SUBJECT: APPROVE and AUTHORIZE the Employment & Human Services Department Director, or
designee, to EXECUTE a non-financial agreement with the Contra Costa Community College
District for Department staff training for the period October 1, 2002 through September 30,
2005.

RECOMMENDED ACTION:
APPROVE and AUTHORIZE the Employment & Human Services Department Director, or designee, to
EXECUTE a non-financial agreement with the Contra Costa Community College District for Department staff
training for the period October 1, 2002 through September 30, 2005.
FINANCIAL IMPACT:
This is a non-financial agreement setting forth the framework for providing Department staff training. This
master agreement establishes course format and the responsibilities of the parties. Separate financial
agreements will be entered into for the payment of training.
BACKGROUND:
For the last two years representatives of EHSD Staff Development and the Contra Costa Community College
District, Los Medanos College have researched methodologies of providing Department staff training. The
result is a master agreement providing the framework for staff training either at District campus' or through
County offices using a variety of instructors. The master agreement's flexible framework will serve as the
basis for the offering of courses for Department employees and contractors to meet Department needs without
the requirement of negotiating each course. Separate financial agreements will be entered into for the
payment of training, but course format and the responsibilities of the parties will be set in this master
agreement.

RECOMMENDATIONN:
Approve and authorize the Health Services Director, Or his designee (Art Lathrop) to execute on
behalf of the County, Ambulance Service Agreement #23-055-17 with San Ramon Fire Protection
District, an independent Fire Protection District, to provide emergency ambulance services in
County's Emergency Response Area 4,for the period from 12:01 a.m. on November 3, 2002 through
12:00 midnight on November 2,2007.
FISCAL IMPACT:
There is no County General Fund impact.
BACKGROUND/REASONS)FOR RECOMMENDATION(S):
On December 2, 1997, the Board of Supervisors approved Ambulance Service Agreement
#23-055-14 (as amended by Administrative Amendment Agreements #23-055-15 and #23-055-16)
with San Ramon Fire Protection District, for the period from 12:01 a.m. on November 3, 1997
through 12:00 midnight on November 2, 2002 for the provision of emergency ambulance services in
County's Emergency Response Area 4.
The Contractor has requested a reciprocal indemnification clause, as a matter of equity, to cover
reimbursement for damages and injuries arising from the County's negligence. In the view of
County Risk Management this indemnification will create a minimal potential loss, The Health
Services Department desires to expedite approval of the Agreement as written.
Approval of Ambulance Service Agreement #23-055-17 will allow the Fire District to continue
providing Emergency Ambulance Service in the County's Emergency Response Area 4 through
November 2, 2007,unless sooner terminated as specified in the Agreement.

Health System (dba John Muir Medical Center)

RECOMMENDED ACTION:
Approve and authorize the Health Services Director to execute on behalf
of the County, Hospital Agreement #23-076-7 with John Muir/ Mt. Diablo
Health System (dba John Muir Medical Center) , for the period from
November 1, 2002 through October _31, 2005, for the County' s designation
of John Muir Medical Center as a Base Hospital for County' s Emergency
Medical Services (EMS) .
FISCAL IMPACT:
This is a non-financial agreement . There is no County General Fund
impact .
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
Base Hospitals provide on-line and retrospective medical direction and
control to paramedic units and are required as a part of an EMS system
providing Advanced Life Support . Base Hospitals provide medical
direction without regard to patient destination, and therefore, provide
services for patients who may be transported to facilities other than
John Muir Medical Center. This agreement designates John Muir Medical
Center as the EMS Base Hospital for Contra Costa County and sets
standards and requirements for base hospital personnel and base hospital
activities.
Approval of Hospital Agreement #23-076-7 will allow John Muir Medical
Center to be the designated Base Hospital for Contra Costa County,
through October 31, 2005 .

CONTINCJE with the emergency action originally taken by the Board on.December 14, 1999 based on
the increasing/epidemic number of AIDS/I-IlV and Hepatitis C cases among injection drug users in
Contra Costa County.
Background:
Continued.Need for Emergency
The spread of disease, particularly HIV and Hepatitis C, through the sharing of needles continues to be
a grave concern in Contra Costa County. 30% of all people living with AIDS in Centra Costa were
infected by injection drug use with the highest number of AIDS cases in West County. As of December
1997, Richmond had the highest cumulative incidence of AIDS (4.8 per 1000 population) among
Contra Costa cities, with at least 50 cases. Injection drug use was the estimated mode of infection
among 40% of all individuals diagnosed with AIDS in Richmond. Among African Americans diagnosed
with AIDS in Richmond 1982-1998 53%were most likely infected through injection drug use.
Contra Costa Health Services does not currently provide needle exchange services(it is prohibited from
doing so absent a declared public health emergency). CCHS does provide services focused on injection
drug users including: I-HIVIAIDS Outreach and Education, 2-11IV antibody testing, 3-Substance
Abuse Treatment, 4-Health on Wheels Van, and 5-Substance Abuse Counseling and Treatment for
HIV-infected clients.

.Emergency Declaration for Support of Needle.Exchange Programs
Exchange 'Works, a community-based needle exchange service currently in operation in Contra Costa
currently distributes clean needles on a one-to-one basis. Their volunteer staff currently exchange 5,000
needles per week at two sites. There are no established needle exchange program in East or Central
County.
PROGRESS SINCE EMERGENCY ON DECEMBER 14, 1999
At the Board of Supervisors December 14, 1999 meeting, CCHS was directed to take four actions. As
of January 7, 2000 the following has been completed:
• CCHS has entered into discussions with Exchange Works in West County.
• Supplies for the ongoing provision of the services have been identified.
• CCHS staff have been identified to assist with the provision of information and referrals.
• Documentation forms are being reviewed and revised as necessary to ensure that specific
information is available to the Board.
• Planning for an East County process will begin later this quarter after the preliminary process in
West County is completed.
