(over) Contra Costa County Claim Form, section 3 On 3;11/02 atihe Superior Courthouse in Martinez,CA.My cellular phone was stolen as I was going through security at the front entrance. As a prospective juror,I entered the Martinez Superior Courthouse on 317 and 3!8/12 and each time I entered(4 times total),I was told to place my cell phone with my other personal items(keys, wallet)while I passed through the metal detector. On 3111102,at approximately,9:50 AM, it was a different story,as I placed my cell phone with other personal items,I was instructed to pass my cell phone through the x-ray machine. I placed my cell phone on the x-rav belt, and walked through the metal detector. I lost sight of my phone at that time, and after I had gone through the metal detector and searched for my phone,it was gone. I reported it to the deputy sheriffs at the time and they informed me that it was probably taken by mistake and it will probably show up later and that this had happened multiple times before. I called my number from another phone a couple of times in the next 5 minutes, but it was not turned on. I checked back during the recess, lunchbreak and at the end of the day,but nothing showed up. / A i 00- o 0 o0 • C3 4 gD UA Cs G V L IW JAW tlP _ut t ltd h C 1 n.9 u I a r9µ WIRELESS Page 1 of 11 Statement Cate: 4103102 - 5102102 Account Number: 136283053 AMY SABA 602 HUNTLEIGH DR LAFAYETTE CA 94549.5518 Previous Payment Ad ustments Past Due Current DUE Balance Received to revious Balance Charges ;.U", on 4/23102 Balance RECEIPT 74.85 74.85CR N/A N/A 94.30 $94.30 ACCOUNT SUMMARY Previous Balance 74.85 Payment Received on 4/23102.
