El'SC-AL IMPA Approval of this agreement will result additional $29,000 of State funding for the Conditional Release Program for FY 2003-2004. No matching County funds are required. On February 10, 2044, the Board of Supervisors approved Standard Agreement#29.441.25 with the State Department of Mental Health, for the Conditional Release Program daring FY 2003.2004. This amendment increases the Contract payment limit by$29,040 which provide additional funds to cover the increased cost of Life Support services. Five certified and sealed copies of this Board Order should be returned to the Contracts and Grants Unit for submission to the State Department of Mental Health. Total revenue to the County will depend entirely upon enrollment volume. No County match is required. The California Major Risk Medical Insurance Board requires that this Contract be a can fidential document for one year and requires that the fiscal provisions attachment remain confidential for a period of four years. This Program provides increased access to maternity, delivery and infant care services for low income women who have neither Medi-Cal nor private health insurance coverage. Under this Agreement, the Contra Costa Health Plan (CCHP) provides comprehensive medical care during a woman's pregnancy and delivery, and for sixty days post-partum. The AIM Program also provides comprehensive medical care to the baby during its first two years of life. Approval of Standard Agreement #29-765-10 will allow CCHP to continue participating in the AIM Program through June 30, 2007. BACKGROUND/REASONS FOR RECOMMENDATIONS Staff seeks to apply for a TLC planning grant to mance the revision of County road design standards, This effort is in response to requests from the Board of Supervisors to develop ways to incorporate recent developments in road modifications that effectively calm traffic in residential areas, as well to ensure our road standards comply with runoff requirements of the standards established by the Regional Water Quality Control Board. Some of these recently developed design standards and requirements are in conflict with the County's existing road design standards. A focal match of$18,750 will be provided via County road funds and/or Measure C funds, for a total project cost of $93,750. Support for City of San Pablo grant application for El Portal Drive Streetscape Project The County and the City of San Pablo jointly developed the El Portal thrive/College Gateway Streetscape Design Plan,for streetscape and safety improvements to El Portal Drive between I- 80 and Rollingwood Drive/Church Lane. The plan was funded through a Transportation for Livable Communities planning grant. The Board of Supervisors approved the plan in 2003. The City of San Pablo has annexed this segment of El Portal Drive, so the City is now the lead agency in implementing the streetscape and safety improvements. The City seeks County support for the grant application. County staff doesn't yet know the amount of funds for which the City of San Pablo will apply. The maximum construction grant in the TLC program is 3 million, with a local match requirement of 11.5 percent, which the City will provide. The Transportation, Water and Infrastructure Committee recommends the Board support the City's application. Although El Portal Drive is now a city street, the unincorporated community of Rollingwood will be among the key beneficiaries of the project, since the neighborhood will have a safer, more attractive street, serving as a gateway into the community. Key features of the project include an attractive masonry safety wall along the north side of El Portal Drive (replacing wooden fences that automobiles sometimes crash through, entering residents' back yards); a wide, multi-use path for bicyclists and pedestrians along El Portal Drive; sidewalk improvements; and streetscaping.Attached is a draft letter of support for the Chair's signature. The County's funds are secured against the property by a deed of trust and specific affordability and use restrictions are incorporated into a loan and regulatory agreement between the County and project developer. in addition to housing, approximately$500,000 in CDBG funds is allocated on an annual basis to economic development projects designed to create jobs for low-income persons.These funds are also typically secured against the project property through a deed and loan agreement. The law firm of Goldfarb and Lipman has previously provided the County with legal assistance in negotiating loan terms and drafting the required legal documents for CDBG and HOME projects.The proposed contract would be a continuation of this relationship. The Welcome Home Baby Program is a prevention and early intervention program providing home visiting for first-time families in Contra Costa County. The identification of first-time families, and provision of weekly home visits emphasizing infant bonding and nurturing child development, health education and medical care, and assistance with employment, housing, transportation and childcare can provide appreciable reductions in child abuse and neglect and other significant health and education concerns.
