The Board of Supervisors of Contra Costa County RESOLVES that:
The County of Contra Costa on May 21, 2003 contracted with Vanguard Construction, for new curb,
gutter, sidewalk and curb ramps at several locations along California Street, Tullibee Road, Mariposa
Street, Trigger Road and Dempsey Way in the Rodeo area, with First National Insurance Company of
America as surety, for work to be performed on the grounds of the County; and
The Public Works Director reports that said work has been inspected and complies with the approved
plans, special provisions and standard specifications and recommends its acceptance as complete as of
June 27, 2003.
Therefore, said work is ACCEPTED as completed on said date, and the Clerk shall file with the County
Recorder a copy of this Resolution and Notice as a Notice of Completion for said contract.
PASSED BY THE BOARD on August 12, 2003 by the following vote:

The Board of Supervisors of Contra Costa County RESOLVES that:
The County of Centra Costa on April 16, 2003 contracted with Redgwick Construction Company, for
overlaying a portion of Amend Road from Heavenly Ridge Lane to Travaiini Court in the Richmond area,
with Safeco Insurance Company of America as surety, for work to be performed on the grounds of the
County; and
The Public Works Director reports that said work has been inspected and complies with the approved
plans,special provisions and standard specifications and recommends its acceptance as complete as of
June 10, 2003.
Therefore, said work is ACCEPTED as completed on said date, and the Clerk shall file with the County
Recorder a copy of this Resolution and Notice as a Notice of Completion for said contract.

The County of Contra Costa on July 24, 2002 contracted with Valentine Corporation, for bridge seismic
rehabilitation at four bridges; Freeman Road Bridge, Hawthorne Drive Bridge, Pacific Avenue Bridge,and
Del Monte Drive Bridge in the west and central County areas, with Travelers Casualty and Surety
Company of America as surety, for work to be performed on the grounds of the County; and
The Public Works Director reports that said work has been inspected and complies with the approved
plans, special provisions and standard specifications and recommends its acceptance as complete as of
June 30, 2043.
Therefore, said work is ACCEPTED as completed on said date, and the Clerk shall file with the County
Recorder a copy of this Resolution and Notice as a Notice of Completion for said contract.

APPROVE Kirker Pass Road Open Grade AC Overlay project contingency fund increase of
$100,000 for a new total of$123,132 effective August 12, 2003 and APPROVE and AUTHORIZE
the Public Works Director, or designee, to execute Contract Change Order No. 1 with MCK
Services, Inc., effective August 12, 2003, in an amount not to exceed $120,000 to a new payment
limit of$351,321, for existing item increases.
11. Financial lmpact:
The project contingency funds are currently insufficient to coverthe increased cost.The increase in
contingency funds will be funded by Measure C Funds.

111. Reasons for Recommendations and Background:
Additional funding is needed to pay the contractor for increases in the contract work.The additional
work was needed because the project limits were extended so that all of the existing open graded
material could be removed and replaced. Extension of the project limits also resulted in an increase
of grinding and striping detail. The additional work costs exceed the project contingency fund.
Approval of the project contingency fund is required to compensate the contractor for the additional
work.
IV. Consequences of Negative Action:
The lack of approval would prevent successful completion of this contract and preclude payment to
the contractor for required work.

Director, or designee, to execute Contract Change Order No. 2 with MCI{ Services, Inc., effective
August 12, 2003, in an amount not to exceed $25,000 to a new payment limit of $197,000, for
unforseen extra work.
ll. Financial lmpact:

Ill. Reasons for Recommendations and Background:
Additional funding is needed to pay the contractor for changes in contract work at Center Avenue,
& Marsh [give (site `B') The changes to work include additional grinding and placement of dense
graded asphalt concrete in lieu of the rubberized asphalt concrete. These changes are necessary
as the condition of the existing asphalt concrete pavement and supporting base was found to be
significantly worse than anticipated during the design phase. The additional work costs exceed the
project contingency fund. Approval of the project contingency fund increase is required to
compensate the contractor for the additional work.
1V. Consequences of Negative Action:
The lack of approval would prevent successful completion of this contract and preclude payment to
the contractor for required work.
