SPECIFIC REQUEST(S)OR RECOMMENDATION(5)&BACKGROUND AND JUSTIFICATION
RECOM ENDATION(S):
Approve and authorize the Health Services Director, or his designee (Donna 'Wigand) to execute
on behalf of the County, Novation Contract #24-680-15 with Telecare Corporation, a non-profit
corporation, in an amount not to exceed $1,128,101, to provide subacute care to severely and
persistently mentally ill (SPMI) clients, for the period from July 1, 2003 through June 30, 2004.
This',Contract includes a six.-month automatic extension through December 31, 2004, in an amount
not to exceed:$564,053.
FISCAL IMEPACT:
This Contract is 100'% funded by Mental Health Realignment funds as a result of savings generated
from the County's reduction of client placements at State Hospitals.
BACKGROUND/REASON(S)FOR COMM ENDATIONLS):
This Contract is part of the Department's cost saving plan to reduce the number of high-cost State
Hospital beds by developing alternative placements.
Can November 5, 2002, the Board of Supervisors approved Novation Contract #24-680-14 with
Telecare Corporation for the period from July 1, 2002 through June 30, 2003, (which included a six-
month automatic extension through December 31, 2003) for the provision of subacute care for
County residents who are severely and persistently mentally ill (SPMI) at Contractor's 'Villa
Fairmont Mental Health Center, Garfield Mental Health Center, Gladman Psychiatric Health
Facility, and Morton Bakar Center.
Approval of Novation Contract #24-680--15 replaces the automatic extension under the prior
agreement and allows the Contractor to continue providing services through June 30, 2004.

RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee,
(Jeff Smith, M.D. ) to execute on behalf of the County, Contract #26-
915-8 with Martha E. Corcoran, M.D. , a self-employed individual,
(Specialty: Otolaryngology) , for the period from October 1., 2003
through September 30, 2006, in an amount not to exceed $435, 000, for
the provision of professional Otolaryngology services for patients at
Contra Costa Regional Medical Center.
FISCAL IMPACT:
Cost to the County depends upon utilization. As appropriate, patients
and/car third party payors will be billed for services .
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
On October 8, 2002, the Board of Supervisors approved Contract #26-
915-7, with Martha Corcoran, M.D. , for the provision of professional
Otolaryngology services to patients at Contra Costa Regional Medical
Center and Contra. Costa Health Centers, for the period from October 1,
2002 ',through September 30, 2003 .
Approval of Contract #26-915-8 will allow Contractor to continue
providing services through September 30, 2006 .

RECOMMENDATION C S) :
Approve and authorize the Health Services Director or his designee (Donna
Wigand, L.C.S.W. ) to execute on behalf of the County, Contract #74-120-3 with
Island View Academy, a non-profit corporation, in an amount not to exceed
$40, 150, for the period from July 1, 2003 through June 30, 2004, to provide AB
3632 required Intensive Day Treatment Program, Mental Health services,
Medication Support, and Case Management services for mentally or emotionally
disturbed children. This Contract includes a six-month automatic extension
through December 31, 2004 , in an amount not to exceed $20, 075 .
FISCAL IMPACT:
This Contract is 100% SB90 State funded.
CHILDREN'S IMPACT STATEMENT:
This Contract supports the following Board of Supervisors community outcomes:
1) Children ready for and succeeding in school; 2) Families that are safe,
stable, nurturing; and 3) Communities that are safe and provide a high quality
of life for children and families . The expected outcomes include all goals
identified by Children' s Statewide System of Care guidelines including: A)
Increase and maintain school attendance as measured by school records; B)
Increase in positive social and emotional development as measured by the Child
and Adolescent Functional Assessment Scale (CAFAS) ; C) Increase in family
satisfaction - as measured by the Parent Satisfaction Survey; D) Decreased used
of acute care system; and E) Placement at discharge to a lower level of care.
BA.CKL ROLTIM/REASON(S) FOR RECOMMENDATION(S) :
In October 2002 , the County Administrator approved and the Purchasing Services
Manager executed Contract #74-120-2 with Island View Academy, for the period
from July 1, 2002 through June 30, 2003, for the provision of an Intensive Day
Treatment Program, Mental Health Services, Medication Support, and Case
Management services for Mentally or emotionally disturbed children., including
Special Education Pupils (SEP) for Individualized Education Plan (IEP) required
services under AB3632 . Approval of Contract #74-120-3 will allow the Contractor
to continue to provide mandated services, through June 30, 2004 .

# 26-996-1 with East .Bay Cardiology Medical Group, a Corporation
(Specialty: Cardiology) , in an amount not to exceed $39, 000, for
the period from October 1, 2003 through September 30, 2005, for the
provision of professional Cardiology physician services for patients
in West County.

For ',a number of years the County has contracted with Medical and
Dental Specialists to provide specialized professional services
which. are not otherwise available in its hospital and clinics .
On September 24, 2002, the Board of Supervisors approved Contract
#26-,996 with East Bay Cardiology Medical Group, for the period from
October 1, 2002 through September 30, 2003 to provide Cardiology
physician services, including, but not limited to, consultation and
clinic coverage, for West County patients at the Richmond Health
Center.
Approval of Contract #26-996-1, will allow East Bay Cardiology
Medical Group to continue to provide services through September 30,
2005 .

Approve and authorize the Health Services Director or his designee (Wendel Brunner, M.D.) to
execute on behalf of the County, Contract#22-877 with Opportunity West, a nonprofit corporation,
in an amount not to exceed$54,400 for the period from September 1, 2003 through August 31, 2004,
for provision of needle exchange services to reduce the transmission of HIV Infection, Hepatitis and
other diseases in Contra Costa County.
USCAL EUPA T.
Neither Federal or State funds may be used to support needle exchange programs. In the past,
County General Funds have been used to purchase syringes and related materials for distribution to
injection drug users in Contra Costa County. This Contract will be funded 100% by County General
Funds in an amount not to exceed $54,400.

Under this Contract, Opportunity West will provide needle exchange services in Contra Costa County
to reduce the transmission of HIV and Hepatitis C among injection drug users, their partners and
from women to their children, through.August 31, 2004.
