 replaces the automatic extension under the prior Contract
and allows the Contractor to continue providing services through June 30,2005.

Approve and authorize the Health Services Director, or his designee (Donna Wigand) to execute on
behalf of the County, Contract x#24-778-13 with FamiliesFirst, Inc., a non-profit corporation, in an
amount not to exceed $503,333, to provide to provide Intensive Day Treatment and Mental Health
Services for Seriously Emotionally Disturbed (SED) Children at Mt. Diablo High School, for the
period from July 1, 2004 through June 30, 2005. This Contract includes a six-month Automatic
Extension through December 31,2005 in an amount not to exceed$251,667.
FISCAL IMPACT:
This Contract is funded 18% by Federal Med-Cal, 18% by State Early and Periodic, Screening,
Diagnosis,and Treatment(EPSDT)Program, and 64%Mental Health Realignment.
CHILDREN'S IMPACT STATEMENT:
This Day Treatment Program supports the following Board of Supervisors' community outcomes:
"Children Ready For and Succeeding in School"; "Families that are Safe, Stable,and Nurturing'; and
"Communities that are Safe and Provide a High Quality of Life for Children and Families".
Expected program outcomes include an increase in positive social and emotional development as
measured by the Child and Adolescent Functional.Assessment Scale(CAFAS)
BACKGROUND/REASQN(S)FOR RECOMMENDATION(S):
This Contract meets the social needs of County's population in that it provides intensive day
treatment, mental health services, medication support, and case management services for SED
children to reduce the need for out-of-home placements.
On August 12, 2003, the Board of Supervisors approved Novation Contract #24-778-12 with
FamiliesFirst, Inc., for the period from. July 1, 2003 through June 30, 2004, for the provision of
Intensive Day Treatment and Mental Health Services for SED Children.
Approval of Contract #24-778-13 will allow the Contractor to continue providing services through
Jude 30,2005.
