R:ECQMMENDM ACTION:
Approve and authorize the Health Services Director, or his designee (Milt Camhi), to execute on behalf
of the County, Contract #27-394-3 with Yogam Krishnamoorthy, MD. (dba Kris Yogam, M.D.), a
professional corporation, in an amount not to exceed $65,000, for the period from August 1, 2003
through July 31, 2004, for the provision of professional DB/GYN services for the Contra Costa Health
Plan.
FISCAL IMPACT:
This Contract is fended by Contra Costa Health Plan (Health Plan) member premiums. Costs depend
upon utilization. As appropriate, patients and/or third party payors will be billed for services.

The Health Plan has an obligation to provide certain specialized professional health care services for
its members under the terms of their Individual and Group Health Plan membership contracts with the
County.
On September 17, 2002, the Board of Supervisors approved Contract #27-394-2 with Yogam
Krishnamoorthy, M.D., for the Period from August 1, 2002 through July 31, 2003,
Approval of Contract #27-394-3 will allow the Contractor to provide professional OB/GYN services
to Contra Costa Health Plan members, through July 31, 2004.

On September 10, 2002, the Board of Supervisors approved Contract#27-480-1 with K.H. Connie Nang-Chen,
O.D., for the period from August 1, 2002 through July 31, 2003.
Approval of Contract #27-480-2 will allow the Contractor to continue to provide services, through July 31,
2004.

RACKCRQUND/REASON(S_FOR RECOMMENDATIONS),:
4n February 1, 1997 the Local Initiative for Medi-Cal managed care in Contra Costa County was implemented. Local
Initiatives are required to include traditional Medi-Cal providers from the community in their provider networks This
Contract is necessary to meet State mandates to expand the number of community providers for the Local Initiative,
along with a Department of Corporations audit finding that requires formal contracts with low volume providers.
On September 24, 2002, the Board of Supervisors approved Contract#27-171-2 with Alliance Medical Group, Inc, for
the period from August 1, 2002 through July 31, 2002.
Approval of Contract#27-171-3 will allow the Contractor to provide professional primary care services to Health Plan
members, through July 31, 2004.

RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee (Jeff Smith,
M.D. ) , to execute on behalf of the County, Sharing Agreement #V612S-6162 (County
#26-346-4) , with the U.S . Department of Veterans Affairs, Northern California.
Health Care System (VANCHCS) , for the period from August 1, 2003 through July
31, 2004 , to provide Nuclear Medicine Services in accordance with the rates set
forth in the Contract .
FINANCIAL IMPACT:
This Contract is funded in the Health Services Department Is Enterprise 1 Budget.
Under the term of the agreement VANCHCS will continue to be paid a fee for each
service in accordance with the attached fee schedule, which is incorporated in
the Contract .
The services provided for the County' s patients under this Contract are billable
to patients and third party payors .
REASONS FOR. RECOMMENDATIONS/BACKGROUND:
For many years the County and U.S. Department of Veterans Affairs has maintained
a mutual sharing agreement which has made available to the County specialized
medical services not otherwise available due to lack of resources, equipment,
and personnel . These services included specialized laboratory testing,
radiology services, nuclear medicine studies, CT scans, MRI ' s, dermatology,
gastroenterology, urology, audiology and speech, and ophthalmology services.
The County provided Emergency Room treatment and inpatient care, including
certain ancillary services, for VA-referred patients .
On September 1, 1998, the Board of Supervisors approved Sharing Agreement #26-
346 (as amended by Contract Modifications #26-346--1 through #26-346-3) with
VANCHCS (under the auspices of the VANCHCS Nuclear Regulatory Commission
License) to provide a full range of Nuclear Medicine Services to County' s
patients at the VA Outpatient Clinic in Martinez and/or the Contra Costa
Regional Medical Center' s Nuclear Medicine Department, through July 31, 2003 .
Approval of Sharing Agreement #26-346-4 will allow VANCHCS to continue providing
nuclear medicine services through July 31, 2004 .
The Contract documents have always been prepared by the Veterans Administration.

Alta Bates Medical Center for its Herrick Hospital
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMENDATIQN(S) :
Approve and authorize the Health Services Director or his designee (Donna
Wigand) to execute on behalf of the County, Contract #24-794-5 (5) with
Alta Bates Medical Center for its Herrick Hospital, for the period from
July 1, 2003 through June 30, 2004, for Fee-For--Service (FFS) Medi.-Cal
acute psychiatric inpatient Services, to be paid in accordance with the
following rate schedule :
$605. 00 For each Adult Medi-Cal "Acute Psychiatric Day of Service" ,
$670 . 00 For each Adolescent Medi-Cal "Acute Psychiatric Day of
Service" ,
$795 . 00 For each non-Medi-Cal "Acute Psychiatric Day of Service" , and
$289 . 13 For each "Administrative Day"
FISCAL IMPACT:
This Contract is funded 100° by acute psychiatric inpatient consolidation
funds .
BACK089M/RE-ASON(S) FOR T
Chapter 633 , Statutes of 1994 , AB 757, authorized the transfer of state
funding for Fee-For-Service/Medi-Cal (FFS/MC) acute psychiatric inpatient
hospital services from the Department of Health Services to the
Department of Mental Health (DMH) . On January 1, 1995, the DMH
transferred these funds and the responsibility for authorization and
funding of Medi-Cal acute psychiatric inpatient hospital services to
counties that chose to participate in this program.
On July 12 , 1994, the Board of Supervisors notified the State of its
intent to function as the Local Mental Health Care Plan and to assume the
responsibility of inpatient consolidation. The State requires that
counties contract with those hospitals which are designated "safety net
providers" and certain hospitals designated as "traditional providers" .
Therefore, the County is required to contract with Herrick Hospital (a
traditional provider. )
Under Contract #24-794-5 (5) Alta Bates Medical Center for its Herrick
Hospital will provide inpatient psychiatric hospital services through
June 30, 2004 .
