Approve and authorize the Health Services .Director, or his designee (Milt Camhi), to execute on behalf of the County,
Contract #27-426-2 with Hilltop Pediatric Group, a coporation, in an amount not to exceed $115,000, for the period
from August 1, 2003 through July 31, 2004, for provision professional primary care services for Centra Costa Health
Plan members.
FISCAL IMPAC
This Contract is funded by Contra Costa Health Plan member premiums. Casts depend upon utilization. As
appropriate, patients and/or third party payors will be billed for services.

On February 1, 1997 the Local Initiative for Medi-Cal managed care in Contra Costa County was implemented. Local
Initiatives are required to include traditional Medi-Cal prodders from the community in their provider networks. This
Contract is necessary to meet State mandates to expand the number of community providers for the Local Initiative,
along with a Department of Corporations audit finding that requires formal contracts with low volume providers.
Under Contract #27-426-2, the Contractor will provide professional primary care services to Health Plan members
through. July 31, 2004.

RECOMMEINDATIQN(S):
Approve and authorize the Health Services Director, or his designee (Wendel Brunner, M.D.) to
execute on behalf of the County, Novation Contract #22-137-32 with Family and Community
Services,Inc. (dba Senior Outreach Services of Contra Costa),a non-profit corporation, in an amount
not to exceed $269,808, to provide home-delivered meal services for the Senior Nutrition Program,
for the period from July 1, 2403 through June 34, 2004. This Contract includes a four-month
automatic extension through October 31,2004,in an amount not to exceed$89,836.
FISCAL IMPACT:
This contract is 100%federally funded under Title III-C(1)of the Older Americans Act of 1965. No
County funding is required.
BACKGR0jjND/REASONS)FOR RECC1MM ENDATIQ S):
This Contract meets the social needs of County's population in that it provides home delivered meals
on 2.50 serving days, to an average of 540 senior citizens, to ensure they receive at least 1l3 of their
daily nutritional requirements.
On October 22, 2002, the Board of Supervisors approved Contract #22-137-31 with Family and
Community Services, Inc. (dba Senior Outreach. Services of Contra Costa) for the period from July
1, 2002 through June 30, 2003, which included a four-month automatic extension through October
31,2403, for the provision of home delivered meals services for the Senior Nutrition Program.
Approval of Novation Contract #22-1.37-32 will replace the four-month automatic extension under
the prior agreement and allow the Contractor to continue providing services through June 30,2004.

RECOMMENDATION(S):
Approve and authorize the Health Services Director, or his designee (Wendel Brunner, M.D.) to
execute on behalf of the County, Novation Contract #22-033-44 with Family and Community
Services, Inc. (dba Senior Outreach Services of Contra Costa), a non-profit corporation, in an amount
not to exceed$173,503, to provide congregate meal services for the Senior Nutrition Program for the
period from July 1, 2003 through June 30, 2004. This Contract includes a four-month automatic
extension through October 31, 2004,in an amount not to exceed$57,829.
FISCAL,IMPACT:
This contract is 100%federally funded under Title III-C(1) of the Older Americans Act of 1965. No
County funding is required.
BACKGROUND/REAS'ON S)FOIL RECOMMENDATIONS}:
This Contract meets the social needs of County's population in that it provides meals on 250 serving
days, at seven sites throughout the County, to an average of 225 senior citizens, to ensure they
receive at least 1/3 of their daily nutritional requirements.
On November 5, 2002, the Board of Supervisors approved. Contract #22-033-43 with Family and
Community Services, Inc. (dba Senior Outreach Services of Contra Costa) for the period from July 1,
2002 through June 30, 2003, which included a four-month automatic extension through October 31,
2003,for the provision of congregate meal services for the Senior Nutrition Program.
Approval of Novation Contract #22-033-44 will replace the four-month automatic extension under
the prior agreement and allow the Contractor to continue providing services through June 30,2004,

Approve and authorize the Health Services Director, or his designee (Chuck Deutschman) to execute on behalf
of the County, Contract #74-144-5 with Ujima Family Recovery Services, a non-profit corporation, in an
amount not to exceed $5403,219, to provide residential substance abuse treatment services for pregnant and/or
parenting women, and their children, for the period from July 1, 2003 through June 30, 2004.
DISCAL IMPACT;
This Contract is funded by State Prop 10 funds through the Contra Costa Children and Families Commission.
C.HILDUNIS ll PACT S AT NT;
This Prop 10 program supports the Board of Supervisors' "Families that are Safe, Stable, and. Nurturing"
community outcome by providing parenting education, stability, and safety for mothers (and their children)
and pregnant women who are alcohol and drug dependent, while they are in substance abuse treatment.
Expected outcomes include delivery of drug-free babies, decreased use of alcohol, tobacco and other drugs,
reduction in the number of relapses, and creation of a sober social network.

This Contract meets the social needs of County's population in that it provides family-centered alcohol and
drug treatment services to pregnant and parenting women, in order to prevent perinatal substance abuse and
maintain appropriate family relationships.
On February 25, 2003, the Board of Supervisors approved Contract 74-144-4 with Ujima Family Recovery
Services, for the period:from January 1, 20103 through June 30, 2003, for the provision of residential substance
abuse treatment services for pregnant and/or parenting women, and their children.
Approval of Contract #74-144-5 will allow the Contractor to continue providing services through June 30,
2004.

Approve and authorize the Health Services Director or his designee (Donna Wigand) to execute on behalf of
the County, Contract#24-920-6 with Fred. Finch Youth Center, a non-profit corporation, in an amount not
to exceed $1,223,100, for the period from July 1, 2003 through June 30, 2004, to provide an intensive Clay
treatment program and medication support services for Seriously Emotionally .Disturbed children at Fred.Finch
Youth Center Residential/Day Treatment Programs.
FISCAL IMPACT:
This Contract is funded 50% by Federal Financial Participation, 20% State EPSDT and. 30% from other
participating Regional Centers and Counties funds.
C'HILPREN'S,IMPACT STATEMENT:
This mental health day treatment program supports the following Board of Supervisors community outcomes:
"Children ready for and succeeding in school", "Families that are safe, stable and nurturing", and
"Communities that are safe and provide a high quality of life for children and families". Expected program
outcomes include all goals identified by Children's Statewide System of Care guidelines including a decreased
use of acute care systems and placement at discharge to a lower level of care.

This Contract meets the social needs of County's papulation in that it provides mental health services,
including: assessments; individual, group and family therapy; and medication support for Seriously
Emotionally Disturbed (SED) and developmentally delayed middle and high school aged children and their
families.
On July 17, 2001, the Board of Supervisors approved Contract#24-920-4 (as amended by #f24-920-5) with
Fred Finch Youth Center, for the period from July 1, 2001 through June 30, 2002, to provide an intensive
day treatment program and medication support services for seriously emotionally disturbed children at its Fred
Finch Youth Center Residential/Day Treatment Programs. Approval of Contract #24-920-6 will allow the
Contractor to continue providing services, through June 30, 2004.
