CERTIFICATE OF COMPLIANCE AND MEDT-CAL
COST AVOIDANCE PROGRAM AGREEMENT
SPECIFIC REQUEST($)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATION(S):
Authorize the chair of the Board of Supervisors to sign the attached County
Subvention Program Certificate of Compliance and the Medi-Cal Cost
Avoidance Program Agreement as administered by the California Depart-
ment of Veterans Affairs (CDVA). Copy of CDVA bulletin attached for
information.
FISCAL. IMPACT:
The above agreements enhance the county general fund. CDVA
estimates the FY 2003-2004 Contra Costa County allocation for County
Subvention to be approximately $50,000.00 per sections 972 and 972.1 of
the Military and Veterans Code of California. This amount is based on
seven Ball-time employees. The estimated allocation for Medi-Cal Cost
Avoidance is approximately $25,000.00 per section 972.5 of the Military
and Veterans Code.
BACKGROUND/REASONS FOR RECOMMENDATION (S):
CDVA administers the above two revenue programs per California Code of
Regulations, Title 12, Subchapter 4. CDVA conducts annual audits of our
operation to determine if our workload is consistent with reported workload
activity.

SUBJECT: Fiscal Forms Available On CDVA Web Page
This is to inform you that all forms and agreements pertaining to fiscal year 2003/2004 are now available on our
CDVA web page. You may download these forms at www.edva.ca.govv, and by double clicking on Veterans
Services,Fiscal Forms for CVSOs. There is no reason to modify these forms but if you do, you will be
prompted to either cancel or save your changes under a different file name. The forms are d esign t o be
downloaded and printed(only.) Please complete and mailed to: California Department of Veterans Affairs,
Veterans Services Division, 1227 "O" Street, Suite 101, Sacramento,California 95814.
Counties that chose to receive these forms via mail will continue to do so. Please note that the package of
forms issued at the CVSO Conference in May are relevant for FY 02/03 for the period of January I --June 30,
2003 which incidentally are due at this office no later than July 31, 2003. The package also included
Subvention and Medi-Cal Agreements for FY 03/04. Although this office does not require counties to submit
Form DVS-19 (daily reports,) it is recommended, that you maintain your DVS-19's for future auditing
purposes.
If you peruse through the forms on CDVA web page, you'll notice that other than the Claim for Subvention
Funds, Medi-Cal and Subvention Agreements, it is your responsibility to establish what periods of the fiscal
year you are submitting. If you are not sure,please contact Ms.Noriega for further instructions.
Since the forms and agreements pertain to an entire Fiscal Year, the web page will be updated once in early
June of each year, unless otherwise necessary. You will, however,receive two(2)e-mail reminders informing
you that the forms are available and due to this office. The first reminder will take place in early to mid
December followed by another in early to mid June.
Finally, as with any new procedure, if you should encounter any difficulties on the web page, please do not

This bulletin format will be used by the CDVA Veterans Services Division as a standard way of communicating information to County
Veterans service Officers. The objective is to provide a format that can be easily maintained by the CVSO. The first two digits in number of
each Bulletin will show the year of issue,the following number indicates the consecutive number of the issue during the year.

SUBVENTION CERTIFICATE OF COMPLIANCE
FISCAL YEAR 2003/2004
COUNTY SUBVENTION PROGRAM
Charge:
Contribution to counties toward compensation and expenses of their County Veterans
Service Office according to Military and Veterans Code Sections 972, and 972.1, a State
General Funds Expenditure, and 972.2, a Special Fund Expenditure.
County Certification:
I certify that Contra Costa County has appointed a veteran to serve as the
County Veterans Service Officer according to California Code of Regulations Title 12,
Subchapter 4. This County Veterans Service Officer will administer the aid provided for
in Military and Veterans Code Division 4, Chapter 5.
I farther certify that the County Veteran Service Officer will assist every veteran of the
United States, as well as their dependents and survivors, in presenting and pursuing such
claim as they may have against the United States, The County Veterans Service Officer
and all accredited staff will also assist in establishing veterans, dependents and survivors'
rights to any privilege,preference, care or compensation provided for by the laws and
regulations of the United States, the State of California, or any local jurisdiction.
I also agree that this county, through the County Veterans Service Office, will maintain
annual records for audit. These records will be maintained until the final allocation of
funds for the subject fiscal year is issued by the CDVA. We will also submit reports in
accordance with the procedures and timelines established by CDVA. The County
Veterans Service Officer will permit CDVA representatives to inspect all facilities and
records.
I further authorize the County Veterans Service Officer to actively participate in the
promotion of the California Veterans License Plate program.

MEDI-CAL CERTIFICATE OF COMPLIANCE

Conga Costa COUNTY
MEM-CAL COST AVOIDANCE PROGRAM
I certify that Contra Costa County has appointed a County Veterans Service
Officer(CVSO) in compliance with California Code of Regulations, Title 12,
Subchapter 4. Please consider this as our application to participate in the Medi-Cal Cost
Avoidance Program authorized by Military and Veterans Code Section 972.5.
1 understand and will comply with the following:
1. All activities of the CVSO for which payment is made by the CDVA under
this agreement will reasonably benefit the Department of Health Services
(DHS)or realize cost avoidance to the Medi-Cal program. All County
Eligibility Workers who generate a Form CA-5 (Veterans Benefits Referral)
will be instructed to indicate the applicant's Welfare Aid Code on the face of
the form.
2. All monies received under this agreement will be allocated to and spent on the
salaries and expenses of the CVSO,
3, This agreement is binding only if federal funds are available to the CDVA
from the.DHS.
4. The CVSO is responsible for administering this program according to the
California Code of Regulations, Title 12, Subchapter 4.
