RECOMMENDED ACTION:
APPROVE and AUTHORIZE the Employment and Human Services, or designee, to AMEND contract(#21-
513-4)with Lao Family Community Development, Inc.,to provide employment services to Ca1WORKs and low
income refugees residing in West Contra Costa County, to increase the payment limit by$46,432 from $92,863
to a new payment limit of $139,295, AND to extend term from December 31, 2002 to June 30, 2003.
FINANCIAL IMI!AC'T`':
$46,432: 49% TANF Incentive Funds (100%Federal); 51% CalWorks funds (85% Federal, 15% County).
CHILDREN'S IMPACT STATEMENT:
Services under this contract directly support three community outcomes identified in the Contra Costa
Children's Report Card: (3)"Families that are Economically Self-Sufficient"; (4) "Families that are Safe Stable
and Nurturing"; and, (5) "Communities that are Safe and Provide a High Quality of Life for Children and
Families"by assisting program participants to secure and maintain non-subsidized employment.
BACKGROUND:
Lao Family Community Development provides pre- and post-employment services to Ca1WORKs refugees
residing in West Contra Costa County. Extension of the contract term will coincide with the Fiscal Year
funding cycle and ensure that Limited English Proficient CalWorks and low income recipients/refugees of
West Contra Costa will continue to receive supportive services.

RECOMMEN IATION(S) :
Approve and authorize the Health Services Director or his designee
(Donna Wigand) to execute on behalf of the County, Novation Contract
##24-680-14 with Telecare Corporation, in an amount not to exceed
$1, 128, 101, to provide subacute care to severely and persistently
mentally ill (SPMI) clients, for the period from July 1, 2002
through June 30, 2003 . This Contract includes a six-month automatic
extension through December 31, 2003, in an amount not to exceed
$564, 053 .
FISCAL IMPACT:
This Contract is 1005 funded by Mental Health Realignment funds as
a result of savings generated from the County' s reduction of client
placements at State Hospitals .
BACKGROUND/REASON(S) FOR RECOMENDATION(S)_
This Contract is part of the Department ' s cost saving plan to reduce
the number of high-cost State Hospital beds by developing
alternative placements .
On December 18, 2001, Board of Supervisors approved Novation
Contract ##24-680 -13 with Telecare Corporation for the period from
July 1, 2001 through June 30, 2002 , which included a six-month
automatic extension through December 31, 2002 , for subacute care for
County residents who are severely and persistently mentally ill
(SPMI) at Contractor' s Villa Fairmont Mental Health Center, Garfield
Mental Health Center, Gladman Psychiatric Health Facility, and
Morton Bakar Center.
Approval of Novation Contract ##24-680-14 will replace the six-month
automatic extension under the prior Agreement and will allow the
Contractor to continue providing services through June 30, 2003 .

Approve and authorize the Health Services Director, or his designee (Wendel Brunner, M.D.) to
execute on behalf of the County, Novation Contract #22-033-43 with Family and Community
Services,Inc. (dba Senior Outreach Services of Contra Costa), a non-profit corporation,in an amount
not to exceed$206,059,to provide congregate meal services for the Senior Nutrition Program for the
period from July 1, 2002 through June 30, 2003. This Contract includes a four-month automatic
extension through October 31, 2003,in an amount not to exceed$68,618.
FISCAL IMWACT:
This contract is 100%federally funded under Title III-C(1)of the Older Americans Act of 1965. No
County funding is required.
BACKGROLTNDIREASON(S)FOR RECOMMENDATION(S):
This Contract meets the social needs of County's population in that it provides meals on 250 serving
days, at seven sites throughout the County, to an average of 225 senior citizens, to ensure they
receive at least 1l3 of their daily nutritional requirements.
On January 22, 2002, the Board of Supervisors approved Contract #22-033-42 with Family and
Community Services, Inc. (dba Senior Outreach Services of Contra Costa) for the period from
November 1,2001 through June 30, 2002, which included a four-month automatic extension through
October 31,2002,for the provision of congregate meal services for the Senior Nutrition Program.
Approval of Novation Contract #22-033-43 will replace the four-month automatic extension under
the prior agreement and allow the Contractor to continue providing services through June 30,2003.

authorizing the Health Services Department to contract wi'th the State
Department of Mental Health to assume responsibility for Medi-Cal mental
health specialty services. Responsibility for outpatient mental health
specialty services involves contracts with individual, group and
organizational providers to deliver these services.
On November F, 2001, the Board of Supervisors approved Contract #24-939-
48 (4). with Jewish Family and Children' s Services of the East Bay, for the
period from July 1, 2001 through June 30, 2002, for Medi-Cal mental
health specialty services.
Approval of Contract #24-939-48 (4) will allow the Contractor to continue
providing services, through June 30, 2003.

For several years the County has contracted with registries to
provide temporary licensed medical personnel to assist the Hospital
and Health Centers during peak loads, temporary absences and
emergency situations.
On December 18, 2001, the Board of Supervisors approved Contract
#26-337-3 (as amended by Contract Amendment Agreement #26-337-5 and
#26-337-6) , with Lighthouse Staffing, Inc. , for the period from
December 1, 2001 through November 30, 2002, for the provision of
temporary Medical Social Workers, to ensure appropriate care for
patients at Contra Costa Regional Medical Center and Contra Costa
Health Centers.
Approval of Contract #26-337-7 will allow the Contractor to provide
additional temporary registry services, through November 30, 2003 .

FISCAL, IMPACT:
Cost to the County depends upon utilization. As appropriate,
patients and/or third party payors will be billed for services.
BACKGROUND/,RZASON(S) FOR RECOMMENDATIONS:
For a number of years the County has contracted with Medical and
Dental Specialists to provide specialized professional services
which are not otherwise available in its hospital and clinics .
On October 5, 1999, the Board of Supervisors approved Contract #26-
970 (as. amended by subsequent Amendments #26-9702 and #26-970-2) ,
with John T. Burton, M.D. , for the period from November 1, 1999,
through October 31, 2002, to provide professional Orthopedic Surgery
services, including, but not limited to clinical coverage,
consultation., training, on-call, medical and/or surgical procedures,
Plastic Hand Surgery services while on--site at Contra Costa Regional
Medical Center, and to act on behalf of the County as the Section
Chief .
Approval of Contract #26-970-3 , will allow Dr. Burton to continue
providing services, through October 31, 2005.

In December 1995, the Purchasing Services Manager executed Contract #23-197 with Mediware
Information Systems, Inc., for the period from December 1, 1995 through November 30, 2002, for
the provision of"Surgiware"software services and license.
Approval of Contract #23-197-1 will allow the Contractor to install and license Contractor's
Operating Room Management Information System and maintain the system through November 30,
2003.

RECONIMENDATION(S):
Approve and authorize the Health Services Director, or his designee (Donna Wigand) to execute on
behalf of the County, Contract #74-183 with Medical Hill Rehabilitation Center, a non-profit
corporation, in an amount not to exceed $127,750, to provide subacute skilled nursing care for
County's Seriously and Persistently Mentally Ill (SPMI) and Neurobehavioral clients, for the period
from March 1,2002 through February 28, 2003.
FISCAL IMPACT:
This Contract is funded 100%by Mental Health Realignment,offset by third party billing.
BACI GROMMEASON(S)FOR RECOMMENDATION(S):
Effective July 1, 1992, State Mental Health Realignment Legislation shifted responsibility for
payment to providers from the State to the Counties and required Counties to assume direct
responsibility for contracting with sub-acute skilled nursing facilities.
For a number of years, Medical Hill Rehabilitation Center has provided services to Contra Costa
County's clients through an Inter-County agreement with Alameda County. Alameda County paid
the Contractor for the services provided, and Contra Costa County reimbursed Alameda County.
Due to a rate change that was effective on March 1, 2002, Alameda County has notified the County
that they were no longer able to provide services for Contra Costa County's clients at this facility.
Under Contract #74-183, Medical Hill Rehabilitation Center will continue to provide subacute
skilled nursing care for County's Seriously and Persistently Mentally Ill (SPMI) and
Neurobehavioral clients through February 28, 2003.

SUBJECT: Request for Retroactive Contract-Medical Hill Rehabilitation Center
STATEMENT OF NEED FOR RETROACTIVE CONTRACT:
For several years, Contra Costa Mental Health has been one of several Bay Area counties who
entered into an inter-county services contract with Alameda County to provide skilled nursing
services for OBS (organic brain syndrone), traumatic brain injured and medically debilitated
patients. These services were provided through Alameda County by two contract agencies --
Medical Dill Rehabilitation Center and Telecare Corporation. Contra Costa clients are placed at
the facilities as appropriate,and Alameda County bills Contra Costa at the end of each fiscal year
based upon bed usage.
In February of this year,Medical Hill raised the daily bed rate from $101.00 to $160.00. At that
time, Contra Costa Mental Health began negotiating with Crestwood, Inc., to move clients
placed at Medical Hill to Crestwood Idlewood, at the daily bed rate of $106.00. All Contra
Costa clients were subsequently moved from Medical Dill,with the exception of one client This
individual is very medically as well as psychiatrically compromised, requiring dialysis three
times daily. Crestwood Idlewood was unable to accommodate this client, so we elected to have
her remain at Medical Hill, rather than move her to a more expensive State Hospital, and away
from her family,who live near the facility.
Dater in February 2002, we received notice from Medical Hill that they were raising the daily
rate for our one remaining client from $160 to $350. It was our intention to continue to pay for
the client's care through our contract with Alameda; however, in May we were informed by
Alameda County that the contract does not allow for payment at the higher rate. At that time,we
contacted Health Services Finance to discuss initiating a contract directly with Medical. Mill so
we could reimburse them for care of our client retroactively to March 2002. We were informed
that there was a mechanism through which a one-time only payment could be made to Medical
Hill for reimbursement of services. In May 2002 we submitted a purchase order request for
payment.

In September 2002 we were notified by Purchasing that payment could not be made as the total
amount outstanding was over$25,000. To date, we owe Medical Hill approximately$74,900 for
care of this client. The only mechanism for payment to Medical Hill for our client's care is to
execute this contract retroactively to March 2002.
BENEFIT TO CONTRA COSTA COUNTY:
Approval of this retroactive contract will allow the Division to reimburse Medical Hill
Rehabilitation Center for services provided and continue to provide the necessary level of care
for the resident of Contra.Costa who is placed there.
AUTHORIZATION OF SERVICES:
These services were requested/authorized by the Adult Program Chief, who is granted this
authority by the Mental Health Director, pursuant to the California Welfare and Institutions
Code,under the Bronzon-McCorquadale Act,WI Code, Section 5608.
