Contra Costa County will be paid a minimum monthly payment of $1, 800 . FISCAL IMPACT: For a .fee the Contra Costa Health Plan will provide Advice Nurse Services to County of Sonoma. The revenue generated by this Contract will be used to offset the cost of Contra Costa Health Plan (Health Plan) Advice Nurse services . BACKGROUNDIREASON(S) FOR RECOMMENDATION(S) : On September 18, 2001, the Berard of Supervisors approved Contract #29-773- 3, for the period from September 1, 2001 through August 31, 2002 , for provision of Advice Nurse services . FISCAL IMPACT: Approval of this Agreement will result in a maximum reimbursable amount of $7, 257 from the State for the Summer Food Service Program. No County funds are required. REASONS FOR RECOMMENDATION/BACKGROUND: The State Department of Education operates summer feeding sites throughout the State of California to assist families who are working while their children are not in school . However, the State does not have enough staff to perform the food service safety evaluations that are needed at each site, and have traditionally relied on the County environmental health staff to provide these services . Approval of Standard Agreement #28600-5 will allow the Department to continue these services through September 30, 2002 . Three certified and sealed copies of this Board Order should be returned to the Contracts and Grants Unit . Approve and authorize submission of Funding Application #29-455-12 to the State Department of Mental Health in the amount of $100,821, for FY 2002- 20103, to continue PATH Formula Grant Funding under the Stewart B. McKinney Homeless Assistance Amendments Acts of 1590 (PL 1.01-645) . B. Authorize the Health Services Director, or his designee (Donna Wigand) to sign required assurances and certifications regarding debarment and suspension, a drug-free workplace, lobbying, and compliance with Federal requirements, and to make any minor technical adjustments to the Grant Application that may be required by the State Department of Mental Health. FISCAL IMPACT: Approval by the State Department of Mental Health of this application will result in $1.00, 821 of Federal PATH Grant Funds for the continuation of outreach services to the homeless mentally disabled. No County funds are required. BACKGp,o=D REASON(S)L FOR RECOMMENDATION(S) : On July 17, 2001, the Board of Supervisors approved the Federal PATH Grant Application #29-455-11 for Fiscal Year 2001-2002. Approval of this application #29-455--12 will continue the PATH project operated by Phoenix Program, Inc. , under County's Contract #24-385, for the provision of mental health outreach services for the Homeless Mentally Ill (two Multi-Services Centers and one 20- bed Shelter/Multi-Services Center) in San Pablo, Concord and Antioch. The Federal PATH Grant Application for 2002-2003 was reviewed and recommended for approval by the County's Mental Health Commission on July 8, 2002. In order to meet the deadline for submission, the application has been forwarded to the State, but subject to Board approval. Three certified copies of the Board Order authorizing submission of the application should be returned to the Contracts and Grants Unit for submission to the State. REASONS FOR RECOMMENDATIONS/BACKGROUND: This project joins Contra Costa County Health Services Department and the City of Richmond, in a cooperative effort to prevent and reduce the probability of accidental release of regulated substances and to increase participation by the community to improve accident preven- tion. Under this Agreement the County will administer and. enforce Chapter 6 .43 of the City' s Ordinance No. 42-01, which imposes regula- tions concerning hazardous materials management . FISCAL IMPACT: Approval of this Agreement will result in a total payment to Contra Costa County of $26, 340 . No County Funds are required. BACKGROUNDIREASONS FOR RECOMMENDATION(S): Contra Costa County, in collaboration with Mount Diablo Unified School District and Tehama County has been working with Families First - Solara Day Treatment Program to provide seriously emotionally disturbed children with the services and the support they need to function effectively in school, at home, and in the community, and to avoid out-of-home placements and hospitalizations . Families First staff will work closely with County and the School District personnel in developing program services and policies . On February 12, 2002 , the Board of Supervisors approved Agreement ##29-523 with Tehama County for the period from August 1, 2001 through June 30, 2002, for the provision of intensive day treatment services for seriously emotionally disturbed and at-risk children.
