CHILDREN'S IMPACT STATEMENT: Services under this contract directly support three community outcomes identified in the Contra Costa Children's Report Card: (3)"Families that are Economically Self-Sufficient"; (4) "Families that are Safe Stable and Nurturing"; and, (5) "Communities that are Safe and Provide a High Quality of Life for Children and Families"by assisting program participants to secure and maintain non-subsidized employment. BACKGROUND: Lao Family Community Development provides pre- and post-employment services to Ca1WORKs refugees residing in West Contra Costa County. Extension of the contract term will coincide with the Fiscal Year funding cycle and ensure that Limited English Proficient CalWorks and low income recipients/refugees of West Contra Costa will continue to receive supportive services. This Contract includes a six-month automatic extension through December 31, 2003, in an amount not to exceed $564, 053 . FISCAL IMPACT: This Contract is 1005 funded by Mental Health Realignment funds as a result of savings generated from the County' s reduction of client placements at State Hospitals . BACKGROUND/REASON(S) FOR RECOMENDATION(S)_ This Contract is part of the Department ' s cost saving plan to reduce the number of high-cost State Hospital beds by developing alternative placements . On December 18, 2001, Board of Supervisors approved Novation Contract ##24-680 -13 with Telecare Corporation for the period from July 1, 2001 through June 30, 2002 , which included a six-month automatic extension through December 31, 2002 , for subacute care for County residents who are severely and persistently mentally ill (SPMI) at Contractor' s Villa Fairmont Mental Health Center, Garfield Mental Health Center, Gladman Psychiatric Health Facility, and Morton Bakar Center. On January 22, 2002, the Board of Supervisors approved Contract #22-033-42 with Family and Community Services, Inc. (dba Senior Outreach Services of Contra Costa) for the period from November 1,2001 through June 30, 2002, which included a four-month automatic extension through October 31,2002,for the provision of congregate meal services for the Senior Nutrition Program. FISCAL IMPACT: This Contract is funded 100%by Mental Health Realignment,offset by third party billing. BACI GROMMEASON(S)FOR RECOMMENDATION(S): Effective July 1, 1992, State Mental Health Realignment Legislation shifted responsibility for payment to providers from the State to the Counties and required Counties to assume direct responsibility for contracting with sub-acute skilled nursing facilities. For a number of years, Medical Hill Rehabilitation Center has provided services to Contra Costa County's clients through an Inter-County agreement with Alameda County. Alameda County paid the Contractor for the services provided, and Contra Costa County reimbursed Alameda County. Due to a rate change that was effective on March 1, 2002, Alameda County has notified the County that they were no longer able to provide services for Contra Costa County's clients at this facility. Under Contract #74-183, Medical Hill Rehabilitation Center will continue to provide subacute skilled nursing care for County's Seriously and Persistently Mentally Ill (SPMI) and Neurobehavioral clients through February 28, 2003. These services were provided through Alameda County by two contract agencies -- Medical Dill Rehabilitation Center and Telecare Corporation. Contra Costa clients are placed at the facilities as appropriate,and Alameda County bills Contra Costa at the end of each fiscal year based upon bed usage. In February of this year,Medical Hill raised the daily bed rate from $101.00 to $160.00. At that time, Contra Costa Mental Health began negotiating with Crestwood, Inc., to move clients placed at Medical Hill to Crestwood Idlewood, at the daily bed rate of $106.00. All Contra Costa clients were subsequently moved from Medical Dill,with the exception of one client This individual is very medically as well as psychiatrically compromised, requiring dialysis three times daily. Crestwood Idlewood was unable to accommodate this client, so we elected to have her remain at Medical Hill, rather than move her to a more expensive State Hospital, and away from her family,who live near the facility. Dater in February 2002, we received notice from Medical Hill that they were raising the daily rate for our one remaining client from $160 to $350. It was our intention to continue to pay for the client's care through our contract with Alameda; however, in May we were informed by Alameda County that the contract does not allow for payment at the higher rate. At that time,we contacted Health Services Finance to discuss initiating a contract directly with Medical. Mill so we could reimburse them for care of our client retroactively to March 2002. We were informed that there was a mechanism through which a one-time only payment could be made to Medical Hill for reimbursement of services. In May 2002 we submitted a purchase order request for payment. To date, we owe Medical Hill approximately$74,900 for care of this client. The only mechanism for payment to Medical Hill for our client's care is to execute this contract retroactively to March 2002. BENEFIT TO CONTRA COSTA COUNTY: Approval of this retroactive contract will allow the Division to reimburse Medical Hill Rehabilitation Center for services provided and continue to provide the necessary level of care for the resident of Contra.Costa who is placed there.
