RECOMMENDATION : Adopt Position Adjustment Resolution # 19699 reallocating the salary
of Staff Nurse Per Diem as recommended by the Director of Human Resources.
FISCAL IMPACT: The Health Services Department estimates an annual cost savings of
approximately $400,000. This savings will be achieved through reductions in both overtime and
registry costs. The nurse registry is similar to an employment agency that supplies nurses to the
Health Services Department to help them cover staffing shortages. The fee for a registry nurse can
be up to $81 per hour.
BACKGROUND: Currently, Staff Nurse Per Diem has a one step salary range. The relatively
low salary has resulted in both recruitment and retention issues. The Health Services Department
has been unable to find and retain sufficient numbers of per diem nurses to maintain appropriate
staffing levels. As a result the department has had to pay millions of dollars in overtime and registry
costs. A salary survey was conducted by Human Resources that showed Contra Costa's salary to be
lower than the market average. Currently, the Staff Nurse Per Diem salary is 19% below the market
mean salary and 22.5% below the market median salary. The proposed broader salary range will
allow the department to be more competitive in the labor market and alleviate the current recruitment
and retention problems. It also will give the department more flexibility in appointing per diem nurses
to a salary step commensurate with their experience and skills.
CONSEQUENCE OF NEGATIVE ACTION If the salary for this classification is not reallocated,
Health Services will continue having difficulty recruiting and retaining per diem nurses and thus will
be unable to maintain appropriate staffing levels. As a result, the department will continue to have to
pay both overtime and registry costs which totaled more than $6 million last fiscal year.

• This request will not only increase the salary of the Staff Nurse Per Diem
classification but will also broaden the salary structure from a one step range to a
ten step range. The department has been unable to find and retain sufficient
numbers of per diem nurses to maintain appropriate staffing levels. As a result the

salary to be lower than the market average. Currently, the Staff Nurse Per Diem

RECOMMENDATION:
A. APPROVE and AUTHORIZE the Community Services Director, or designee, to
ABOLISH ONE (1) FACS Site Supervisor I — Project position effective August 1, 2004
to realize cost-savings in the Head Start program.
B. ABOLISH project position as specified in Attachment A.
BACKGROUND:
The Board most recently approved cost-saving measures for the Community Services
Department (CSD) on June 26, 2004 as part of the County's reduced funding and increased
funding requirements. As part of the expenditure reduction plan, CSD closed several of it's
one (1) classroom child care centers. As a result of providing adequate notice of site closure
to the children and families served, CSD closed the Silver Child Care Center in Richmond on
July 16, 2004.
The position abolished, as specified in Attachment A (Resolution Number 20041 )
incorporated herein, will allow CSD to realize the projected costs savings for the 20041016 fiscal
year.

SUBJECT: Abolishing One (1) Community Services Department FACS Site Supervisor 1--Project Position
and Laying Off Employee
The Contra Costa County Board of Supervisors in its capacity as governing Board of the County
of Contra Costa and all districts of which it is ex-officio governing beard RESOLVE THAT:
1. The Board has considered the Community Services Department (CSD) staff reduction plan as it
pertains to the affected Head Start program.
2. CSD will issue layoff or displacement notices, as the case may be, and give notice to the affected
employee of the Board's action.
3. Abolishing this position will facilitate the ability of CSD to keep expenditures within the limits of
available funding for its early childhood education program.
4. To the extent that the subjects of this Resolution are within the scope of representation pursuant
to the Meyers-Milias-Brown Act (Government Code Section 3500 et seq.), this Board has offered
to meet with recognized employee organizations upon request concerning this resolution.
5. The Employee Relations Officer shall give notice of this Resolution to recognized employee
organizations representing employees impacted by this action.
6. Recognized employee organizations may submit to the Employee Relations Officer written
requests to meet and confer on specific proposals with respect to this Resolution or resulting
layoffs. This authorization and direction is given without prejudice to the Board's right to reduce
or terminate the operations and services of the County and of districts governed by this Board,
and to eliminate classes of employees or positions as involving the merits, necessity, or
organization of services or activities of the County or districts governed by the Board and not
within the scope of representation.
7. This action is taken without prejudice to pending consulting, meeting, and meeting and conferring
with recognized employee organizations.
& To keep expenditures within available funding, the following position shall be abolished effective
August 1, 2004.

RECOMMENDATION : Adopt Position Adjustment Resolution # 19741 retitiing the
classification of Health Plan Medi-Cal Outreach Representative to Health Plan Outreach
Representative as recommended by the Assistant County Administrator - director of Human
Resources.
FISCAL IMPACT: There is no cost associated with this action.
BACKGROUND: The title change is being requested to more accurately reflect the duties of
the position. In 1997 the Outreach Representative classification was Created to perform door-to-door
marketing of the Contra Costa Health Plan to Medi-Cal recipients. The State Department of Health
Services, which regulates how HMO's market to Medi-Cal recipients, has recently disallowed door-
to-door marketing by HMO's . As a result, the incumbents in this Classification have had a shift in job
duties. They now market the health plan to non Medi-Cal individual enrollees and work to retain
current members.
RESULT OF NEGATIVE ACTION: AS HMO's can no longer market their services doer-to-door to
Medi-Cal recipients, the classification title is misleading. The new title more accurately reflects the
duties of the position.

RECO'MMEN'DATION : Adopt Position Adjustment Resolution # 19746 reallocating the
salaries of the following classifications: (1) Environmental Health Technician (2)
Environmental Health Specialist l (3) Environmental Health Specialist 11 and (4) Director of
Environmental Health - Exempt
FISCAL IMPACT: The estimated annual cost associated with this action is $164,670 and will
be offset by Environmental Health fees charged by the department.
BACKGROUND: The salary increases are needed to address both recruitment and retention
issues. The department has been unable to fill vacant positions for the past three years despite
rigorous recruitment efforts. In addition, staff has been resigning to take positions with agencies that
pay higher wages. A salary survey was conducted by Human Resources which showed Contra
Costa's salaries to be lower than the market average. The Environmental Health Technician's salary
is 15% below the market mean, The Environmental Health Specialist I's salary is 4% below the
market mean and 10% below the median, the Environmental Health Specialist II's salary is 6% below
the market mean and 10% below the median, and the Director of Environmental Health — Exempt's
salary is 6% below the market mean and 4% below the median. This request represents an 8%
salary increase for the Environmental Health Technician, Environmental Health Specialist 1, and
Environmental Specialist 11 classifications and a 6% salary increase for the Director of Environmental
Health -- Exempt classification.
CONSEQUENCE OF NEGATIVE ACTION: If these classifications are not given salary increases,
Health Services may be unable to fill their current vacancies as well as lose current employees to
agencies offering higher wages. Inadequate staffing levels will result in decreased food safety and
security for the people of Contra Costa County as well as delays in approvals for plan checks,
alcoholic beverage control licenses, and complaint investigations.
